A Study on the Influence of Patient Participation on Patient Trust-Based on Sample Survey in China.
Frontiers in psychology, 2018Co-Authors: Tongwei YangAbstract:Trust plays a central role in a doctor - patient relationship, and patient's trust in doctors is the most important factor in trust relationship between the doctor and the patient. By reviewing the existing studies, this study proposes that patient participation is an Antecedent Variable influencing patient trust, and the patient can take the initiative to participate in diagnostic activities with the aim of establishing and maintaining the psychological contract with the doctor, thereby strengthening doctor-patient trust. In this study, we propose the model on the relationship between the patient participation, the patient's psychological contract and the patient trust, and then conduct an empirical study. The research findings indicate that patient participation is an Antecedent Variable for the patient's psychological contract and patient trust; the patient's psychological contract is a mediator Variable between the patient participation and the patient's trust in doctor; patient participation can be categorized as three dimensions: information search, responsible behavior, and interpersonal relationship. Thus, we should encourage patient participation, advocate the shared decision-making, and promote the synergic improvement of interpersonal trust and universal trust between doctors and patients.

Methods A cross-sectional survey of current patient portals users ( N = 333) was conducted online. Participants completed the Visual Aesthetics of Website Inventory, along with items measuring perceived ease of use (PEU), perceived usefulness (PU), and behavioral intentions (BIs) to use the patient portal. Results The hypothesized model accounted for 29% of the variance in BIs to use the portal, 46% of the variance in the PU of the portal, and 29% of the variance in the portal’s PEU. Additionally, one dimension of the aesthetic evaluations functions as a predictor in the model – simplicity evaluations had a significant positive effect on PEU. Conclusion This study provides evidence that aesthetic evaluations – specifically regarding simplicity – function as a significant Antecedent Variable to patients’ use of patient portals and should influence patient portal design strategies.

Does information overload prevent chronic patients from reading self-management educational materials?

However, various kinds of self-management educational materials which government or healthcare institutions provide for patients may not achieve the expected outcome. One of the critical reasons affecting patients’ use intention could be patients’ perceived information overload regarding the self-management educational materials. Purpose This study proposed an extended model of the Theory of Planned Behavior (TPB), which incorporated perceived information overload, to explore if information overload will prevent chronic patients from reading educational materials for self-care management. The independent Variables are attitude, subject norm, perceived behavior control and perceived information overload while the dependent Variable is behavior intention to use the self-management educational materials. Perceived information overload is also referred to as an Antecedent Variable which may has impacts on attitude and perceived behavior control. Methods The cross-sectional study interviewed newly diagnosed chronic patients with coronary artery disease, who are the potential users of the self-management educational materials, in a medical center in Taiwan. Data were analyzed using descriptive statistics of the basic information distribution of the respondents, and structural equation modeling to study the reliability and validity for testing hypotheses. Results A total of 110 respondents were enrolled in this study and successful interview data were collected from 106 respondents. The result indicates that the patients’ perceived information overload of self-management educational materials was validated to have impacts on attitude and perceived behavioral control constructs of the TPB as well as contributing a direct impact on patients’ intentions to use self-management educational materials. Besides, subjective norm and perceived behavioral control constructs were validated to have significant impacts on behavioral intentions, whereas the attitude construct was not. Finally, the relationships between information overload and attitude, information overload and intention, subjective norm and intention, as well as perceived behavioral control and intention varied significantly between highly- and less-educated respondents. Differing self-management educational materials for respondents of various educational levels could be formulated to substantially boost the use of self-management educational materials. Conclusions This study demonstrated a comprehensive framework, which extended perceived information overload into the TPB model, to predict patients’ behavioral intention of using self-management educational materials. We expect the results of this study will provide useful insights for studying self-management educational materials usage and information overload from the perspectives of academia, governments, and healthcare providers.

Exploring critical factors influencing physicians’ acceptance of mobile electronic medical records based on the dual-factor model: a validation in Taiwan

However, such acceptance is always influenced by both positive and negative factors and it is necessary to validate both in order to get a complete understanding. This study aims to explore physicians’ acceptance of mobile electronic medical records based on the dual-factor model, which is comprised of inhibitors and enablers, to explain an individual’s technology usage. Following an earlier healthcare study in the USA, the researchers conducted a similar survey for an Eastern country (Taiwan) to validate whether perceived threat to professional autonomy acts as a critical inhibitor. In addition, perceived mobility, which is regarded as a critical feature of mobile services, was also evaluated as a common Antecedent Variable in the model. Methods Physicians from three branch hospitals of a medical group were invited to participate and complete questionnaires. Partial least squares, a structural equation modeling technique, was used to evaluate the proposed model for explanatory power and hypotheses testing. Results 158 valid questionnaires were collected, yielding a response rate of 33.40%. As expected, the inhibitor of perceived threat has a significant impact on the physicians’ perceptions of usefulness as well as their intention to use. The enablers of perceived ease of use and perceived usefulness were also significant. In addition, as expected, perceived mobility was confirmed to have a significant impact on perceived ease of use, perceived usefulness and perceived threat. Conclusions It was confirmed that the dual-factor model is a comprehensive method for exploring the acceptance of healthcare information technologies, both in Western and Eastern countries. Furthermore, perceived mobility was proven to be an effective Antecedent Variable in the model. The researchers believe that the results of this study will contribute to the research on the acceptance of healthcare information technologies, particularly with regards to mobile electronic medical records, based on the dual-factor viewpoints of academia and practice.
