    Aortic Valve Stenosis is one of the most common valvular heart disorders and the prevalence will rise as the population ages. Once symptomatic patients with Aortic Valve Stenosis tend to fare worse with high mortality rates. Aortic Valve replacement is indicated in these patients and besides the standard surgical replacement, a less invasive approach, transcatheter Aortic Valve implantation, has gained momentum and has showed promising and solid results in patients with high surgical risk. An important aspect of evaluating patients with Aortic Valve Stenosis is the ability to choose the best possible candidate for the procedure. In addition, predicting the short and long-term clinical outcomes after the Valve replacement could offer the treating physicians a better insight and provide information for optimal therapy. Biomarkers are biological parameters that can be objectively measured and evaluated as indicators of normal biological processes and are easily monitored. The aim of this review is to critically assess some of the most widely used biomarkers at present (natriuretic peptides, troponins, C-reactive protein) and provide an insight in novel biomarkers that are currently being investigated (galectin-3, growth differentiation factor-15, microRNAs) for possible diagnostic and prognostic use in Aortic Valve Stenosis and transcatheter Aortic Valve implantation respectively.

    Abstract:

    Aortic Valve Stenosis is becoming a burden to society due to the constantly aging population. After the onset of the first symptoms it usually follows an ominous route with high mortality levels even at two years without any medical intervention. The gold standard for treating Aortic Valve Stenosis is surgical replacement; nonetheless, 30% of patients are left untreated due to increased risk of surgery. A less invasive method has been developed in the past decade, transcatheter Aortic Valve implantation (TAVI), which allows implantation of prosthesis in the native stenosed Aortic Valve position, using a catheter that enters the body in a transvascular way or through a minimal surgical cut down. In this review, we briefly discuss where the TAVI field begun, focus on the recent Valves that are being used and finally look at what lies ahead. The review of the patents will assist in the understanding of how this field evolved, how it became established and what is to be expected in the future.

  • Pressure recovery in Aortic Valve Stenosis.

    Pressure recovery is the variable increase in lateral pressure downstream from a stenotic orifice. The magnitude and clinical significance of pressure recovery in Aortic Valve Stenosis are poorly defined. We obtained high-fidelity pressure and velocity recordings in 11 patients with isolated significant Aortic Valve Stenosis at the time of diagnostic cardiac catheterization. Systematic catheter pullback from the left ventricular cavity revealed a consistent although variable subvalvular gradient. Further pullback across and distal to the region of the stenosed Aortic Valve revealed a consistent and progressive increase in the ascending Aortic pressure. This increase in lateral pressure occurred pari passu with a diminution in amplitude of the velocity pulse. The extent of pressure recovery was directly related to systemic blood flow and transvalvular flow but inversely related to the Gorlin-derived Aortic Valve area. These findings have potentially important implications for the hemodynamic evaluation of mild to moderately severe Aortic Valve Stenosis. The extent of pressure recovery may be of additional utility in the assessment of Aortic Valve Stenosis under varying physiological states.

    Aortic Valve replacement via open heart surgery, although still the standard treatment for severe symptomatic Aortic Valve Stenosis, is not an option for many patients with severe symptoms, and these patients are often left with suboptimal strategies such as medical management or balloon valvuloplasty. But over the last 5 years improvements in percutaneous approaches to implantation of prosthetic Aortic Valves have made it a potential therapeutic option for these patients. Technical and device issues are being refined, and percutaneous Aortic Valve replacement is showing promise in ongoing clinical trials.
