    Abstract:

    Atopic Dermatitis is a common chronic pruritic inflammatory skin disorder, characterized by an abnormal skin barrier, immune dysfunction, and an altered skin microbiome. Atopic Dermatitis may be seen in conjunction with a variety of other skin disorders due to the complex pathogenesis of Atopic Dermatitis, involving genetic and environmental factors that are associated with immune dysfunction, barrier defects, and altered skin microbiomes. Skin disorders associated with Atopic Dermatitis include diseases sharing similar genetic origins like ichthyosis vulgaris, infectious diseases such as impetigo, and eczema herpeticum, in addition to the cutaneous autoimmune diseases, alopecia areata, and vitiligo. Atopic Dermatitis is also often linked to such benign conditions as pityriasis alba and keratosis pilaris. This review discusses the cutaneous comorbidities of Atopic Dermatitis and their relationship via their occurrence in conjunction with Atopic Dermatitis.

    Atopic Dermatitis is a complex, systemic inflammatory disorder associated with a variety of clinical features. The original criteria of Hanifin and Rajka include major criteria and a list of about two dozen minor criteria however, even the minor criteria do not include some features of Atopic Dermatitis noted less commonly but still seen with some frequency. This contribution first reviews the common clinical appearance of Atopic Dermatitis in infancy, childhood, and adulthood, as well as the less typical appearances, including lichenoid Atopic Dermatitis; juvenile plantar dermatosis; nummular-type Atopic Dermatitis; follicular Atopic Dermatitis; alopecia of Atopic Dermatitis; eczema coxsackium; and psoriasiform, perineal, and lip licker's Dermatitis. The clinician will be able to recognize and treat rarer forms of Atopic Dermatitis and incorporate this into their daily practice.

  • Adult-Onset Atopic Dermatitis: Characteristics and Management

    Recent epidemiological studies found the US prevalence of Atopic Dermatitis in adults to be approximately 7%. In particular, one in four adults with Atopic Dermatitis report adult onset of their disease. Adult-onset compared to child-onset Atopic Dermatitis is associated with distinct risk factors, lesional distribution and morphology, associated signs, genetics, and comorbidities. Adult-onset Atopic Dermatitis is a clinical diagnosis, and must be distinguished from other entities in the differential diagnosis, e.g., allergic contact Dermatitis and cutaneous T-cell lymphoma. Further research is necessary to better understand the pathogenesis and optimal treatment approaches in adult-onset/recurrent Atopic Dermatitis.

    Atopic Dermatitis is a chronic inflammatory disorder of the skin that has become very common in childhood. Similarly, obesity has increased in the USA and developed countries to epidemic proportions. Recent clinical and epidemiological studies have demonstrated an association between obesity and Atopic Dermatitis in childhood. Obesity is associated with increased prevalence and severity of, and healthcare utilization from, Atopic Dermatitis. Furthermore, obesity may be a modifiable risk factor for Atopic Dermatitis, such that weight loss may prevent or perhaps alleviate Atopic Dermatitis. Future studies are needed to determine whether weight loss in obese individuals improves Atopic Dermatitis.

Atopic Dermatitis develops as a result of a complex interrelationship of environmental, immunologic, genetic, and pharmacologic factors. Efforts to understand the relative contributions of these factors have led to research seeking to identify the relevant effector cells and mediators involved in the pathogenesis of Atopic Dermatitis. These factors include the pattern of local cytokine release, the differentiation of helper T cells, multiple roles of IgE, skin-directed cell responses, infectious agents, and superantigens. This article reviews these cellular and immunologic mechanisms underlying Atopic Dermatitis and discusses how an understanding of their role in the inflammatory process may lead to improved treatments for Atopic Dermatitis.

    Atopic Dermatitis is characterized by Staphylococcus aureus colonization and recurrent skin infections. In addition to an increased risk of invasive infections by herpes simplex or vaccinia viruses, there is ample evidence that microbial pathogens, particularly S aureus and fungi, contribute to the cutaneous inflammation of Atopic Dermatitis. The authors describe recent developments in the pathogenesis of Atopic Dermatitis in relation to the role of microbial pathogens. Understanding how microbial pathogens interact or evade the cutaneous immunity of Atopic Dermatitis may be crucial in preventing infections or cutaneous inflammation in this disease.

    Atopic Dermatitis is a chronic inflammatory skin disease associated with cutaneous hyperreactivity to environmental triggers and is often the first step in the Atopic march that results in asthma and allergic rhinitis. The clinical phenotype that characterizes Atopic Dermatitis is the product of interactions between susceptibility genes, the environment, defective skin barrier function, and immunologic responses. This review summarizes recent progress in our understanding of the pathophysiology of Atopic Dermatitis and the implications for new management strategies.

    Atopic Dermatitis is a common inflammatory skin disease with a complex pathogenesis that includes imbalanced immune system signalling, impaired skin barrier and enhanced Staphylococcus aureus skin colonization. The skin bacterial communities are characterized by increasing abundance of S. aureus, leading to reduced diversity compared with the bacterial communities on healthy skin, and increasing disease severity. In contrast, fungal communities are richer and more diverse on the skin of patients with Atopic Dermatitis, although distribution of the most common species is similar in patients and controls. Filaggrin deficiency in Atopic Dermatitis skin might be related to the enhanced skin colonization by S. aureus. In addition, S. aureus expressing variant virulence factors have been shown to elicit Atopic Dermatitis-like phenotypes in mice, indicating that specific S. aureus strains can induce flare-ups. This review aims to provide an overview of the recent literature on the skin microbiome in Atopic Dermatitis.
