    Abstract:

    To avoid tracheal wall damage or inadvertent falls of the endotracheal tube cuff pressure ( P cuff ) in intubated and mechanically-ventilated patients, the authors devised a simple procedure for Automatic and continuous Regulation of P cuff . The procedure, only requiring a simple aquarium air pump and conventional tubing, was first tested at the bench when applied to an intubated and ventilated lung model, including an artificial trachea with an externally-variable section. The clinical performance of the procedure was tested in eight intubated patients, in whom the endotracheal tube cuff was connected to the designed P cuff regulator during 24 h. The bench test showed that the procedure was able to maintain P cuff constant, regardless of the changes imposed in the tracheal section. It was also effective in maintaining P cuff during routine mechanical ventilation. Actual P cuff recorded over the 24-h period always coincided with the target value within ±2 cmH 2 O in all the patients. The procedure devised to maintain endotracheal tube cuff pressure is readily implemented, cheap, easy to operate and can be used regardless of the specific ventilator or tube used. Routine implementation of this procedure may be useful for protecting the trachea from tissue damage and for reducing the risk of ventilator-associated pneumonia.

    Abstract:

    An infinitely variable transmission (IVT) is a continuously variable transmission with an infinite ratio range, which means that the transmission ratio may be zero. IVTs are generally made up of three elements: a continuously variable transmission (CVT), a planetary gear train and a fixed ratio mechanism. Automatically regulated CVTs offer the advantage of Automatically changing the transmission ratio without requiring sophisticated control systems. In this work the possibility of realizing an Automatically regulated IVT is appraised, i.e. with Automatic Regulation of the transmission ratio, in relation to operational conditions. The use of the Automatically regulated V-belt CVT, endowed with simple devices of axial force, is hypothesized. For the different types of devices the characteristic curves of operation, the power flows and the efficiency of the Automatically regulated IVT are determined.

    Abstract:

    Automatic Regulation of tidal volume (VT) maintains CO2 homeostasis when spontaneous respiratory rhythm is replaced with a cortically triggered rhythm. We examined whether Automatic Regulation of respiratory frequency (fR) could maintain CO2 homeostasis at rest if the VT is cortically designated in experiments performed in 21 conscious humans. First, volitionally controlled fR at levels lower than baseline resulted in a larger VT, maintaining end-tidal CO2 fraction constant at eupneic levels. However, when fR was volitionally controlled at levels higher than baseline, end-tidal CO2 fraction decreased unexpectedly. Next, when the VT was volitionally constrained but fr was freely chosen, end-tidal CO2 fraction decreased. The present study revealed some limitations in the control of CO2 homeostasis by Automatic Regulation of fR, probably because respiratory rhythm is susceptible to non-metabolic factors. This study also showed the importance of Automatic Regulation of VT in maintaining CO2 homeostasis at rest. Nevertheless, Automatic Regulation of VT was incomplete when fR was volitionally imposed at high levels.

    Abstract:

    We investigated the relationship between deep acting, Automatic Regulation and customer tips with 2 different study designs. The first study was a daily diary study using a sample of Dutch waiters and taxi-drivers and assessed the link of employees' daily self-reported levels of deep acting and Automatic Regulation with the amount of tips provided by customers (N = 166 measurement occasions nested in 34 persons). Whereas deep acting refers to deliberate attempts to modify felt emotions and involves conscious effort, Automatic Regulation refers to automated emotion regulatory processes that result in the natural experience of desired emotions and do not involve deliberate control and effort. Multilevel analyses revealed that both types of emotion Regulation were positively associated with customer tips. The second study was an experimental field study using a sample of German hairdressers (N = 41). Emotion Regulation in terms of both deep acting and Automatic Regulation was manipulated using a brief self-training intervention and daily instructions to use cognitive change and attentional deployment. Results revealed that participants in the intervention group received significantly more tips than participants in the control group.
