Factors influencing the incidence of middle ear Barotrauma in traditional divers are colds, smoking, frequency of diving and length of diving. The purpose of explaining the factors related to the occurrence of Barotrauma in the middle ear of traditional breath-resistant divers. The analytic observational research design with cross sectional approach is supported by in-depth interviews, by conducting interviews using questionnaires and otoscopy examinations of respondents to determine the events of middle ear Barotrauma. The study population was traditional divers, totaling 78 respondents. The dependent variable is the occurrence of Barotrauma in the middle ear of traditional divers, the independent variable with colds, smoking habits, frequency of diving and length of diving, data analysis using bivariate and multivariate. Results as many as 32 people (41.0%) of 78 respondents experienced middle ear Barotrauma. Bivariate analysis showed a correlation between the frequency of diving with the events of the middle ear barotrauama p = 0.012. Logistic regression test showed the significance value of the frequency of diving ≥ 4 days / week (p = 0.0106; PR = 5.310; 95% CI = 1.619-17.413). Conclusion of factors related to the incidence of middle ear Barotrauma in traditional divers is the frequency of diving hari 4 days / week, with a probability of 38.13%

Cedera paling banyak terjadi pada penyelaman di kedalaman dangkal yaitu 4,3-17,4 kaki (1,3–5,3 meter), dapat menyebabkan pecahnya gendang telinga. Tujuan : Menjelaskan pengaruh faktor host dan environment terhadap kejadian Barotrauma telinga tengah pada penyelam tradisional. Metode : Penelitian ini merupakan penelitian dengan desain cross sectional di tunjang dengan pendekatan kualitatif melalui indepth interview. Populasi studi adalah penyelam tradisional tahan nafas, berjumlah 78 reponden. Variabel dependen penelitian adalah kejadian Barotrauma telinga tengah penyelam tradisional dengan pemeriksaan otoskopi. Variabel independen meliputi faktor host dan environment. Analisis data menggunakan bivariat dan multivariat. Hasil : Hasil penelitian menunjukkan bahwa sebanyak 32 orang (41,0%) dari 78 penyelam tradisional yang diperiksa mengalami Barotrauma telinga tengah. Hasil uji regresi logistik menunjukkan variabel yang terbukti berpengaruh terhadap kejadian Barotrauma telinga tengah adalah tidak penyuluhan kesehatan (p = 0,009; PR = 3,920; 95% CI = 1,405 – 10.936), frekuensi penyelaman sering (p = 0,0106; PR = 5,310; 95% CI = 1,619 – 17,413) Kesimpulan : Faktor yang berpengaruh terhadap kejadian Barotrauma telinga tengah pada penyelam tradisional adalah tidak penyuluhan kesehatan, frekuensi penyelaman sering atau ≥ 4 hari/minggu, dengan probabilitas 70,72%. Airway Pressures and Early Barotrauma in Patients with Acute Lung Injury and Acute Respiratory Distress Syndrome

MatthayAbstract:The determinants of Barotrauma in mechanically ventilated patients with acute lung injury or acute respiratory distress syndrome (ALI/ARDS) have not been clearly established. Using data from ARDS Network randomized controlled trials, we retrospectively examined the association between airway pressures and the risk of early Barotrauma in a cohort of 718 patients with ALI/ARDS and no baseline Barotrauma. We studied airway pressures at three exposure intervals: baseline, one day preceding the Barotrauma event (one-day lag), and concurrent with the Barotrauma event. During the first four study days, the cumulative incidence of Barotrauma was 13% (95% confidence interval [CI] 10.6 to 15.6%). In a forward stepwise Cox proportional hazards analysis using time-dependent variables, higher concurrent positive end-expiratory pressure (PEEP) was associated with an increased risk of early Barotrauma (relative hazard [RH] 1.67 per 5-cm H2O increment; 95% CI 1.35–2.07). Paranasal sinus Barotrauma in sports self-contained underwater breathing apparatus divers
The Journal of laryngology and otology, 2008Co-Authors: Cem UzunAbstract:Objective: To evaluate potential pre-dive parameters in relation to paranasal sinus Barotrauma in sports self-contained underwater breathing apparatus divers. Study design: The pre-dive and follow-up records of 46 healthy sports self-contained underwater breathing apparatus divers were analysed. Methods: In the routine pre-dive examination of diving candidates with no symptoms of rhino-sinus disease, rhinoscopy, sinus radiography and nasal endoscopy indicated no signs of rhinitis and/or sinusitis. Any signs of paranasal sinus Barotrauma were noted, and several parameters were evaluated in relation to occurrence of Barotrauma. Results: Of the studied parameters, a history of sinusitis and of middle-ear Barotrauma was significantly more frequent in divers in the Barotrauma group (p < 0.05). Conclusions: In divers undergoing a routine pre-dive examination, paranasal sinus Barotrauma was not related to sex, alcohol consumption, smoking, decongestant use, mild nasal septal deviation, or inability to perform the Valsalva or Toynbee manoeuvres. However, divers with a history of sinusitis or middle-ear Barotrauma may be more prone to paranasal sinus Barotrauma. A more detailed pre-dive examination may be useful for such individuals.

Study Design Prospective, blinded. Material and Methods Twenty-four sports scuba divers (48 ears), who were fit to dive in the predive and otolaryngologic examination, were included in the study. Size of mastoid pneumatization was measured by simplified rectangular dimension method on a mastoid x-ray taken at Schuller's view. Divers were counseled to refer to the investigators if any symptoms occurred during and/or after diving. All symptomatic ears were examined within 24 hours of diving by the same investigator, who was blinded to the degree of pneumatization. Results ME Barotrauma occurred in 15 ears (31%) of 11 divers (46%) at one time or another. The median degree of pneumatization in ears with Barotrauma (22.9 cm2) was significantly smaller than that in unaffected ears (34.1 cm2; (P <.001). Furthermore, findings showed that with increasing degree of pneumatization, there was a decreasing risk of symptomatic Barotrauma (P <.001). No Barotrauma occurred in ears with a pneumatization greater than 34.7 cm2. However, Barotrauma occurred in all 3 ears with a pneumatization degree smaller than 13.6 cm2. Conclusion Our findings indicate an inverse relationship between size of pneumatization and risk of symptomatic ME Barotrauma in sport scuba divers.

PLOS ONE, 2013Co-Authors: Ping Wang, Xiaoming Zhang, Zhaohua ZhaiAbstract:Background and Purpose To study the MRI findings of otic and sinus Barotrauma in patients with carbon monoxide(CO) poisoning during hyperbaric oxygen (HBO) therapy and examine the discrepancies of otic and sinus abnormalities on MRI between Barotrauma and acute otitis media with effusion. Materials and Methods Eighty patients with CO-poisoning diagnosed with otic and sinus Barotrauma after HBO therapy were recruited. Brain MRI was performed to predict delayed encephalopathy. Over the same period, 88 patients with acute otitis media with effusion on MRI served as control. The abnormalities of the middle ear and paranasal sinuses on MRI were noted and were compared between groups. Nine patients with Barotrauma were followed up by MRI. Results In the Barotrauma group, 92.5% of patients had bilateral middle ear abnormalities on MRI, and 60% of patients had both middle ear cavity and mastoid cavity abnormalities on MRI in both ears. Both rates were higher than those in the control group (p = 0.000). In the two groups, most abnormalities on MRI were observed in the mastoid cavity. The rate of sinus abnormalities of Barotrauma was 66.3%, which was higher than the 50% in the control group (p = 0.033). In the nine patients with Barotrauma followed up by MRI, the otic Barotrauma and sinus abnormalities had worsened in 2 patients and 5 patients, respectively. Conclusion MRI is able to depict the abnormalities of otic and sinus Barotrauma in patients with CO-poisoning during HBO therapy and to differentiate these from acute otitis media with effusion.
