Gastrointestinal disease following heart transplantation.

We reviewed our experience with gastrointestinal disease in 92 patients undergoing 93 heart transplants. All had follow-up, with the median time 4.8 years (range 0.5–9.6 years). During the period of the study we progressively adopted a policy of low immunosuppression aiming toward monotherapy with cyclosporine. Nineteen patients (20.6%) developed 28 diseases related to the gastrointestinal Tract. Thirteen patients required 18 surgical interventions, five as emergencies: closure of a duodenal ulcer, five cholecystectomies (one with Biliary Tract Drainage), a sigmoid resection for a diverticulitis with a colovesical fistula, a colostomy followed by a colostomy takedown for an iatrogenic colon perforation, appendectomy, two anorectal procedures, and six abdominal wall herniorrhaphies. At the onset of gastrointestinal disease, 8 patients were on standard triple-drug immunosuppression, all of them within 6 months of transplantation; 13 were on double-drug immunosuppression; and 7 were on cyclosporine alone. All the patients with perforations/fistulas were on steroids. Among the 11 infectious or potentially infectious diseases, 10 were on triple- or double-drug immunosuppression. One death, a patient who was on triple-drug immunosuppression, had a postmortem diagnosis of necrotic and hemorrhagic pancreatitis. Except for an incisional hernia following a laparoscopic cholecystectomy, there was no morbidity and, importantly, no septic complications. We concluded that a low immunosuppression policy is likely to be responsible for the low morbidity and mortality of posttransplant gastrointestinal disease, with a lower incidence of viscous perforation/fistula and infectious gastrointestinal disease.

What are the risk factors of conversion from total cholecystectomy to bailout surgery?
Surgical Endoscopy, 2020Co-Authors: Mitsugi Shimoda, Ryousuke Imasato, Yukio Oshiro, Shuji SuzukiAbstract:Background Laparoscopic cholecystectomy (LC) is regarded as the first choice for patients with gallbladder diseases, but Biliary injury (BDI) still poses serious risks upon implementation of LC. Recently, bailout surgery (BOS; partial cholecystectomy or subtotal cholecystectomy) has been proposed to avoid not only BDI but also major vessels injuries. In this retrospective study, we evaluated the preoperative and perioperative risk factors regarding conversion from total cholecystectomy (TC) to BOS. Methods A total of 584 patients who underwent elective LC for gallbladder diseases between January 2006 and April 2018 were analyzed. The patients were divided into the TC group (including conversion open TC) and the BOS group. Univariate and multivariate analyses using preoperative and perioperative clinicolaboratory characteristics were performed to investigate the most significant risk factors associated with conversion to BOS. Results There were a total of 33 patients in the BOS group (35 men and 18 women), with 19 patients who underwent open BOS and 14 patients who underwent laparoscopic BOS. From the univariate analyses, age, albumin level, CRP level, WBC, lymph. ratio, neutro. ratio, platelet count (PLt), neutrophil-to-lymphocyte ratio, platelet-to-lymphocyte ratio, CRP-to-alb ratio, intercurrent acute cholecystitis (AC), and previous Biliary Tract Drainage (PBTD) were considered as risk factors for the conversion to BOS. Multivariate analysis using the 13 parameters selected from the univariate analyses demonstrated that AC ( p = 0.04), albumin level ( p = 0.01) and age ( p = 0.04) were significant risk factors. Conclusion Patients with PBTD and AC have a high risk upon conversion from LTC to BOS, and for such patients, LC should be performed cautiously.

The effect of remifentanil on Biliary Tract Drainage into the duodenum.

Remifentanil is metabolized enzymatically throughout the body. Its context-sensitive half-time is 3-4 min. The effect of remifentanil on the sphincter of Oddi, is unknown. We studied, in six healthy adult volunteers, the effect of remifentanil on the flow of dye from the gall bladder into the duodenum. Control hepatoBiliary imaging with 5 mCi of technetium-labeled derivatives of iminodiacetic acid was performed on each volunteer. The time from IV dye (radiopharmaceutical) injection until its appearance in the duodenum was determined by continuous scanning. Two weeks later, each volunteer received remifentanil, 0.1 microg x kg(-1) x min(-1) infused for 30 min IV before the same dose of technetium-labeled derivatives of iminodiacetic acid was injected, and for the time of their control scan plus 10 min after the injection. When the dye appeared in the duodenum, the total time from injection was compared with the control value. The time from stopping the infusion until the dye appeared in the duodenum was the "recovery time." Control scan time was 20.5+/-9.9 min (mean +/- SD; range 10-33 min). Total scan time during and after the remifentanil infusion was 50.3+/-17.3 min (range 30-81 min) (P < 0002). The recovery time was 19.8+/-12.4 min (range 5-40 min). We conclude that remifentanil delays the Drainage of dye from the gall bladder into the duodenum, but the delay is shorter than that reported after other studied opioids. IMPLICATIONS Radioactive dye was injected IV into healthy volunteers to determine the time it took for the dye to appear in the duodenum. This was repeated under the influence of a short-acting narcotic analgesic, remifentanil. Remifentanil caused a much shorter delay than previously reported after morphine or meperidine.
