Clotting Disorder in severe acute respiratory syndrome coronavirus 2.

RasgonAbstract:The severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) is a novel human respiratory viral infection that has rapidly progressed into a pandemic, causing significant morbidity and mortality. Blood Clotting Disorders and acute respiratory failure have surfaced as the major complications among the severe cases of coronavirus disease 2019 (COVID-19) caused by SARS-CoV-2 infection. Remarkably, more than 70% of deaths related to COVID-19 are attributed to Clotting-associated complications such as pulmonary embolism, strokes and multi-organ failure. These vascular complications have been confirmed by autopsy. This study summarizes the current understanding and explains the possible mechanisms of the Blood Clotting Disorder, emphasizing the role of (1) hypoxia-related activation of coagulation factors like tissue factor, a significant player in triggering coagulation cascade, (2) cytokine storm and activation of neutrophils and the release of neutrophil extracellular traps and (3) immobility and ICU related risk factors.

Discovery of An Orally Effective Factor IX-Transferrin Fusion Protein for Hemophilia B.

FIX is not bioavailable when given orally due to poor stability and permeability in the gastrointestinal tract. The feasibility of fusing FIX with transferrin (Tf) to enhance the oral bioavailability of FIX is explored. Seven recombinant fusion proteins (rFIX-Tf) with different linkers were constructed and expressed in HEK293 cells and characterized by in vitro transcytosis and transferrin receptor (TfR) binding assay in Caco-2 cells and a one-stage Clotting assay. The in vivo efficacy study was performed using a tail-bleeding model in hemophilia B mice. Fusion proteins rFIX-Tf/G2 and rFIX-Tf/SVSQ were most permeable and showed a specific binding ability to TfR in Caco-2 cells. Both proteins retained FIX activity in Clotting generation. The in vivo efficacy study showed that both proteins by intravenous injection significantly reduced Blood loss. Most significantly, rFIX-Tf/G2 demonstrated anti-bleeding activity when administered orally. Our results showed that the fusion protein technique with Tf could be potentially used for oral delivery of FIX and the linker between FIX and Tf in the fusion protein is crucial. rFIX-Tf/G2 appears to be the most promising fusion protein as potential oral therapeutics for hemophilia B.

Clinical analysis of coagulation for 112 patients with traumatic brain injury
Hainan Medical Journal, 2008Co-Authors: Chen Wei-mingAbstract:Objective 112 cases with isolated head injury were analyzed retrospectively in the coagulation assays in order to explore the relationship between Blood Clotting function and the condition and the prognosis of the patients. Methods All 112 patients (79 males and 33 females) admitted in Haikou municipal hospital from July 2004 to Jan. 2006. The venous Blood samples were obtained from every patients on admission to detect TT、PT、APTT and Fbg respectively. The result was statistically analyzed according to their: GCS,GOS, have subsequent hematoma or not, have traumatic infarction or not. We used SPSS11.5 software package for statistical analysis. Significance was set at P0.05. Results 1. PT in both the patients with subsequent hematoma or not and the patients with traumatic infarction or not had significant difference. APTT in the patients with traumatic infarction or not had significant difference. TT had significant difference in all the different GCS groups. Fbg had significant difference in two conditions: different GCS scores and had subsequent hematoma or not. 2. TT in GOS 1 group prolonged significantly than that in the GOS2、3、4、5 group and Fbg in GOS 1 group was significantly lower than that in GOS2、3、4、5 group. Conclusion The detection about coagulation tests within 24h showed Blood-Clotting Disorder (coagulopathy) in the acute period post-head injury corresponded with patients ’ condition, outcome and complication. So, it is necessary to monitor continually in order to find the change of the coagulation of the patients as soon as possible. In addition, more distinctive test about coagulation is benefi-cial for diagnosis of coagulopathy.

Exploring the Hemodynamic Benefits of Cupping Therapy at the Upper Trapezius

The treatment for musculoskeletal pain conditions varies, but recent focus is on complementary and alternative medicine, such as cupping therapy. Cupping therapy is an ancient treatment method that involves the use of a cup to produce sub-atmospheric pressure on the skin. There is much speculation around its true mechanisms on the human body; theories indicate that cupping therapy has an effect on Blood flow, inflammation, and pain. Additionally, there has not been pre-defined parameters for the treatment of musculoskeletal pain conditions.Objective: To examine if different dry cupping treatment times altered changes in superficial and deep subcutaneous tissue hemodynamics. Design: Single-blinded, randomized crossover study Setting: Athletic Training Laboratory Participants: 32 participants volunteered for this study. Participants were included if they were healthy individuals with non-specific neck pain. Participants were excluded if they had cupping therapy or any treatment performed within the past three months to the neck or shoulder area; history of head, neck, or shoulder injury within the past six months resulting in medical care; known Blood Clotting Disorder; allergy to lubricant; or the following medical conditions: hypertension, diabetes, cancer, pregnancy, cardiac failure, renal failure, allergic purpura, hernia, psoriasis, eczema, rosacea, varicose veins, phlebitis, hepatocirrhosis, allergic dermatitis, sunburn, open wound, fever, or were taking anticoagulants.Interventions: Dry cupping therapy for 5, 7.5, and 10 minutes in a randomized order repeated once per week. One stationary cup was placed directly over the midpoint of the upper trapezius muscle for each participant for the allotted time.Outcome Measures: Subcutaneous hemodynamics (superficial and deep oxygenated, deoxygenated, and total hemoglobin) were collected and exported for data analyses using the NIRS Portamon. Change scores were calculated between baseline and immediate postintervention, immediate and 10 minute post-intervention, and baseline and 10 minute postintervention measurements. Statistical analyses were completed using repeated measures ANOVAs to compare changes in subcutaneous hemodynamics following different treatment times (5, 7.5, and 10 minutes).Results: There was a main effect for superficial and deep oxygenated, deoxygenated, and total hemoglobin (p ≤ 0.001). Post hoc analyses revealed that all treatment times increased hemoglobin levels immediately after intervention and maintained this increase over the 10 minute period for oxygenated and total hemoglobin levels.Conclusions: The results demonstrated that dry cupping therapy increases deep and superficial oxygenated, deoxygenated, and total hemoglobin levels at treatment times of 5, 7.5, and 10 minutes. This indicates that clinicians can apply cupping therapy for a shorter period of time and maintain a significant effect on Blood flow. Aptamer-antibody dual probes on single-walled carbon nanotube bridged dielectrode: Comparative analysis on human Blood Clotting factor.

Bleeding leads various issues, such as chronic pain, arthritis and a serious complication during the surgery. Identifying this disease is mandatory to take the necessary treatment and maintains the normal Clotting. It has been proved that the level of factor IX (FIX) is lesser with haemophilia patient and the attempt here is focused to quantify FIX level by interdigitated electrode (IDE) sensor. Single-walled carbon nanotube (SWCNT) was utilized to modify IDE sensing surface. On this surface, dual probing was evaluated with aptamer and antibody to bring the possible advantages. The detection limit with antibody was found to be 1 pM, while aptamer shows 100 fM. Further, a fine-tuning was attempted with sandwich pattern of aptamer-FIX-antibody and antibody-FIX-aptamer and compared. Specific elevation of detection with 10 folds was noticed and displayed the detection at 100 f. in both sandwich patterns. In addition, FIX was detected in the diluted human serum by aptamer-FIX-antibody sandwich, it was found that FIX detected from the dilution factor 1:640. A novel demonstration is with higher discrimination against other Clotting factors, XI and VII.
