CLINICAL PROGRESS Beriberi Heart Disease
2016Co-Authors: H. JonesAbstract:Two cases of beriberi heart disease are presented together with a discussion of the history, diagnosis, and treatment of this condition. Special emphasis is given to the broadened concept of the diagnosis of beriberi heart disease, particularly as it occurs in the Occident. DURING THE PAST several decades sig-nificant advances have been made in the diagnosis and therapy of various types of heart disease. Intensive investigative studies have now assigned specific causes to certain cardiac disorders that formerly, for want of a more accurate classification, were loosely grouped under the category of "arterioscle-rotic, " "coronary, " or "idiopathic " heart disease. Beriberi heart disease is one such abnormality and it is the purpose of this com-munication to describe the clinical manifesta-tions, the diagnosis, and the treatment of this condition as illustrated by 2 cases. Although classical descriptions of the poly-neuritic form of beriberi have been available in Japanese and Chinese literature for many centuries, it was not until 1929 that Aalsmeer, a Javanese physician, and Wenekebach ' pre-sented an adequate description of the cardio-vascular manifestations of this disorder. These authors emphasized the Bounding Pulse, pistol-shot sounds over the larger arteries, predomi-nant right heart failure, venous engorgement, and frequent syncope and shock They thought that there was peripheral vasodilatation, de-scribed the rapid circulation, and noted that improvement often occurred after the admin-istration of thiamine. direct or modified seldinger guide wire directed technique for arterial catheter insertion

We compared the success rate of arterial catheter placement in patients undergoing aortocoronary bypass operations using the direct and the modified Seldinger techniques. The effects of gender and quality of the Pulse were also investigated. Our study group consisted of 42 female and 96 male patients. The overall success rate with the guide wire was 82% compared with that of the direct method of 65% (P = 0.02). The success rate for arterial cannulation in male patients was high whether direct cannulation or a guide wire was used. In female patients, on the other hand, the failure rate with the direct technique was high (57%) and was significantly lower (14%) when the guide wire was used (P < 0.001). We conclude that the success rate for cannulation is high in male patients, and patients with a Bounding Pulse regardless of the use of the direct or guide-wire techniques. The guide wire is recommended as the initial technique for cannulating the radial artery of female patients. In patients with a thready Pulse, no significant advantage could be obtained by using a guide wire, but in salvaging an arterial line the guide wire is efficacious.

monckeberg s arteriosclerosis vascular calcification complicating microvascular surgery
International Journal of Oral and Maxillofacial Surgery, 2015Co-Authors: B Castling, S Bhatia, F AhsanAbstract:Monckeberg's arteriosclerosis is often an incidental finding, identified either clinically or on plain radiography. It can occasionally be associated with diabetes mellitus or chronic kidney disease. It differs from the more common atherosclerosis in that the tunica intima remains largely unaffected and the diameter of the vessel lumen is preserved. Despite such vessels appearing hard and Pulseless throughout their affected length, they deliver relatively normal distal perfusion, indeed there is often a Bounding Pulse at the end of the calcified zone. They appear unremarkable on magnetic resonance angiography but visibly calcified on plain radiography. Monckeberg's arteriosclerosis has a prevalence of < 1% of the population, but when it does occur it can cause consternation at the prospect of using these vessels for microvascular anastamosis. We report our experience of deliberately using these vessels in an osseocutaneous radial forearm free flap reconstruction. Although there are some technical considerations to bear in mind, we would suggest that unlike vessels affected by atherosclerosis, anastomosis of arteries affected by Monckeberg's arteriosclerosis has little or no impact on free flap survival.
