Campylobacteriosis in Urban versus Rural Areas: A Case- Case Study Integrated with Molecular Typing to Validate Risk Factors and to Attribute Sources of Infection

From July 2005 to December 2007 we conducted a prospective case-case study of sporadic, domestically-acquired Campylobacter Enteritis in rural versus urban areas and a prevalence study of Campylobacter in animal and environmental sources in the Eastern Townships, Quebec. Isolates were typed using Multilocus Sequence Typing (MLST) to reinforce the case-case findings and to assign a source probability estimate for each human isolate. The risk of human Campylobacteriosis was 1.89-fold higher in rural than urban areas. Unconditional multivariate logistic regression analysis identified two independent risk factors associated with human Campylobacter infections acquired in rural area: occupational exposure to animals (OR = 10.6, 95 % CI: 1.2–91, p = 0.032), and household water coming from a private well (OR = 8.3, 95 % CI: 3.4–20.4, p,0.0001). A total of 851 C. jejuni isolates (178 human, 257 chicken, 87 bovine, 266 water, 63 wild bird) were typed using MLST. Among human isolates, the incidence rates of clonal complexes (CC) CC-21, CC-45, and CC-61 were higher in rural than urban areas. MLST-based source attribution analysis indicated that 64.5 % of human C. jejuni isolates were attributable to chicken, followed by cattle (25.8%), water (7.4%), and wild birds (2.3%). Chicken was the attributable source for the majority of cases, independent of residential area, sex and age. Journal of Medical Microbiology, 2001Co-Authors: Sophie Michaud, Suzanne Menard, Christiane Gaudreau, Robert D ArbeitAbstract:Pulsed-field gel electrophoresis (PFGE) was used to analyse 147 isolates collected in two regions of Quebec province (Estrie and Montreal) between March 1998 and Feb. 1999, to determine the utility of molecular strain typing for a population-based collection of Campylobacter jejuni and to compare directly the discriminatory power of SmaI and KpnI restriction digests. With a combination of epidemiological criteria including space and time plus molecular strain typing, 49% of isolates from Estrie and 39% of isolates from Montreal were identified as belonging to a putative cluster. For 41% of the cases, sources were either missing or explicitly unknown; the remaining sources were subject to recall bias. Thus, the evaluation of sporadic cases of Campylobacter Enteritis by descriptive clinical investigation alone is neither sensitive nor reliable for identifying sources of infection. In the PFGE analysis, KpnI digests provided appreciably greater discriminatory power than SmaI digests. When combining the PFGE analyses with basic epidemiological criteria, 30% of the putative SmaI clusters were inconsistent with the epidemiological criteria compared with 17% of the KpnI clusters. Among the 98 isolates assigned to clusters by SmaI, only 65% gave concordant results with KpnI. In contrast, among the 81 isolates assigned to clusters by KpnI, 92% gave concordant results with SmaI. Finally, clusters that were epidemiologically related to ingestion of raw milk and specific water sources correlated better with the typing results based on KpnI than SmaI. Thus, KpnI is the enzyme of choice for molecular epidemiology studies of C. jejuni. The combination of continuous epidemiological surveillance and molecular strain typing may be useful for identifying new sources and mechanisms of transmission for community-acquired C. jejuni infection and ultimately for developing new approaches to prevention.

American Journal of Epidemiology, 1993Co-Authors: Mahdi A Saeed, Noreen V Harris, Ronald F DigiacomoAbstract:To determine the role of animals as possible sources for human infection with Campylobacter jejuni/coli, 218 human cases of Campylobacter Enteritis diagnosed among members of Group Health Cooperative of Puget Sound, King County, Washington, from April 1982 through September 1983 were compared with 526 controls, randomly selected from Group Health Cooperative members. All subjects were questioned regarding animal exposures one week prior to illness (cases) or interview (controls). There was no increase in risk for C. jejuni/coli Enteritis associated with contact with various animals. However, exposure to diarrheic animals was associated with a fourfold increase in the risk of C. jejuni/coli Enteritis (odds ratio = 4.3, 95% confidence interval (CI) 1.9-9.7). Adjustment for potential confounding factors by logistic regression analysis yielded an odds ratio of 3.3 (95% CI 1.2-7.5) associated with such exposure. An estimated 6.3% of cases of C. jejuni/coli Enteritis was attributed to exposure to diarrheic animals.

an outbreak of Campylobacter Enteritis associated with a community water supply on a u s military installation

Campylobacter jejuni was isolated from 81 of 163 (50%) persons cultured. Seventeen isolates of C. jejuni available for serotyping were Lior serotype 5. The outbreak remained restricted to one recruit barracks area and adjacent Junior Reserve Officer Training Corps cadet barracks. Infection of sequential cohorts of recruits over an interval of 3 weeks suggested a continuing or intermittent common source. Contaminated food was not implicated because affected persons ate at separate dining facilities and other facilities with the same food sources had no associated illnesses. There was a strong association between the amount of water consumed by recruits and risk of diarrhea (chi-square test for trend, p<0.001). Samples of drinking water collected in the affected area had no residual chlorine and when cultured yielded greater than 200 colonies of coliform bacteria per 100 mL of water sampled. Although Campylobacter was not isolated from water, living and dead birds were found in an elevated water storage tank providing drinking water to the affected area. This and other similar outbreaks indicate that contamination of water storage tanks can lead to large outbreaks of Campylobacter Enteritis.

Stool cultures were obtained and symptoms were recorded on study days 0, 1, 2, 3, and 10. Campylobacter species were the most common pathogen isolated (44 isolates from 42 patients). All Campylobacter isolates were susceptible to azithromycin; 22 were resistant to ciprofloxacin. Among the 42 patients with Campylobacter infection, there were 2 clinical and 6 bacteriologic treatment failures in the ciprofloxacin group and no treatment failures in the azithromycin group (P = .021 for bacteriologic failures). Overall, azithromycin was as effective as ciprofloxacin in decreasing the duration of illness (36.9 hours vs. 38.2 hours, respectively) and the number of stools (6.4 vs. 7.8, respectively). Among those not infected with Campylobacter species (n = 30), the duration of illness was 32.9 hours vs. 20.7 hours (P = .03) for the azithromycin and ciprofloxacin groups, respectively. Azithromycin is superior to ciprofloxacin in decreasing the excretion of Campylobacter species and as effective as ciprofloxacin in shortening the duration of illness. Azithromycin therapy may be an effective alternative to ciprofloxacin therapy in areas where ciprofloxacin-resistant Campylobacter species are prevalent.

Illness in estate residents was associated with tap water consumption [population attributable risk (PAR) 50 %, relative risk (RR) 2 . 53, 95% confidence interval (CI) 1 . 9–3 . 37] and residence in the upper estate (PAR 49 %, RR 2 . 44, 95 % CI 1 . 83–3 . 24). Amongst upper estate residents, rates of diarrhoeal illness increased with rates of water consumption (OR 18, 95% CI 3 . 5–92 . 4 for heaviest consumers, x 2 trend P<0 . 0001). The upper estate received mains water via a covered holding reservoir. A crack in the wall of the holding reservoir was identified. Contamination with surface water from nearby pasture land was the likely cause of this outbreak. Service reservoirs are common in rural communities and need regular maintenance and inspection. The role of water in sporadic cases of Campylobacter Enteritis may be underestimated.
