    Abstract:

    Abstract The use of magnesium sulfate infusion for the management of Cardiac Dysrhythmia has recently gained popularity. Magnesium sulfate has been advocated for the management of torsade de pointes and other ventricular Dysrhythmias. We report the case of a 38-year-old firefighter with atrial tachycardia that was treated unsuccessfully according to Advanced Cardiac Life Support guidelines with IV adenosine. Subsequently, 2 g of magnesium sulfate was administered intravenously over 5 minutes with resulting conversion of the patients' supraventricular tachycardia to normal sinus rhythm, with complete resolution of symptoms.     Abstract:

    Endotracheal suctioning is one of the most important care that done continuously in patient with Brain injuries. Type of suctioning affected on the hemodynamic status of patients, this study was performed with the aim of comparing changes in Arterial Blood Oxygen Saturation and Occurrence of Cardiac Dysrhythmia in head trauma patients with open and closed suction. In this cross-over clinical trial study, 88 patients who under mechanical ventilator were enrolled randomly divided into two groups. Closed and open suctioning each done for each patient and changes on Arterial Blood Oxygen Saturation and Occurrence of Cardiac Dysrhythmia at intervals before, during. Immediately, five, ten and fifteen minutes by using paired t-test, ANOVA with repeated data and Fisher's exact test were performed by software SPSS version 22. 88 patients with ranging from 15 to 45were enrolled. 68% of patients were male. The most cause of head injury was car accident (75%) that the most frequent CT scan findings associated with intraventricular hemorrhage (22%). The occurrence of Cardiac Dysrhythmia and changes on Arterial Blood Oxygen Saturation was more in open method of endotracheal suctioning. There were significant relationships between Arterial Blood Oxygen Saturation and occurrence of Cardiac Dysrhythmia in Multiple time intervals with type of suctioning.(p<0.001). According to the findings, to reduce Arterial Blood Oxygen Saturation and incidence of Cardiac Dysrhythmia due to endotracheal suctioning, the use of closed suction in the intensive care unit is recommended.

    Abstract:

    Twenty different point mutations have been identified in the gene coding for the α subunit of the adult skeletal muscle sodium channel in families with hyperkalemic periodic paralysis, paramyotonia congenita, and the potassium-aggravated myotonias. One novel mutation (Val781Ile) was reported in an adopted boy with potassium-sensitive weakness and Cardiac Dysrhythmia. The confidence in establishing this rare amino acid substitution as a causative mutation was limited by the absence of family members for segregation analysis. Functional expression studies herein show that Val781Ile is most likely a benign polymorphism and not a disese-associated mutation.

    Purpose:Central venous catheterization is a standard procedure in intensive care therapy. In developing countries, this intervention is frequently performed by physicians in training and without the availability of ultrasound guidance. Purpose of this study was to determine the incidence and potential risk factors for mechanical complications during central venous catheterization in an intensive care setting performed by a mixed group of practitioners without the use of adjunct ultrasound.Methods:Prospective observational cohort study in a university teaching hospital. Three hundred critically ill patients requiring their first central venous catheter insertion were enrolled. All patients were observed for 24 hours for mechanical complications (pneumothorax, hemothorax, arterial puncture, incorrect tip position, Cardiac Dysrhythmia, and/or subcutaneous hematoma).
