    Summary: We report a case of Cauda equina syndrome caused by Gnathostoma spinigerum, which was confirmed by an immunoblotting test. MR imaging of the lumbosacral spine showed long, segmented hyperintensity along the Cauda equina with irregular enhancement on the postcon-trast study. The conus medullaris was slightly enlarged with abnormal enhancement. The patient was treated with corticosteroids, and her clinical condition improved. MR imaging, 9 months after treatment, showed the condition to be completely resolved. Gnathostomiasis (Gnathostoma spinigerum infesta-tion) is commonly found in Thailand and Japan and is caused mainly by eating raw infested fish.   • Plexiform neurofibroma of the Cauda equina. Case report.

    Abstract:

    Plexiform neurofibroma of the Cauda equina has been reported only twice previously. The authors report the first pediatric patient in whom such a tumor has been found. A 4-year-old boy presented with low-back pain that radiated bilaterally into the L-4 and L-5 dermatomes. A dermal sinus noted at the midthoracic level was surrounded by a hemangiomatous lesion. Magnetic resonance imaging confirmed the presence of the dermal sinus and revealed a well-defined lumbosacral mass that showed heterogeneous intensity with irregular enhancement. Intraoperatively, a solid mass, which engulfed the entire Cauda equina, could not be dissected from the roots. The dermal sinus tract, however, was excised from the thoracic spine. The patient underwent radiotherapy to control the tumor and relieve his pain. Plexiform neurofibromas of the Cauda equina are characterized by an insidious and progressive clinical course. The tumor mass may engulf all the roots of the Cauda equina. No plexiform neurofibroma of the Cauda equina has been reported to be associated with neurofibromatosis Type 1. The authors assume that the thoracic-level dermal sinus observed in this child was an incidental finding.

    ve To study the recovery of nerve function of Cauda equina syndrom after lumbar disc herniation operation and its clinical significance Methods 33 cases of Cauda equina syndrom induced by lumbar disc herniation were reviewed and divided into two groups: early operation group and delayed operation group. In the former group,20 cases had undergone operation within 48 hours including 17 in 24 hours,and in the latter group, 13 cases after 48 hours following Cauda equina syndrom occurrence with an average delay of 7 days,also included 9 delayed cases caused by iatrogenic reasons. Results The incidence rate of bladder dys-function dyskinesia and rest pain in the delayed operation group was obviously higher than that of the early op-eration group(P0.05). Conclusion For the patients with Cauda equina syndrom induced by lumbar disc herniation, the operation should be done as early as possible.

    BACKGROUND AND PURPOSE: Cauda equina syndrome typically requires emergent MR imaging to detect compressive lesions on the Cauda equina, which may require surgical decompression. While CT is sometimes performed as a complementary imaging technique to evaluate osseous integrity in patients with Cauda equina syndrome, the accuracy of CT in detecting significant spinal stenosis and Cauda equina impingement is not well-defined in the literature. We hypothesized that percentage thecal sac effacement on CT of the lumbar spine would have high sensitivity and high negative predictive value in evaluating significant spinal stenosis and Cauda equina impingement. MATERIALS AND METHODS: We analyzed imaging studies for 151 consecutive patients with clinically suspected Cauda equina syndrome. The percentage thecal sac effacement ( RESULTS: Forty of 151 patients had a percentage thecal sac effacement of ≥50% on MR imaging. Nineteen of 40 had Cauda equina impingement. Readers determined that there was a CT percentage thecal sac effacement of CONCLUSIONS: CT percentage thecal sac effacement of ≥50% predicts significant spinal stenosis on MR imaging in patients with clinically suspected Cauda equina syndrome.
