Cerebellum Tumor presenting itself with positional vertigo and benign paroxysmal positional vertigo
Journal of Craniofacial Surgery, 2021Co-Authors: Ceren Gunebakan, Abdullah KinarAbstract:ABSTRACT The purpose of this case is to describe the positional vertigo observed in a patient diagnosed with cerebellar arteriovenous malformation, pay attention to the importance of medical history taking and physical examination in vertigo patients.A 51-year-old patient went to the Ear, Nose, and Throat clinic with a complaint of vertigo. His vertigo was like peripheral vertigo at the beginning. Dizziness was triggered by head movements. He experienced tinnitus in the left ear during vertigo attacks. The patient also had neck pain. In physical examination, natural bilateral tympanic membrane and facial examination were observed. Other physical examinations were normal. In the positional vertigo tests, the right Dix-Hall Pike test was positive and a downbeating geotropic nystagmus was found. The patient was treated with canalith repositioning maneuver (Epley maneuver). Oral medical treatment started and after 4 days, the patient reported that his gait balance was disturbed and his neck pain continued. After that magnetic resonance imaging was requested. Magnetic resonance imaging was consistent with cerebellar arteriovenous malformation. The patient was consulted to the neurology service.Cerebellar arteriovenous malformation had features like to peripheral vertigo, and the correct diagnosis is made due to suspected headache and other neurological symptoms.

Material and methods: The study included 14 patients in the age between 21–55 years old with the Tumor of Cerebellum, who were operated in the Neurosurgery Clinic of the Pomeranian Medical University in Szczecin. The comprehensive ophthalmic examination were performed before and after 5 days from surgery. The examinations included evaluation of: pupillary reactions, visual acuity, fundus ophthalmoscopy, intraocular pressures, eye motility, visual field, optometrical tests and visual manual localization test. Results: The symptoms found before surgery of Cerebellum Tumors: diplopia (3 persons), early papilloedema (4 persons), nystagmus (2 persons), lack (5 persons) and weakened of convergence reflex (3 persons), latent strabismus (5 persons), manifest strabismus (3 persons). On the 5th day after the surgery were found: nystagmus (1 person), lack (5 persons) and weakened of the convergence reflex (3 persons). Visual manual localization was disparate depending on location of the Tumor in the Cerebellum; prevailed crossed and uncrossed localizations. Conclusions: 1. The natural dynamic of the Tumor and surgical damage to the posterior cranial cavity structures can be monitored by assessment of the condition of the organ sight. 2. It is recommended to make the visual manual localization tests such as exponent disorders of senso-motorical deviations, which may occur in the patients with the Cerebellum Tumor.

effect of nursing intervention on advanced cognitive function and mood disorder in patients with Cerebellum Tumor
Chinese Journal of Modern Nursing, 2018Co-Authors: Fei YaoAbstract:Objective To study the effectiveness of nursing intervention on advanced cognitive function and mood disorder in patients with Cerebellum Tumor and provide basis for clinical care. Methods A total of 144 patients with cerebellar Tumor were selected from February 2014 to April 2016 in Neurosurgery Department, Beijing Tian Tan Hospital. Patients were divided to observation group (n=72) and control group (n=72) according to admission order. Patients of the observation received nursing intervention that could improve advanced cognitive function and mood disorder while patients of the control group underwent usual care. Mini-mental State Examination (MMSE) , Memory Quotient (MQ) , Hamilton Anxiety Scale (HAMA) and Hamilton Depression Scale (HAMD) were applied to evaluate patients' advanced cognitive function and mood disorder before and after intervention in both groups. Results For the observation group, score of MMSE was (19.8±1.6) points and MQ was (107.0±14.6) points which were both higher than those of the control group (t=12.834, 4.234; P<0.01) at 90 days after operation. At 180 days after operation, for the observation group, score of HAMA was (11.6±2.1) points and HAMD was (15.0±2.8) points which were both lower than those of the control group (t=8.116, 8.583; P<0.01) . Conclusions Nursing intervention could improve the advanced cognitive function and mood disorder in patients with Cerebellum Tumor clearly which could improve cognitive function in language and memory, relieve mood disorder like depression and anxiety, and it is suitable for clinical promotion. It has been approximated that 2.2 million people in America and 4.5 million in European Union have paroxysmal or persistent atrial fibrillation. And atrial fibrillation itself is an independent long-term risk factor of stroke. We present a case of patient referred to our center for surgical removal of suspected Cerebellum Tumor, a case that had a rather unexpected ending.
