    A novel dispersive liquid-liquid microextraction (ILDLLμE) method using an extracting solvent (ionic liquid) and dispersant (Triton X-114) was developed for the separation and preconcentration of hexavalent chromium (Cr6+) in artificial saliva extract (ASE) of Chewing Tobacco products, gutkha, and mainpuri (n = 23). In the proposed method, the extraction of Cr6+ was accomplished by using ammonium pyrrolidinedithiocarbamate (APDC) as complexing agent and 1-butyl-3-methylimidazolium hexafluorophosphate [C4MIM] [PF6] as extracting solvent. The tiny droplet of metal chelate was then dispersed into TX-114 emulsion, using vortex mixer. Various parameters such as concentration of APDC, pH of the solution, volume of [C4MIM] [PF6], and TX-114 as well as extraction time were studied. Health risks associated with the intake of total Cr in gutkha and mainpuri were assessed in terms of estimated daily intake, such as carcinogenic and noncarcinogenic risks. Estimated daily intake of Cr via Chewing 10 g/day of gutkha and mainpuri was found to be below the maximum tolerable daily intake, whereas the calculated risk of cancer for Cr was observed in the acceptable range of 10E-6-10E-4, except some brands of gutkha.     Epidemiologic studies suggest that zinc (Zn) deficiency and high accumulation of cadmium (Cd) may be associated with increased risk of cancer. The incidence of mouth cancer has increased among females, who possess habits of Chewing Tobacco with gradients (areca nut and betel quid) and smoking Tobacco in Pakistan. In present study, we measured the concentration of Cd and Zn in 96 mouth cancer patients (MCPs) and 110 female controls/referents (67 smoker and Chewing Tobacco), while 43 have none of smoking and Chewing Tobacco habits, belongs to different cities of Pakistan. Both controls and patients have same age group (ranged 35-65 years), socio-economic status, localities and dietary habits. The Zn and Cd were determined by flame/graphite furnace atomic absorption spectrophotometer, prior to microwave assisted acid digestion method. The Cd/Zn ratio in both biological samples was also calculated. The results of this study showed that the mean value of Zn was lower, while the mean concentration of Cd was higher in the blood and scalp hair samples of MCPs as compared to control subjects (p<0.001). The controls Chewing and smoking Tobacco have high level of Cd in both biological samples as compared to those have not smoking or Chewing Tobacco (p<0.012). The Cd/Zn ratio was higher in MCPs than control subjects. This study is compelling evidence in support of positive associations between cadmium, cigarette smoking, deficiency of Zn and cancer risk.

  • Whole genome expression profiling in Chewing-Tobacco-associated oral cancers: a pilot study.

    Abstract:

    The current study was undertaken with a view to identify differential biomarkers in Chewing-Tobacco-associated oral cancer tissues in patients of Indian ethnicity. The gene expression profile was analyzed in oral cancer tissues as compared to clinically normal oral buccal mucosa. We examined 30 oral cancer tissues and 27 normal oral tissues with 16 paired samples from contralateral site of the patient and 14 unpaired samples from different oral cancer patients, for whole genome expression using high-throughput IlluminaSentrix Human Ref-8 v2 Expression BeadChip array. The cDNA microarray analysis identified 425 differentially expressed genes with >1.5-fold expression in the oral cancer tissues as compared to normal tissues in the oral cancer patients. Overexpression of 255 genes and downregulation of 170 genes (p < 0.01) were observed. Further, a minimum twofold overexpression was observed in 32 genes and downregulation in 12 genes, in 30–83 % of oral cancer patients. Biological pathway analysis using Kyoto Encyclopedia of Genes and Genome Pathway database revealed that the differentially regulated genes were associated with critical biological functions. The biological functions and representative deregulated genes include cell proliferation (AIM2, FAP, TNFSF13B, TMPRSS11A); signal transduction (FOLR2, MME, HTR3B); invasion and metastasis (SPP1, TNFAIP6, EPHB6); differentiation (CLEC4A, ELF5); angiogenesis (CXCL1); apoptosis (GLIPR1, WISP1, DAPL1); and immune responses (CD300A, IFIT2, TREM2); and metabolism (NNMT; ALDH3A1). Besides, several of the genes have been differentially expressed in human cancers including oral cancer. Our data indicated differentially expressed genes in oral cancer tissues and may identify prognostic and therapeutic biomarkers in oral cancers, postvalidation in larger numbers and varied population samples.

    With a view to identify genomic risk variants in Chewing-Tobacco associated oral cancer patients, a genome-wide association study was conducted in patients of Indian ethnicity with long term Tobacco Chewing habit. We analyzed 55 oral cancer patients and 92 healthy controls for single nucleotide polymorphisms, using high throughput microarray Illumina Infinium II Assay platform and Human CNV370k-bead chip containing 370,000 single nucleotide polymorphisms. The PLINK software platform defined 298 SNPs with minor allele frequency of several genes significantly increased in oral cancer patients as compared to the controls (p<0.001). Illumina Genome Viewer Software Version 3.2.9, further delineated 93 SNPs with p-values ranging from 9.3×10(-4) to 1.38×10(-5) and Odd's ratio of 2.18-8.48, associated with 70 genes. Analysis using Kyoto Encyclopedia of Genes and Genome Pathway database, indicated SNP association with several genes including GRIK2, RASGRP3, CAMK4, SYK, RAPTOR, FHIT, DCC, active in signal transduction; MMP2, CNTNAP2, PTPRJ associated with tumor cell migration; and apoptotic gene IRAK3. The data indicates an inherent role for the genetic constitution of individuals in oral carcinogenesis, with the genomic variants contributing to increased risk or susceptibility to oral cancer.

  • Concurrent hypermethylation of multiple regulatory genes in Chewing Tobacco associated oral squamous cell carcinomas and adjacent normal tissues

    Abstract:

    The methylation pattern in the promoter region of p16, DAPK, MGMT and GSTP1 genes was investigated in oral cancer tissues and tumor associated adjacent tissues, using methylation specific PCR assay. The samples constituted 60 primary oral tumors and corresponding adjacent clinically and histopathologically normal mucosa, and buccal epithelial scrapings from 20 normal healthy individuals without any Tobacco habits. The incidence of hypermethylation in oral tumor and adjacent mucosa for p16 gene was 66.7 and 50%, for DAPK was 68.3 and 60%, and MGMT gene was 51.7 and 26.7%, respectively. The overall hypermethylation in the three genes in the primary tumor was 86.7%, and corresponding adjacent normal mucosa tissues 76.7%. Hypermethylation was not observed in the promoter region of GSTP1 gene in either the primary tumors or the corresponding adjacent normal mucosa. Absence of aberrant methylation in the four genes was noted in buccal scrapings from normal healthy individuals with no Tobacco habits. Thus, a high frequency of promoter region hypermethylation was observed in p16, DAPK and MGMT genes in oral cancer tissues as well as in corresponding adjacent normal mucosa. Our results indicate that epigenetic alteration of these genes is a frequent event in oral cancer, and is an early event observed in normal oral mucosa of the patients, indicating the critical importance of the epigenetic alteration in Chewing Tobacco associated oral carcinogenesis.

    Abstract:

    The mitogen activated serine/threonine kinases (MAPKs) constitute extracellular signal-regulated protein kinases (ERKs), c-Jun N-terminal kinases (JNKs) and p38 MAPK, with an important role in cell proliferation and transformation. Earlier studies from our laboratory had indicated a role for MAPK pathway in oral cancer. Our current study was aimed at examining the role of a MAPK-ERK3, in Chewing-Tobacco associated oral squamous cell carcinoma. We constructed a cDNA library from primary oral cancer tissue, cloned and isolated the ERK3 gene. The gene was sequenced and the sequence submitted to GenBank (Accession number AF420474). The oral cancer ERK3 clone demonstrated 100% homology to human ERK3 isolated from fetal skeletal muscle, with four specific nucleotide alterations in the non-coding region of the gene, comprising deletion of 'TTT' between 2701 and 2705 nt; 'G' to 'T' substitution at 188 nt; insertion of 'A' between 121 and 122 nt, and insertion of 'CTTTA' between 3391 and 3392 nt. Southern analysis of EcoRI genomic digests indicated ERK3 specific fragments of 11, 8.6, 6.5 and 3.2 kb sizes. The mRNA transcript analysis defined a single transcript of 4.5 kb. RT-PCR analysis revealed a three- to eight-fold increase in ERK3 expression in a majority (90%) of oral cancer tissues and peripheral blood cells (61.5%) of the patients, whereas absence or low levels of expression was observed in peripheral blood cells of 74% clinically normal healthy individuals with no Tobacco habits, and overexpression in PBC from 26% normal individuals. The alterations in the non-coding region of ERK3 gene cloned from oral cancer tissue, may affect stability or regulation of mRNA, resulting in overexpression in the patient samples. The overexpression of the gene in the normal healthy individuals may be indicative of increased risk of developing oral cancers in this group.

    Abstract:

    Epidemiologic studies suggest that zinc (Zn) deficiency and high accumulation of cadmium (Cd) may be associated with increased risk of cancer. The incidence of mouth cancer has increased among females, who possess habits of Chewing Tobacco with gradients (areca nut and betel quid) and smoking Tobacco in Pakistan. In present study, we measured the concentration of Cd and Zn in 96 mouth cancer patients (MCPs) and 110 female controls/referents (67 smoker and Chewing Tobacco), while 43 have none of smoking and Chewing Tobacco habits, belongs to different cities of Pakistan. Both controls and patients have same age group (ranged 35-65 years), socio-economic status, localities and dietary habits. The Zn and Cd were determined by flame/graphite furnace atomic absorption spectrophotometer, prior to microwave assisted acid digestion method. The Cd/Zn ratio in both biological samples was also calculated. The results of this study showed that the mean value of Zn was lower, while the mean concentration of Cd was higher in the blood and scalp hair samples of MCPs as compared to control subjects (p<0.001). The controls Chewing and smoking Tobacco have high level of Cd in both biological samples as compared to those have not smoking or Chewing Tobacco (p<0.012). The Cd/Zn ratio was higher in MCPs than control subjects. This study is compelling evidence in support of positive associations between cadmium, cigarette smoking, deficiency of Zn and cancer risk.
