Design Randomised controlled trial. Setting Department of Child Psychology. Participants 71 consecutively referred patients with chronic fatigue syndrome; 36 were randomly assigned to immediate cognitive behaviour therapy and 35 to the waiting list for therapy. Intervention 10 sessions of therapy over five months. Treatment protocols depended on the type of activity pattern (relatively active or passive). All participants were assessed again after five months. Main outcome measures Fatigue severity (checklist individual strength), functional impairment (SF-36 physical functioning), and school attendance. Results 62 patients had complete data at five months (29 in the immediate therapy group and 33 on the waiting list). Patients in the therapy group reported significantly greater decrease in fatigue severity (difference in decrease on checklist individual strength was 14.5, 95% confidence interval 7.4 to 21.6) and functional impairment (difference in increase on SF-36 physical functioning was 17.3, 6.2 to 28.4) and their attendance at school increased significantly (difference in increase in percentage school attendance was 18.2, 0.8 to 35.5). They also reported a significant reduction in several accompanying symptoms. Self reported improvement was largest in the therapy group. Conclusion Cognitive behaviour therapy is an effective treatment for chronic fatigue syndrome in adolescents.

Journal of Child Psychology and Psychiatry, 2019Co-Authors: Nathaniel J Williams, Rinad S BeidasAbstract:Background Scientists have developed evidence-based interventions that improve the symptoms and functioning of youth with psychiatric disorders; however, these interventions are rarely used in community settings. Eliminating this research-to-practice gap is the purview of implementation science, the discipline devoted to the study of methods to promote the use of evidence-based practices in routine care. Methods We review studies that have tested factors associated with implementation in Child Psychology and psychiatry, explore applications of social science theories to implementation, and conclude with recommendations to advance implementation science through the development and testing of novel, multilevel, causal theories. Results During its brief history, implementation science in Child Psychology and psychiatry has documented the implementation gap in routine care, tested training approaches and found them to be insufficient for behavior change, explored the relationships between variables and implementation outcomes, and initiated randomized controlled trials to test implementation strategies. This research has identified targets related to implementation (e.g., clinician motivation, organizational culture) and demonstrated the feasibility of activating these targets through implementation strategies. However, the dominant methodological approach has been atheoretical and predictive, relying heavily on a set of variables from heuristic frameworks. Conclusions Optimizing the implementation of effective treatments in community care for youth with psychiatric disorders is a defining challenge of our time. This review proposes a new direction focused on developing and testing integrated causal theories. We recommend implementation scientists: (a) move from observational studies of implementation barriers and facilitators to trials that include causal theory; (b) identify a core set of implementation determinants; (c) conduct trials of implementation strategies with clear targets, mechanisms, and outcomes; (d) ensure that behaviors that are core to EBPs are clearly defined; and (e) agree upon standard measures. This agenda will help fulfill the promise of evidence-based practice for improving youth behavioral health.

Journal of Child Psychology and Psychiatry, 2012Co-Authors: Joel T NiggAbstract:As the incoming Research Reviews Editor for The Journal of Child Psychology and Psychiatry (JCPP), it is a privilege to participate in helping a flagship international journal to advance knowledge and practice through well-targeted reviews. Readers may be aware that JCPP has three kinds of review outlets. Practitioner Reviews are directed at applied questions, and are handled by Jonathan Green. Annual Research Review issues (currently under the direction of James Leckman) pull together expert reviews and commentary, often on an annual theme. Finally, the regular Research Reviews are published throughout the year (approximately every other issue). Most reviews are commissioned (invited), because this allows us to balance the topical areas year-to-year and ensure that the overall review-set is coherent. Nonetheless, we occasionally consider excellent proposals from authors that happen to coincide with that year’s goals and priorities, or which show us that we have missed a critical and timely topic in our planning.
