Our results indicate that only phospholipase production, induced by egg yolk substrate, was useful for the differentiation of the taxonomically related species studied, based on their enzymatic profile

The medullary lesion biopsy revealed phaeohyphomycosis caused by Cladophialophora species. Myelitis caused by Cladophialophora bantiana is a rare disease associated with high mortality.

Il s’agit d’une patiente de 35 ans, suivie aux Hospices Civils de Lyon pour une mucoviscidose, diabetique insulinodependante, greffee bipulmonaire en 2003 avec insuffisance renale post-transplantation. Ses antecedents infectieux recents comportent un episode de grippe severe en mars 2013 avec complications pulmonaires (notamment infection a Absidia sp., traite par Ambisome), renales et hepatiques d’evolution favorable apres 4 mois de Reanimation. Une paraplegie progressivement complete apparait en automne 2013, avec decouverte a l’IRM d’une lesion expansive situee en regard de T10, T11, T12 mesuree a 40 × 10 mm et d’une lesion cerebelleuse gauche. Finalement une biopsie medullaire sur la lesion medullaire identifiee revelera de tres nombreux myceliums septes a l’examen direct. Les cultures sur milieu de Sabouraud montraient des colonies noires floconneuses. L’aspect microscopique etait compatible avec Cladophialophora sp., l’espece bantiana etant suspectee du fait de la localisation. Le sequencage confirmait l’identification de Cladophialophora bantiana . Dans ce contexte d’immunodepression le traitement antifongique instaure reposait sur une triple association d’Ancotil®, d’Ambisome® et de Noxafil® longue duree. Malgre une diminution de la lesion a l’IRM, aucune recuperation n’etait observee. La bonne tolerance au traitement et la stabilite de l’etat de la patiente permit neanmoins une hospitalisation a domicile. La patiente decedera un an plus tard des suites d’un accident vasculaire cerebral. Cladophialophora bantiana est classiquement associe a des abces cerebraux mais nous rapportons ici, a notre connaissance le deuxieme cas de myelite. Pres de 80 % des cas surviennent chez des patients immunocompetents, majoritairement en zones subtropicales. La patiente ne presentait aucune notion de voyage, suggerant une acquisition en France. Avec un taux de survie inferieur a 30 %, les phaeo-hyphomycoses a C. bantiana sont mortelles, le cas rapporte ici est exceptionnel de par la survie prolongee de la patiente sous triple association d’antifongiques mais aussi de par sa localisation.

Cladophialophora bantiana brain abscess in a solid organ transplant recipient case report and review of the literature

We describe a heart and bilateral lung transplant recipient who was unsuccessfully treated for a C. bantiana brain abscess. This report compares the present case to those of other solid-organ transplant recipients with the same infection and to those of patients who did not receive transplants.
