    Abstract:

    This study aimed to obtain the optimum concentration of Sappan Wood Extract, Quartz Sand and Clamshell mixture. For this purpose, Response Surface Methodology (RSM) was used to optimize the factors which influenced the quality of rubber compound. There were two factors to be observed in this study. They were the concentration of Sappan ood Extract (8 phr, 9 phr and 10 phr) and the mixture Quartz Sand and Clamshell (63:37, 73:27 and 83:17). The observed parameters were the hardness, the tensile strength and the elongation at break. The results of the study showed that the optimum hardness was reached at the treatment combination of 8.55 phr for the Sappan Wood Extract and 77.7:22.3 for the mixture of Quartz Sand and Clamshell mixture. Furthermore, the optimum tensile strenght was obtained at the treatment combination of 8.48 phr for the Sappan Wood Extract and 60.3:39.7 for the mixture of quartz sand and seashell. Finally, the optimum elongation at break was reached at the treatment combinatioan of 9,55 phr for the sappan wood extract and 70.65:29.35 for the mixture of of quartz sand and Clamshell. Based on these three parameters, the optimum usage of sappan wood extract, quartz sand and Clamshell mixture in the making of rubber compound was reached in the proportion of 8.48 phr–9.55 phr for the sappan wood extract, and 60:39.7; 77.7:22.3 for the mixture of quartz sand and Clamshell.ABSTRAKPenelitian ini bertujuan untuk mendapatkan konsentrasi ekstrak kayu secang dan campuran pasir kuarsa dan kulit kerang yang optimum. While DLT was historically performed via Clamshell thoracotomy, recently the median sternotomy has emerged as a viable alternative. As the ideal surgical approach remains unclear, the aim of our study was to compare the short and long-term outcomes of these two surgical approaches in DLT. Methods We retrospectively reviewed 192 consecutive adult patients who underwent primary DLT at our institution between 2012 and 2017 (Sternotomy, n=147; Clamshell, n=45). The impact of each surgical approach on post-transplant morbidity and overall survival probability analysis was performed. Results There were no significant differences in recipients’ baseline and donors’ characteristics and bilateral allograft ischemic time (Table). Freedom from primary graft dysfunction, acute rejection episode, post-operative prolonged ventilator support, tracheostomy, postoperative stroke, and airway dehiscence was comparable between these two groups (p=NS for all). Methods We retrospectively reviewed 192 consecutive adult patients who underwent primary DLT at our institution between 2012 and 2017 (sternotomy, n = 147; Clamshell, n = 45). The impact of each surgical approach on post-transplant morbidity was investigated, and the overall survival probability analyses were performed. Results There were no significant differences in recipients’ baseline and donors’ characteristics and bilateral allograft ischemic time. Freedom from primary graft dysfunction, acute rejection episodes, postoperative prolonged ventilator support, tracheostomy, postoperative stroke, and airway dehiscence were comparable between these 2 groups. The duration of cardiopulmonary bypass and operative time were significantly longer in the Clamshell thoracotomy group. Postoperative extracorporeal membrane oxygenation usage tended to be more frequent in the Clamshell thoracotomy group than the median sternotomy group, despite no statistical significance. Length of hospital and intensive care unit stay were not influenced by the type of incision. There was no significant difference in overall survival between these 2 procedure groups (P = .61, log-rank test). Conclusions The median sternotomy approach in DLT decreases operative time and more importantly leads to a shorter duration of cardiopulmonary bypass. The type of surgical approach did not show any statistically significant impact on adult DLT recipients’ morbidity and survival.

    Intisari- Clamshell electro hydraulic grab adalah jenis grab yang menggunakan sistem hydraulic sebagai penggeraknya yang mana sistem hydraulic tersebut digerakan oleh elektro motor yang sumbernya berasal dari deck crane,Clamshell electro hydraulic grab digunakan sebagai alat bongkar muat pada kapal curah,fungsi dari Clamshell electro hydraulic grab adalah sebagai penggaruk muatan lalu memindahkan muatan dengan bantuan crane.Di MV.KT 02 tampat peneliti melekukan praktik,terdapat empat buah clamshel electro hydraulic grab, Metode penelitian ini menggunakan metode fishbone dan SHELL. Sumber data yang dikumpulkan dalam penyusunan skripsi ini diperoleh dari sumber data primer dan sumber data sekunder. Metode pengumpulan dan penarikan data menggunakan teknik observasi, wawancara, studi pusaka dan studi dokumentasi.Dari hasil penelitian yang penulis lakukan, maka ditemukan faktor-faktor penyebab kurang optimalnya perawatan Clamshell electro hydraulic grab antara lain : perawatan yang tidak sesuai prosedure.Clamshell electro hydraulic grab yang sudah tua mengakibatkan sering terjadi kerusakan seperti pipa hight pressure yang bocor, joint point conector yang berkarat, terbakarnya elektro motor, serta tidak tersedianya spare part. Tindakan yang dilakukan untuk mengatasi agar Clamshell electro hydraulic grab berfungsi secara maksimal yaitu melakukan perawatan secara berkala, tersedianya spare part. Hasil penelitian yang di lakukan peneliti dapat disimpulkan yaitu melakukan perawatan Clamshell electro hydraulic grab secara terjadwal serta tersedianya spare part di kapal dapat meminimalisir terjadinya kerurakan pada Clamshell electro hydraulic grab proses bongkar muat dapat berjalan dengan lancar. Perencanaan pembentukan organisasi kerja yang dilakukan pada safety meeting sebelum melaksanakan pekerjaan, peran perwira dalam mengawasi semua pekerjaan, serta peralatan yang dapat menunjang kelancaran seluruh rangkaian pekerjaa
