    Este trabajo deriva del Proyecto de Investigación Consolidado de la Universidad Nacional de San Luis: “El climaterio femenino. Una indagación de los procesos de simbolización de esta compleja experiencia emocional en mujeres de la ciudad de San Luis”.El marco teórico está constituido por las conceptualizaciones de Bion y Meltzer quienes revisan el concepto de identificación proyectiva de Melanie Klein y postulan la identificación proyectiva realista, hipertrófica, identificación adhesiva e identificación introyectiva.El objetivo general de este artículo es indagar en un grupo de mujeres que transitan la etapa del climaterio las mencionadas modalidades identificatorias. En este recorte se analiza desde un enfoque cualitativo, a través de entrevistas clínicas en profundidad, el material que brindan16 sujetos que integran una muestra total de 50 mujeres climatéricas.Se parte de la conjetura, en base a estudios previos realizados por el equipo de investigación y en acuerdo con otros autores especialistas en el tema, que el climaterio constituye un proceso de intensos cambios por las profundas transformaciones y la turbulencia emocional implicada.Se advierte que en trece de las dieciséis mujeres predomina el uso de identificaciones proyectivas hipertróficas e identificaciones adhesivas. Las tres restantes implementan identificaciones proyectivas realistas e identificaciones introyectivas.The present work derives from the UNSL (Universidad Nacional de San Luis) consolidated research project, “Female Climacterium. A Study of the Symbolization Processes of this Complex Emotional Experience in Women from San Luis”.The theoretical framework arises from Bion’s and Meltzer’s conceptualizations, who revise the concept of projective, realistic, hypertrophic identification, adhesive identification, and introjective identification.The general objective of this research work is to study a group of women who are going through Climacterium in the identificatory modes mentioned above. The analysis is done from a qualitative perspective, through comprehensive clinical interviews comprising the data obtained from 16 out of a total 50 women who make up the sample.Our belief that Climacterium is a process involving intense changes due to the deep transformations and the emotional turbulence characteristic of this stage takes into account previous works carried out by this research group, as well as those of other authors specialized in the topic.The results indicate that there is a predominant use of projective hypertrophic and adhesive identifications in thirteen out of 16 sixteen women.     Abstract:

    Aim To analyze the influence of the duration of habitual physical activity (PA) on the symptoms of Climacterium/menopause and on several domains of the health-related quality of life (QOL) in middle-aged women. Methods One hundred and four 45- to 59-year-old women were placed into three groups: group A, subjects who maintained PA less than 30 minutes/day; group B, subjects who maintained or began to perform PA 30-60 minutes/day; and group C, subjects who maintained or increased PA to more than 60 minutes/day. Symptoms of menopause, QOL (physical, psychological, and social), and PA were assessed through the Kupperman Menopausal Index, World Health Organization Quality of Life Brief Version questionnaire, and International Physical Activity Questionnaire, respectively. Results The analysis of covariance (ANCOVA) results, adjusted for age, initial body mass index, schooling years, hormonal replacement therapy, and the number of diseases, indicated that the women who maintained or increased their total habitual PA to more than 60 minutes/day had reduced symptoms of Climacterium/menopause (-5.4 ± 0.5; P = 0.001) and improved QOL in the psychological (4.4% ± 0.8%; P = 0.001) and social domains (2.0% ± 0.9%; P = 0.035). ANCOVA revealed a further improvement of approximately 5% in the psychological domain of QOL in group C, who also experienced decreased menopause symptoms (P = 0.001) and lost weight (P = 0.009). Conclusion The habitual practice of at least moderate-intensity PA for 60 minutes/day has a favorable effect on Climacterium/menopause symptoms and on QOL, particularly on its psychological and social domains. The influence of habitual PA at the psychological level seems to be at least partially associated with a decrease in menopause symptoms and/or weight loss.

    Abstract:

    Objective: identifying the instruments used to measure quality of life during menopausal transition and post-menopause in women with obesity or overweight. Methods: a systematic search using the Embase, Pubmed and Cochrane databases, and the following key-words: menopause/climacteric, quality of life, overweight/obesity. Nineteen papers that fulfilled the including criteria were found. Results: eighteen studies using generic health questionnaires (global quality of life) were identified, six of them were specific for menopause and one specific for obesity. Eleven studies used generic questionnaires, while only eight questionnaires were specific for menopause. Conclusion: there was no consensus in the literature about the use of quality of life questionnaires in women with overweight and obesity in Climacterium, which makes de comparison and reproducibility of the results difficult. This is an alert about the need for standardization to better evaluate this specific population.

    Abstract:

    Objective The aim of this study was to construct and to validate a measure of the consequences of domestic violence on women's health during Climacterium. Methods A questionnaire was administered at the Outpatient Climacterium Clinic to 124 women aged 40 to 65 years who were the victims of domestic and/or sexual violence (experimental group). They were divided into three groups: (1) those who were victims of violence exclusively during childhood/adolescence, (2) those who were victims of violence exclusively during adulthood, and (3) those who were victims of violence throughout their lives. The instrument included 34 items evaluating the beginning, frequency, and type of violence; the search for health assistance and reporting of the violence; the violence and the number of comorbidities; and violence and the Kupperman Menopausal Index. We also included a control group composed of perimenopausal and postmenopausal women who did not experience any violence (n = 120). Results The instrument presented a Cronbach α = 0.82, good reliability among the examiners (+0.80), and a good possibility of reproducibility. The mean age of menopause was 45.4 years, and the mean age in the control group was 48.1 years. Group 1 showed a mean of 5.1 comorbidities, Group 2 had 4.6, and Group 3 had 4.4. Sexual violence (43.5%) and other types of violence both presented average comorbidities (4.60) but represented a significant impairment in the victim's sexual life. There were significant associations in group 3 and a high Kupperman Menopausal Index score. In the experimental group, 80.6% did not seek health services for the violence they experienced. Conclusions The questionnaire presented good internal consistency and a validated construction. It can be easily reproduced and is indicated to evaluate the consequences of domestic and/or sexual violence on women's health during Climacterium.

    Abstract:

    OBJECTIVE: Most postmenopausal women have insomnia. Some of these women also have respiratory sleep disorders. Recent reports have documented that the phytohormones, isoflavones, are capable of reducing the symptoms of Climacterium. The purpose of this investigation was to examine subjective and objective sleep parameters and to measure changes in these parameters during treatment with isoflavones in a controlled, double-blinded study in postmenopausal women with insomnia. METHODS: Two groups of postmenopausal women with insomnia participated in the study: the first received 80 mg isoflavones daily for 4 months, and the second received a placebo for the same period. Sleep analysis consisted of questionnaires and polysomnography. Student's t test and analysis of variance were applied for comparisons between groups, and correlations were tested with Pearson's correlation coefficient. RESULTS: Thirty-eight women were enrolled in the study. Polysomnography revealed a significant increase in sleep efficiency in the isoflavone group (from 77.9% to 83.9%) when compared with the placebo group (from 77.6% to 81.2%). Isoflavones induced a decrease in the intensity and number of hot flashes and the frequency of insomnia: among the women in the placebo group, 94.7% had moderate or intense insomnia at the beginning of the study, compared with 63.2% at the end, whereas in the isoflavone group, these percentages were 89.5% and 36.9%, respectively. CONCLUSIONS: In postmenopausal women with insomnia, isoflavone treatment was effective in reducing insomnia symptoms, which was confirmed by increased sleep efficiency as observed by polysomnographic analysis.

    Abstract:

    Recently, middle-aged men who have begun frequently complaining of erectile dysfunction and nonspecific symptoms similar to those of postmenopausal women, visited a male Climacterium clinic in Japan. Some patients, who were already taking antihypertensive medication, discontinued or reduced their dosages of antihypertensive medication after anti-depressant therapy. Forty-nine males over the age of 40 years were studied to evaluate the relationships between blood pressure, mental stress, and testosterone levels. The systemic blood pressure (sBP) of 24 patients was higher than the criteria for mild hypertension: 140/90 mmHg (HT group) at first visit. The sBP of the other 25 patients was normal (N group). The international index of erectile function (IIEF5) score (normal >21), self-rating depression scale (SDS) score (normal <40), and plasma testosterone levels were also evaluated before and after anti-depressant therapy without androgen replacement therapy.There were no significant differences between the groups in IIEF5 or SDS scores. The plasma testosterone levels in the HT group at first visit were significantly lower than those in the N group (230±77 vs. 343±92 ng/dL, p<0.001). After treatment, the IIEF5 scores were unchanged, whereas SDS scores were lower in both groups. Mean systemic blood pressure (mBP) in the HT group significantly decreased from 112±7 to 94±7 mmHg after treatment, concomitant with the disappearance of nonspecific complaints and the increase of testosterone levels. In the N group, however, neither mBP nor testosterone levels changed. Psychotherapy can ameliorate mild systemic hypertension in climacteric men with low testosterone levels. Mental stress might suppress the hypothalamic-pituitary-gonadal axis to decrease testosterone levels.
