The purpose of this study was to determine the relationships among the data obtained through health department inspections, the contributing factors to foodborne illness identified by the Centers for Disease Control and Prevention, and the risks of outbreaks of norovirus, Salmonella, and Clostridium Perfringens Infection associated with a specific restaurant. These agents were chosen for the analysis because they cause the majority of foodborne illnesses. A theoretical predictive assessment tool was built that extracts data from routine health department inspection reports for specific restaurants to establish a risk profile for each restaurant and identify the likelihood of a norovirus, Salmonella, or C. In an attempt to relate the data collected during inspections in Minnesota to illness likelihood, data from routine inspections conducted at outbreak restaurants were compared with data from routine inspections conducted at nonoutbreak restaurants. The goal was to identify differences in recorded violations. Significantly more violations were recorded at restaurants that had outbreaks. The majority of these violations were related to contamination in the facility and environment and to food handling procedures. Relative risks also were calculated for violations significantly more likely to occur at locations that had outbreaks of norovirus Infection, Clostridium Perfringens Infection or toxin-type illness, and Salmonella Infection. Pathogenesis of Clostridium Perfringens Infection: Mechanisms and Mediators of Shock

BryantAbstract:Shock, a common complication of gas gangrene caused by Clostridium Perfringens, is related to the elaboration of α- and θ-toxins in vivo. This study compared the relative potencies of θ- and α-toxins in a rabbit model and determined the role of endogenous mediators of toxin-induced shock. α-Toxin decreased cardiac index, mean arterial pressure, and heart rate and caused death in 83% of animals. θ-Toxin did not alter these parameters and caused death in only 25% of animals. α-Toxin also inhibited ex vivo cardiac contractility in a dose-dependent manner. Finally, both α-and θ-toxins were potent inducers of endothelial cell-derived platelet activating factor. α-Toxin, but not θ-toxin, also stimulated production of tumor necrosis factor by peripheral blood mononuclear cells. Thus, the fulminant nature of shock in patients with gas gangrene caused by C. Perfringens is the sum of α-toxin's direct effects on myocardial contractility and both toxins' ability to induce production of potent endogenous mediators.

Clostridium Perfringens Infection after transarterial chemoembolization for large hepatocellular carcinoma
World journal of gastroenterology, 2015Co-Authors: Rong-rong Yao, Hujia Shen, Lan Zhang, Xiaoying Xie, Rong-xin Chen, Yan-hong Wang, Zhenggang RenAbstract:We report an unusual case of Clostridium Perfringens liver abscess formation after transcatheter arterial chemoembolization (TACE) for large hepatocellular carcinoma. Severe deterioration in liver and renal function accompanied with hemocytolysis was found on the 2(nd) day after TACE. Blood culture found Clostridium Perfringens and abdominal computed tomography revealed a gas-containing abscess in the liver. Following antibiotics administration and support care, the Infection was controlled and the liver and renal function turned normal. The 2(nd) TACE procedure was performed 1.5 mo later and no recurrent Clostridium Perfringens Infection was found.

CNS Clostridium Perfringens Infection: A Rare Complication of Preoperative Embolization of Meningioma

HsuAbstract:Gas gangrene is a severe form of gangrene (tissue death) that usually is caused by Clostridium Perfringens. It generally occurs at the site of trauma or a recent surgical wound. We report the case of a 45-year-old woman with sphenoid-ridged meningioma who received preoperative transarterial embolization with polyvinyl alcohol. The patient later developed an intratumoral C Perfringens Infection and died despite intensive medical care. The case represents an extremely rare complication following transarterial embolization of meningioma.
