    Abstract:

    Latar Belakang: Burning mouth syndrome ( BMS ) didefinisikan sebagai rasa sa kit seperti terbakar yang melibatkan lidah atau membrana mukosa, biasanya secara klinis maupun laboratoris tidak dijumpai adanya kelainan. Keadaan ini sering dihubungkan dengan kondisi menopause, hiposalivasi Coated Tongue dan kondisi psikologis. Tujuan: Melaporkan kasus burning mouth syndrome dengan hiposalivasi, Coated Tongue dan gangguan pengecapan pada wanita menopause serta penatalaksanaannya. Kasus dan penatalaksanaannya: Kasus BMS pada wan ita postmenopausal disertai hiposalivasi, Coated Tongue dan gangguan pengecapan yang diatasi dengan medikasi gabapentin 100 mg, permenkaret probiotik, diazepam 2mg dan vitamin B1, B6 dan B12 sekali sehari cukup efektif mengatasi BMS. Kesimpulan: Keluhan BMS banyak dijumpai pada wanita, khususnya wanita postmenopause. Penderita biasanya mempunyai keluhan oral multipel yaitu, burning, mulut kering dan gangguan pengecapan. Perawatan dengan gabapentin, diazepam, permen karet probiotik bebas gula cukup efektif. Maj Ked GiJuni 201219(1): 82-8

    Abstract:

    Blackground: Burning mouth syndrome (BMS) is a disorder that is characterized by a burning sensation of the oral cavity in the absence of visible local or systemic abnormalities. Affected patient often present with multiple oral complaints, including burning, dryness and taste alterations. The exact cause of burning mouth syndrome often is difficult to pin point. Conditions that have been reported in association with burning mouth syndrome include menopause, hyposlivation, Coated Tongue, taste alterations and psychologic condition. Objective: To report a case of burning mouth syndrome in postmenopausal women with hyposalivation, Coated Tongue, taste alterations and psychologic condition and its management. Case and management: a case of burning mouth syndrome in women with menopause, hyposalivation, Coated Tongue, and taste alterations, was managed effectively by gabapentin 100mg, probiotic chewing gum, diazepam 2mg and vitamin B1, B6, B12. Conclusion: Oral burning appears to be most prevalent in postmenopausal women often present with multiple oral complaints, including burning, dryness and taste alterations, in this case was managed effectively by gabapentin 100mg, prebiotic chewing gum, diazepam 2mg and vitamin B1, B6, B12

    O conhecimento epidemiológico regionalizado das doenças bucais nas populações é essencial para um planejamento efetivo na prevenção destas doenças. No Brasil são poucas as pesquisas sobre etiologia e prevalência das lesões bucais em crianças com deficiências. Portanto, a proposta deste estudo clínico epidemiológico descritivo foi analisar as principais variações de normalidade e patologias bucais em 293 crianças com deficiência na faixa etária de zero a 12 anos de idade, atendidas em primeira consulta no CEOPE - Centro Estadual de Odontologia para Pacientes Especiais do Mato Grosso - Brasil, no ano de 2010, cujo projeto foi aprovado pelo Comitê de Ética em Pesquisa da Escola de Saúde Pública/ SES – MT (n°441/09). As variáveis dependentes analisadas foram cárie dentária, alterações gengivais, variações de normalidade da cavidade bucal, patologias bucais e anormalidades dentárias. As variáveis independentes foram: idade, sexo, raça, patologia de base, município de residência, perfil demográfico do cuidador (idade, renda familiar mensal, escolaridade). O estudo foi dividido em dois capítulos, no primeiro, avaliou-se as crianças com deficiência de zero a cinco anos (n=105 crianças). Dentre estas, 71,5% delas possuíam idade igual ou superior a 3 anos, 52,4% eram do sexo masculino e em relação à raça, 62,9% eram pardas. Quanto as variáveis idade, nível de escolaridade e renda familiar do cuidador, 52,4% eram maiores que 30 anos, possuíam ensino médio completo e 60% dos participantes relataram renda familiar entre 1 e 2 salários mínimos. Quanto a patologia de base, as mais frequentes foram as más-formações congênitas, deformidades e anomalias cromossômicas (34,3%). Therefore, the purpose of this clinical descriptive epidemiological study was to analyze the main buccal variations of normality and oral pathologies in 293 disabled children aged zero to 12 years of age, attending the first appointment at CEOPE - State Center of Dentistry for Special Patients of Mato Grosso - Brazil, in 2010, whose project was approved by the Ethics Committee of the School of Public Health / SES - MT (no. 441/09). The dependent variables analyzed were dental caries, gingival alterations, normal variations of the oral cavity, oral pathologies and dental anomalies. The independent variables were age, sex, race, underlying pathology, city of residence, caregiver demographic profile (age, family income, education). The study was divided into two chapters, in the first, we evaluated children with disabilities aged zero to five years (n = 105 children). Among these, 71.5% of them were aged less than 3 years, 52.4% were male and in relation to race, 62.9% were brown. From the variables age, education level and family income of the caregiver, 52.4% were older than 30 years had high school and 60% reported family income between 1 and 2 minimum wages. As the underlying pathology, the most frequent were congenital malformations, deformations and chromosomal abnormalities (34.3%). From these 105 children assessed, it was found 81 buccal variations of normality, the most common being a Coated Tongue (48.1%).     Long standing hyperglycaemia besides damaging the kidneys, eyes, nerves, blood vessels, heart, can also impair the function of the salivary glands leading to a reduction in the salivary flow. When salivary flow decreases, as a consequence of an acute hyperglycaemia, many buccal or oral alterations can occur such as: a) increased concentration of mucin and glucose; b) impaired production and/or action of many antimicrobial factors; c) absence of a metalloprotein called gustin, that contains zinc and is responsible for the constant maturation of taste papillae; d) bad taste; e) oral candidiasis f) increased cells exfoliation after contact, because of poor lubrication; g) increased proliferation of pathogenic microorganisms; h) Coated Tongue; i) halitosis; and many others may occur as a consequence of chronic hyperglycaemia: a) Tongue alterations, generally a burning mouth; b) periodontal disease; c) white spots due to demineralization in the teeth; d) caries; e) delayed healing of wounds; f) greater tendency to infections; g) lichen planus; h) mucosa ulcerations. Buccal alterations found in diabetic patients, although not specific of this disease, have its incidence and progression increased when an inadequate glycaemic control is present
