Skin sparing mastectomy and immediate breast reconstruction
Chinese Journal of Reparative and Reconstructive Surgery, 2004Co-Authors: Wu Y, Wang LAbstract:OBJECTIVE: To investigate the results of skin sparing mastectomy and immediate breast reconstruction with transverse rectus abdomins musculotaneous (TRAM) flap or latissimus dorsi musculocutaneous flap plus placement of a mammary implant. METHODS: From June 1997 to June 2002, 11 patients were proven to have ductal carcinoma in situ or huge breast carcinoid by pathological examination. The site of the biopsy incision was around the areola. The patients underwent mastectomy with skin sparing by a circumareolar incision and immediate breast reconstruction with TRAM flap or latissimus dorsi musculocutaneous flap plus placement of mammary implant. Autogenous tissue was used to fill the skin envelop. The second stage operation of nipple-areola reconstruction was performed on the replaced skin. RESULTS: Eleven patients were followed up 1 month to 6 years. The operative result was good and all patients had no relapse. The reconstructed breast achieved good results in shape, Colour, Sensation, symmetry and incision scar. CONCLUSION: The skin sparing mastectomy and immediate autograft tissue breast reconstruction is an ideal reconstructive method for the patients with breast ductal carcinoma in situ or huge breast carcinoid in condition that there were strict operative indication and relapse can be prevented.

Skin sparing incision mastectomy and immediate TRAM flap breast reconstruction
Chinese journal of plastic surgery, 2000Co-Authors: Liu L, Pand RAbstract:Objective In order to achieve a high\|quality reconstructed breast, we improved the operation method using the skin sparing incision mastectomy and immediate TRAM flap breast reconstruction. Methods Patients with duct carcinoma in situ were selected to undergo mastectomy with a skin sparing incision. The site of the previous biopsy incision was marked around the areola, and then the preferred skin sparing incision included the nipple\|areola complex and a margin around the biopsy site for the non\|ptotic breast. The excised skin was replaced as precisely as possible with the TRAM flap skin immediately. While the mastectomy was being performed the TRAM flap was incised and elevated. The pedicled or free TRAM flap has been used for 34 patients. Nipple\|areola reconstruction was made at the second stage. Results The results of the skin sparing breast reconstruction are better than other breast reconstruction techniques in the breast's shape,Colour,Sensation,symmetry and incision scar. Conclusions Skin sparing incision mastectomy with immediate TRAM flap breast reconstruction technique is an ideal breast reconstructive method for the patients with breast duct carcinoma in situ. The advantages of the method include ① The TRAM flap is an autograft without rejection; ② The reconstructed breast has good shape and better Sensation than other autograft reconstructive techniques since most breast skin has been spared; ③ The symmetry of the reconstructed breast with the contralateral breast is improved.

Colour Sensation and empathy arthur schopenhauer s negative philosophy and Colour theory as the basis of an elemental Colour architecture
Architectural Theory Review, 2014Co-Authors: V A WatsonAbstract:Arthur Schopenhauer proposed a theory of Colour as a consequence of his first hand knowledge of J.W. Goethe’s experiments with color phenomena. This Colour theory can be used to explore an interesting proposition Schopenhauer made about architecture. For Schopenhauer, architecture is about feelings, not about functions or forms, its purpose as an art is to reveal the principles of primitive forces, specifically gravity and rigidity. For Schopenhauer, architecture expresses these forces in the poised equilibrium of massive structures built out of stone. Schopenhauer was inclined to believed that architecture had already achieved its most perfect expression in Greek temple architecture. However; he did offer one possibility for architectural research: this was the suggestion that architecture was also concerned with the expression of light. It seems never to have occurred to Schopenhauer to use his Colour theory to speculate about light in architecture. This paper explores some of the implications of Schopenhauer’s theory of Colour for his aesthetics of architecture?
