    Abstract:

    Abstract Science Communication is increasingly important for scientists, although research, teaching and administration activities tend to eat up our time already, and budgets for science Communication are usually low. It appears impossible to combine all these tasks and, in addition, to develop engagement activities to a quality and impact that would make the efforts worth their while. Here we argue that these challenges are easier addressed when centering science Communication initiatives on a long-term vision with a view to eventually forming outreach networks where the load can be shared whilst being driven to higher momentum. As one example, we explain the science Communication initiative of the Manchester Fly Facility. It aims to promote public awareness of research using the model organism Drosophila , which is a timely, economic and most efficient experimental strategy to drive discovery processes in the biomedical sciences and must have a firm place in the portfolios of funding organisations. Although this initiative by the Manchester Fly Facility is sustained on a low budget, its long-term vision has allowed gradual development into a multifaceted initiative: (1) targeting university students via resources and strategies for the advanced training in fly genetics; (2) targeting the general public via science fairs, educational YouTube videos, school visits, teacher seminars and the droso4schools project; (3) disseminating and marketing strategies and resources to the public as well as fellow scientists via dedicated websites, blogs, journal articles, conference presentations and workshops − with a view to gradually forming networks of drosophilists that will have a greater potential to drive the science Communication Objective to momentum and impact. Here we explain the rationales and implementation strategies for our various science Communication activities − which are similarly applicable to other model animals and other areas of academic science − and share our experiences and resources to provide ideas and readily available means to those who are actively engaging or intend to do so.

    Abstract:

    The paper describes results on two components of a research program focused on motion-based Communication mediated by the dynamics of a control system. Specifically we are interested in how mobile agents engaged in a shared activity such as dance can use motion as a medium for transmitting certain types of messages. The first part of the paper adopts the terminology of motion description languages and deconstructs an elementary form of the well-known popular dance, Salsa, in terms of four motion primitives (dance steps). Several notions of dance complexity are introduced. We describe an experiment in which ten performances by an actual pair of dancers are evaluated by judges and then compared in terms of proposed complexity metrics. An energy metric is also defined. Values of this metric are obtained by summing the lengths of motion segments executed by wheeled robots replicating the movements of the human dancers in each of the ten dance performances. Of all the metrics that are considered in this experiment, energy is the most closely correlated with the human judges' assessments of performance quality. The second part of the paper poses a general class of dual Objective motion control problems in which a primary Objective (artistic execution of a dance step or efficient movement toward a specified terminal state) is combined with a Communication Objective. Solutions of varying degrees of explicitness can be given in several classes of problems of communicating through the dynamics of finite dimensional linear control systems. In this setting it is shown that the cost of adding a Communication component to motions that steer a system between prescribed pairs of states is independent of those states. At the same time, the optimal encoding problem itself is shown to be a problem of packing geometric objects, and it remains open. Current research is aimed at solving such Communication-through-action problems in the context of the motion control of mobile robots.

    Importance Clinician Communication about goals of care is associated with improved patient outcomes and reduced intensity of end-of-life care, but it is unclear whether interventions can improve this Communication. Objective To evaluate the efficacy of a patient-specific preconversation Communication-priming intervention (Jumpstart-Tips) targeting both patients and clinicians and designed to increase goals-of-care conversations compared with usual care. Design, Setting, and Participants Multicenter cluster-randomized trial in outpatient clinics with physicians or nurse practitioners and patients with serious illness. The study was conducted between 2012 and 2016. Interventions Clinicians were randomized to the bilateral, preconversation, Communication-priming intervention (n = 65) or usual care (n = 67), with 249 patients assigned to the intervention and 288 to usual care. Main Outcomes and Measures The primary outcome was patient-reported occurrence of a goals-of-care conversation during a target outpatient visit. Secondary outcomes included clinician documentation of a goals-of-care conversation in the medical record and patient-reported quality of Communication (Quality of Communication questionnaire [QOC]; 4-indicator latent construct) at 2 weeks, as well as patient assessments of goal-concordant care at 3 months and patient-reported symptoms of depression (8-item Patient Health Questionnaire; PHQ-8) and anxiety (7-item Generalized Anxiety Disorder survey; GAD-7) at 3 and 6 months. Analyses were clustered by clinician and adjusted for confounders. Results We enrolled 132 of 485 potentially eligible clinicians (27% participation; 71 women [53.8%]; mean [SD] age, 47.1 [9.6] years) and 537 of 917 eligible patients (59% participation; 256 women [47.7%]; mean [SD] age, 73.4 [12.7] years). The intervention was associated with a significant increase in a goals-of-care discussion at the target visit (74% vs 31%; P P P  = .01). Patient-assessed goal-concordant care did not increase significantly overall (70% vs 57%; P  = .08) but did increase for patients with stable goals between 3-month follow-up and last prior assessment (73% vs 57%; P  = .03). Symptoms of depression or anxiety were not different between groups at 3 or 6 months. Conclusions and Relevance This intervention increased the occurrence, documentation, and quality of goals-of-care Communication during routine outpatient visits and increased goal-concordant care at 3 months among patients with stable goals, with no change in symptoms of anxiety or depression. Understanding the effect on subsequent health care delivery will require additional study. Trial Registration ClinicalTrials.gov identifier:NCT01933789

    Background: Improved Communication with family members of critically ill patients can decrease the prolongation of dying in the intensive care unit (ICU), but few data exist to guide the conduct of this Communication. Objective: Our Objective was to identify missed opportunities for physicians to provide support for or information to family during family conferences. Methods: We identified ICU family conferences in four hospitals that included discussions about withdrawing life support or delivery of bad news. Fifty-one conferences were audiotaped, including 214 family members. Thirty-six physicians led the conferences and some physicians led more than one. We used qualitative methods to identify and categorize missed opportunities, defined as an occurrence when the physician had an opportunity to provide support or information to the family and did not. Main results: Fifteen family conferences (29%) had missed opportunities identified. These fell into three categories: opportunities to listen and respond...

    Importance Reviews of patient and caregiver experience with health practitioners are increasingly important to consumers. Understanding physician qualities valued by patients and their families may facilitate more effective care and Communication. Objective To identify physician qualities and care dimensions valued by parents during their child’s outpatient surgical care encounter. Design Mixed-methods analysis of comments from patient-satisfaction surveys. Setting Outpatient pediatric otolaryngology and surgical specialty clinics in an urban tertiary care center. Participants Parents of children seen in pediatric otolaryngology and surgical specialty clinics who completed the physician section of the patient satisfaction survey. Main Outcomes and Measures Free-text comments written in the physician section of the survey. Results A total of 195 survey comments from visits to 18 surgical specialty clinics were analyzed; 64 comments (33%) were global in nature, of which 58 (90%) were positive; 25 of 195 comments (13%) discussed recommendation of care. Of the 131 comments (67%) with specific content, 79 (60%) were positive. Major themes were physician interpersonal style (n = 78, 45%), physician technical skills/knowledge/preparation (n = 82, 47%), and systems issues (n = 14, 8%). The most commonly noted interpersonal subthemes were inclusion/interaction with the child (20%) and empathy/concern for the patient’s needs (29%). The most common skills themes related to whether the physician answered questions (16%) or explained treatment (23%). The most common negative subgroup was inadequate explanation of treatment (18%). The majority of comments related to systems were negative (13 of 14, 93%). Conclusions and Relevance Parents highly value physician interpersonal style and visit-specific skills (involving the child in discussion; showing empathy/concern for the patient/family; answering questions; explaining treatment). While most comments about surgeons were affirmative, clinical systems were more often negatively perceived. Attention to parent-reported experiences may help physicians improve Communication and overall effectiveness of care provided.
