    Abstract:

    L’orthopédie connaît son essor au début du XIXe siècle. Ceux qui étaient appelés les « boîteux » et les « bossus » voient apparaître des traitements se situant entre avancées scientifiques et progrès techniques. L’essentiel de leur prise en charge se fait par des moyens mécaniques, des « machines » constituées de lits et de fauteuils pourvus de systèmes de traction. François Humbert (1776-1850), médecin de campagne et collectionneur, fonde en 1817, à Morley (Meuse) le premier établissement orthopédique de France, qui accueille environ trois cents patients. Dans cet établissement éloigné des centres universitaires, il fut le premier à se pencher sur le traitement des luxations congénitales de la hanche, réputées incurables. Détaillées dans un ouvrage expliquant le fonctionnement de ces inventions, une quarantaine de maquettes de belle facture sont utilisées pour diffuser ces travaux novateurs et en favoriser la reconnaissance institutionnelle ainsi que la vulgarisation auprès du public. La collection a connu une histoire mouvementée avant de rejoindre le Musée barrois. Aujourd’hui, un projet de conservation préventive et de restauration s’élabore, grâce au partenariat entre le musée et une association. Au-delà d’un projet d’exposition célébrant le bicentenaire de la création de l’établissement de Morley, en 2017, une réflexion a été engagée à propos de l’inscription de l’ensemble, très spécifique, dans la muséographie des salles d’exposition permanente, en lien avec le patrimoine technique et scientifique meusien.The early nineteenth century can be considered as a turning point in orthopaedics. Supported by a book which explains the operation of his inventions, about forty well-made models constitute an element of communication about this innovative work, well worth better institutional recognition and accessibility to the public. The collection had a turbulent history before arriving in the Barrois museum in the city of Bar-le-Duc. Today, a project of preventive conservation and restoration is being developed through a partnersHip between the museum and an association. Beyond a project of an exhibition to celebrate the bicentennial of the creation of the Morley institution in 2017, reflection has been initiated concerning the inclusion of this highly specific collection in the museum presentation of the permanent exhibition rooms, in the broader context of the technical and scientific heritage of the Meuse department

    Abstract:

    Francois Humbert (1776-1850) created in 1817 the first French orthopaedic institution, at a great turning point in orthopedics. Interested in "lame people" and "hunchbacks", he treated Congenital Hip Dislocation and scoliosis, for about 30 years. Humbert's medical practice illustrated very well the deep transformation which occured in orthopedics at the beginning of the 19th century. As testimonies of Humbert's work, there are the books he published, his Memoirs, some buildings of his institution which have been reconverted into houses, but above all thirty-eight models of his "machines". In spite of the fact that he was the first to consider Congenital Hip Dislocation like a curable disease, his work quickly became unknown after his death.

    Abstract Midterm results of cementless total Hip arthroplasty in patients with Crowe type IV Congenital Dislocation of the Hip were evaluated. A modified oblique subtrochanteric shortening osteotomy was used in all patients. A cylindrical femoral stem was used in all patients to stabilize the osteotomy. Mean follow-up was 82 months in 20 Hips of 16 patients. Mean Merle D'Aubigne pain score increased from 2.52 to 5.65 points, function score improved from 4.0 to 5.3 points, and mobility score improved from 3.95 to 5.35. Mean greater trochanter height relative to the estimated Hip center was 6.8 ± 2.0 cm preoperatively and −1 ± 0.2 cm postoperatively. Complications were Dislocations in 3 patients, which were successfully managed without reDislocation and fracture of greater trochanter in 3 patients, which healed uneventfully in 2 but with residual Trendelenburg gait in one. Total Hip arthroplasty with modified oblique subtrochanteric shortening osteotomy is an effective technique for the treatment for Crowe type IV Hip Dislocation.

    Kasus dislokasi panggul kongenital (DDH) masih terjadi setelah usia berjalan karena terlambat atau tidak terdiagnosis. Pilihan pengobatan untuk DDH setelah usia berjalan adalah prosedur bedah telah menjadi tantangan pada bidang ortopaedi. Tujuan penelitian ini adalah menilai hasil klinis dan radiografi pengobatan bedah DDH setelah usia berjalan. Penelitian ini melibatkan 13 pasien (15 panggul) di Rumah Sakit Dr. Hasan Sadikin Bandung tahun 2012–2015 yang telah dilakukan tindakan operatif pada usia 1 sampai 7 tahun dengan tindakan reduksi terbuka, femoral osteotomi, dan osteotomi panggul. Hasil klinis akhir setelah follow up (rerata 22 bulan) memiliki hasil excellent 5 pasien, good 7 pasien, fair 2 pasien, dan poor satu pasien. Secara radiologi hasilnya adalah Kelas I (excellent) 8 pasien, Kelas II (good) 5 pasien, Kelas III (fair) satu pasien, dan kelas IV (poor) satu pasien. Hasil yang memuaskan adalah 10 pasien secara klinis dan 11 pasien (dinilai dari radiologi). Simpulan, hasil fungsional pasien DDH yang dilakukan perawatan setelah operasi di usia berjalan mayoritas memiliki hasil yang baik. The treatment of choice for DDH after walking age is surgical procedure, which has been a challenge in the orthopedic field. The aim of this study was to assess the clinical and radiographic results of surgical treatment of DDH after walking age. The study included 13 patients (15 Hips) who underwent surgeries at the age of 1 to 7 years with open reduction, femoral osteotomy, and pelvic osteotomy procedures  in Dr. Hasan Sadikin General Hospital Bandung from 2012–2015. The final clinical outcomes at the end of the follow up period (mean = 22 months) was excellent in 5 patients, good in 7 patients, fair in 2 patients, and poor in one patient. The final radiological results presented Class I (excellent) in 8 patients, Class II (good) in 5 patients, Class III (fair) in one patient, and Class IV (poor) in one patient. The number of patients that had satisfactory outcome was 10 patients clinically and 11 patients radiologically.
