An international analysis of Contact Lens compliance

NicholsAbstract:To evaluate compliance to Contact Lens use in 14 countries, and to identify wearer attributes which indicate lower compliance levels. Methods: A web-based survey was undertaken by 4021 Contact Lens wearers, revealing information about demographics, and Lens and solution information including daily handling details, case care and aftercare frequency. Particular attention was paid to eight modifiable compliance-related behaviours which are associated with an increased risk of microbial keratitis. Results: Full compliance was very rare for most Lens users, although better (15% of wearers) for daily disposable Lenses. Reduced compliance was demonstrated in South Korea, and for young male full-time Contact Lens users, especially those who have not consulted their eyecare practitioner for some time. The behaviours associated with the lowest levels of compliance were rubbing and rinsing, handwashing, correct Lens replacement and case cleaning. Conclusions: Given other recent literature, which suggests improved Contact Lens compliance with regular self-review exercises, we encourage clinical colleagues to particularly examine case cleaning, handwashing and rubbing and rinsing at aftercare examinations, especially in young male wearers. © 2011 British Contact Lens Association.

Data from our international Contact Lens prescribing survey were mined to assess recent trends in toric soft Contact Lens fitting. This survey was conducted by sending up to 1000 survey forms to Contact Lens fitters in Australia, Canada, Japan, the Netherlands, Norway, the UK and the USA each year between 2000 and 2009. Practitioners were asked to record data relating to the first 10 Contact Lens fits or refits performed after receiving the forms and to return them to us for analysis. The data revealed a gradual increase in the extent of toric soft Lens fitting this century. Excluding Japan – which had a consistently low rate of soft toric Lens fitting over the survey period – soft toric Lenses now represent over 35% of all soft Lenses prescribed; it can be assumed that, on average (and again excluding Japan), all cases of astigmatism 0.75 D or less remains uncorrected among Contact Lens wearers. Toric Lenses are fitted more to those who are older, full-time wearers and reusable Lens wearers, and less to those wearing silicone hydrogel and extended wear Lenses.

The survey collected information about present age and sex, history of Contact Lens wear, types of Lenses worn, age at starting wear, current wearing schedule (hours per day, days per week), self-perceived Contact Lens satisfaction, and Contact Lens-related problems. A variety of statistical analyses including analysis of variance, logistic regression, and repeated-measures logistic regression were used to model the data. Results Current or previous experience with Contact Lenses was reported by 453 (62%) of the subjects. Of these subjects, 119 (26.3%) reported that Contact Lenses were not the ideal form of visual correction for them (Contact Lens dissatisfaction) and another 109 (24.1%) had permanently discontinued Contact Lens wear. Dissatisfied Contact Lens wearers had reduced self-reported wearing times compared with satisfied Contact Lens wearers. Previous Lens wearers were more likely than current Lens wearers to be men, older (by approximately 9.5 years), have started Contact Lens wear at a later age (approximately 4-5 years later), and have tried either rigid or both soft and rigid Lenses. The primary self-reported reason for both Contact Lens dissatisfaction and discontinuation was ocular symptoms (dryness and discomfort), followed by preference for another corrective modality. Conclusion A significant number of Contact Lens wearers are not satisfied with Contact Lenses and are at risk for discontinuation.

Nichols, Loraine T SinnottAbstract:Purpose To examine tear film, Contact Lens, medical, and patient-related factors associated with self-reported Contact Lens-related dry eye. Methods Four hundred fifteen Contact Lens wearers were recruited and enrolled in this phase of a larger cross-sectional study. A variety of tear film (e.g., interferometry, osmolality, phenol red thread, meibography, fluorescein, and lissamine green staining), Contact Lens (i.e., water content, refractive index, material), and patient-related (e.g., gender, sociodemographic, education, income, and medical health) factors were examined in relation to dry eye status. Univariate and multivariate logistic regression models were used to examine the relation between these tear film, Contact Lens, and patient-related factors associated with dry eye status. Results Of the 415 enrolled, the data from 360 were used in the analyses. The average age was 31.1 +/- 11.5 years, 245 (68%) participants were female, and 55.3% were classified as having-Contact Lens-related dry eye via self-report. Overall, 327 (90.8%) were hydrogel Lens wearers and 33 (9.2%) were gas-permeable Lens wearers. Several factors were shown to be related to dry eye status in multivariate modeling, including female gender (P = 0.007), Lenses with higher nominal water content (P = 0.002), rapid preLens tear film thinning time (P = 0.008), frequent usage of over-the-counter pain medication (P = 0.02), limbal injection (P = 0.03), and increased tear film osmolality (P = 0.05). Conclusions Contact Lens-related dry eye may be explained mechanistically by increased tear film thinning times (evaporation or dewetting) resulting in increased tear film osmolality. Other contributing factors include the use of high-water-content Lenses, which have traditionally been reported to be associated with less patient comfort than lower-water-content Lenses, potentially due to spoilation and deposition. As found in other studies of dry eye, women are more likely to report Contact Lens-related dry eye than are men.

To determine the extent of orthokeratology (OK) Contact Lens fitting worldwide and to characterize associated demographics and fitting patterns. Methods . Survey forms were sent to Contact Lens fitters in 45 countries between January and March every year for 14 consecutive years (2004 – 2017, inclusive). Practitioners were asked to record data relating to the first 10 Contact Lens fits or refits performed after receiving the survey form. Results . Data were obtained for 295,044 Contact Lens fits, of which 2,702 were with OK Lenses and 292,342 were with other Lens types (non-OK). Overall, OK Lenses represented 1.2% of all Contact Lens fits, with significant differences between nations (p Conclusions . OK Contact Lens prescribing is a niche activity, with this Lens type typically being fitted in high oxygen permeable materials on a planned replacement basis to younger males. The slightly increasing rate of OK fitting, albeit at a very low level, may be attributed to practitioner interest in the reported myopia control properties of this Lens type.

Results: Data were received relating to 16,680 presbyopic (age 45 years or older) and 84,202 pre-presbyopic (15 to 44 years) Contact Lens wearers. Females are over-represented in presbyopic versus pre-presbyopic groups, possibly reflecting a stronger desire for the cosmetic benefits of Contact Lenses among older women. The extent to which multifocal and monovision Lenses are prescribed for presbyopes varies considerably among nations, ranging from 79 per cent of all soft Lenses in Portugal to zero in Singapore. There appears to be significant under-prescribing of Contact Lenses for the correction of presbyopia, although for those who do receive such corrections, three times more multifocal Lenses are fitted compared with monovision fittings. Presbyopic corrections are most frequently prescribed for full-time wear and monthly replacement. Conclusions: Despite apparent improvements in multifocal design and an increase in available multifocal options in recent years, practitioners are still under-prescribing with respect to the provision of appropriate Contact Lenses for the correction of presbyopia. Training of Contact Lens practitioners in presbyopic Contact Lens fitting should be accelerated and clinical and laboratory research in this field should be intensified to enhance the prospects of meeting the needs of presbyopic Contact Lens wearers more fully.

The performance of the Contact Lens dry eye questionnaire as a screening survey for Contact Lens-related dry eye

In this report, we examine the CLDEQ as a screening survey for Contact Lens-related dry eye and compare it with McMonnies' questionnaire. METHODS The CLDEQ and McMonnies' questionnaire were administered to 367 unselected Contact Lens wearers at six clinics across the United States and Canada. After completion of the surveys, doctors unaware of the survey results completed a separate form indicating Contact Lens-related dry eye diagnosis at the end of a nondirected clinical examination. The CLDEQ is composed of nine habitual symptom subscales and a self-diagnosis question, which were tested for their predictive value for a diagnosis of Contact Lens-related dry eye. McMonnies' instrument was scored with use of the algorithm suggested in the literature. Sensitivity, specificity, and receiver operator characteristic (ROC) curve analyses were performed for each instrument on the basis of logistic regression results. RESULTS The area under the ROC curve for the CLDEQ was 0.74, indicating moderate Contact Lens-related dry eye discrimination, and the Hosmer-Lemeshow goodness-of-fit test indicated that the CLDEQ was well calibrated (p = 0.84). The area under the ROC curve for McMonnies' questionnaire was 0.56, indicating poorer discrimination, and the Hosmer-Lemeshow goodness-of-fit test indicated it was also poorly calibrated (p = 0.08) for Contact Lens wearers. CONCLUSIONS These analyses suggest that the CLDEQ is capable of discriminating Contact Lens-related dry eye and is accurate in doing so, especially in comparison with McMonnies' questionnaire. The CLDEQ is an efficient screening survey and may be used in future clinical research and epidemiologic studies of Contact Lens-related dry eye.
