Cornell Medical Index a comparative study on health problems among thai and japanese nursing students

All of the Thai (114) and 96.2% (102) of the Japanese subjects self- administered the CMI. The internal consistency reliability of each CMI was acceptable. Mean ages ofthe Thai and the Japanese subjects were 19.3 (SD = 0.8) and 18.4 (SD = 0.5) years respectively, which was not significantly different. Mean CMI for the physical, psychological and overall health complaints of the Thai subjects were 29.8 (SD = 13.5), 14.5 (SD = 8.0), and 41.3 (SD = 19.3) respectively, and those for the Japanese subjects were 15.7 (SD = 11.8), 7.3 (SD = 6.1), and 23.1 (SD = 15.9) respectively, which were significantly different. Findings indicate that the Thai nursing students have more health problems than the Japanese subjects. These mostly concerned respiratory, cardiovascular, digestive, musculoskeletal, nervous, genitourinary systems, fatigability, frequency of illnesses, miscellaneous diseases, mood and feeling patterns, sensitivity, anger and tension. The only one section that the Japanese reported significantly higher health problems than the Thai subjects concerned daily living habits pertaining to questions concerning such matters as drinking, smoking, exercise, sleeping and rest. Percentages of emotional disturbances or neurotic tendencies of the subjects using the conventional CMI score and the Fukamachi criteria are discussed.

Data collected with the self-administered Cornell Medical Index Health Questionnaire Japanese version (JCMI) from 102 Japanese first year nursing students were analyzed. The total CMI score (section A-R) ranged from 2-108 with an average of 23.1 (SD = 15.9). The mean CMI for physical (section A-L) and psychological health complains (section M-R) were 15.7 (SD = 11.8) and 7.3 (SD = 6.1), respectively. Two-way analysis of variances with multiple comparison of means revealed significant relationships between age and psychological health complaints and both physical and overall health complaints. The older nursing students reported more physical and overall health complaints than the youngest. The nursing students who reported more complaints about psychological health also reported significantly more physical and overall health complaints. A comparison of Fukamachi's and Brodman's criteria for classification of neurotic traits is discussed. Findings from this study support the hypothesis that psychological health is somewhat reacted to physical health.

Thai version. The subjects were 17 to 24 years old in their 1st, 2nd, 3rd, or 4th year-class of nursing school in the northeast region of Thailand. The Pearson correlation coefficient shows an adequate internal consistency of the C.M.I. The observed C.M.I. scores ranged from 1 to 110 with a mean of 36.6. The highest percentages of yes-response, among physical and psychological complaints, were observed in the questions concerning the genitourinary system, and sensitivity respectively. A statistically significant inverse correlation was observed between year of nursing school and C.M.I. scores in physical, psychological and overall health complaints: the mean C.M.I. score on physical and overall health complaints of the 1st and the 3rd year-class students were significantly higher than those of the 2nd and the 4th year-class.

Methods: Forty-eight consecutive patients received IFN for chronic viral hepatitis for 8–24 weeks. CMI was measured before IFN therapy. Serial EEGs were recorded before IFN therapy, 2, 4 weeks, and thereafter every 4 weeks in the therapy. Results: Psychoneurological symptoms including insomnia, depression, and restlessness were seen in 11 (23%) of 48 patients. Five (13%) of 40 patients with CMI I and II and six (75%) of eight with CMI III developed psychoneurological symptoms (P<0.001). Sensitivity, specificity, and predictive accuracy of CMI III were 55%, 95%, and 75%, respectively. Abnormal EEG such as slow basic rhythm, appeared in 13 patients (27%) during IFN therapy. Psychoneurological symptoms were seen in six (46%) of the 13 patients with abnormal EEG, and in five (14%) of 35 in whom EEG remained normal (P<0.05). Conclusions: CMI is useful for the prediction of IFN-induced psychoneurological symptoms in patients with chronic viral hepatitis. Serial EEGs contribute to the screening and auxiliarily assessing the adverse effects of IFN on the central nervous system.

relationship between self reported prevalence of diabetes mellitus using the Cornell Medical Index cmi and prevalence determined by glycosylated hemoglobin hba1c in an elderly community dwelling population

A further 3% of unknown diabetes mellitus subjects are suggested when using formal biochemical methods of diagnosis. In this study, we present the prevalence of diabetes mellitus by self-reports using the CMI and concomitant biochemical detection in 436 community-dwelling older adults who have participated in a 20-year-study of age and cognitive performance in Manchester, UK. Twenty-three of the group reported that they had diagnosed diabetes mellitus, three individuals had a raised HbA(1c) of greater than 7.0% on random testing, but no knowledge of having diabetes mellitus. These individuals were re-contacted and three said they subsequently had a diagnosis of diabetes mellitus made within the two years following the questionnaire. We conclude that in an older population of community-dwelling subjects the numbers of undiagnosed cases of diabetes mellitus is lower than anticipated, based on large unselected population samples. The greater opportunity to interact with health care professionals who may consider screening for diabetes mellitus may explain these findings.

The Cornell Medical Index is one such tool used to determine the presence of physical and psychiatric illness but its accuracy and value have been questioned. In this study we have assessed the ability of the CMI to predict health status in two separate patient populations (n = 101, 88) by comparison to a structured Medical assessment based on the SENIEUR protocol by two physicians. There was good agreement between medication use reported on the CMI and on Medical assessment (k = 0.79; CI: 0.70-0.88). Accuracy of prediction of the CMI for specific Medical conditions was good 89-99%. A threshold score from the CMI was not predictive of health as determined by the SENIEUR protocol. In our older populations, we conclude that the CMI accurately predicted health status. The determination of normal health by a threshold score was poorly predictive of heath status. Self-reported medication use was the best predictor of health status.
