MckelvieAbstract:An extremely mounted Cuirass Ventilator, the Hayek Oscillator, was used on 41 patients undergoing surgery to the larynx without the use of tracheal tube. Gas exchange and cardiovascular parameters remained satisfactory during the use of this technique, which offers a significant advance over existing tubeless methods of anaesthesia.

A new method of tubeless anaesthesia for upper airway laser surgery

MonksAbstract:We have successfully used an externally mounted Cuirass Ventilator called the Hayek Oscillator to ventilate 15 patients undergoing laser surgery to the head and neck. Ventilation frequencies were in the range 60–180 cycles min^−1, inspiratory pressures varied from −15 to −25 cm of water, expiratory pressures from +5 to +10 cm of water. This machine has overcome the major problem of safely oxygenating patients undergoing laser surgery to the upper aerodigestive tract while leaving the surgical field unencumbered. No complications were encountered.

We have used it to ventilate patients undergoing microlaryngeal surgery. It was found to be a relatively safe method of ventilation in these cases, with the advantage of dispensing with any form of endolaryngeal or endotracheal intubation.

Severe tracheal stenosis and operative delivery.

MasonAbstract:We report the management of anaesthesia for Caesarean section in a woman with severe extensive tracheal stenosis. Management was initially with spinal anaesthesia, but general anaesthesia became necessary as a result of profuse intra-operative bleeding. We describe the use of the Hayek oscillator Cuirass Ventilator to allow instrumentation of the larynx whilst maintaining respiratory support, and for weaning from mechanical ventilation.
