    Abstract:

    Chronic syndrome of inappropriate secretion of antidiuretic hormone (chronic SIADH) has been reported in adults after traumatic brain injury (TBI) but few similar cases have been reported in the pediatric population. We present a case of a 14-year-old boy who developed chronic SIADH after TBI in which caloric needs could not be adequately provided, with concomitant fluid restriction as the first line of treatment. Demeclocycline was ultimately used, which allowed for increased fluid liberalization and for provision of adequate calories. This form of therapy should be considered early in a child's course to prevent nutritional decline. Demeclocycline may be used for prolonged periods until the child's clinical condition permits fluid restriction to be effective therapy.

    Abstract:

    1. Osmotic water movement across toad isolated hemibladders was measured by a gravimetric method. 2. The influence of carbamazepine, chlorpropamide and Demeclocycline on the antidiuretic hormone (ADH)-induced water flow rate was examined. 3. No antidiuretic activity due to carbamazepine alone was observed but a slight inhibition due to ADH-induced water flow was observed in the presence of carbamazepine over a selected dose-range. This was unexpected and is inconsistent with data from in vivo studies in man. 4. Chlorpropamide potentiated ADH-induced water flow, in keeping with the hypothesis that chlorpropamide sensitizes the renal tubules to ADH-induced water flow. 5. Demeclocycline inhibited ADH-induced water flow. The mechanism of action remains unclear.

  • Spectrophotometric method for the determination of Demeclocycline using sodium molybdate as analytical reagent

    Abstract:

    By means of spectroscopic methods it has been found that Demeclocycline reacts with molybdate ions forming a complex compound [MoO_3HDMTC]^2−. The relative stability constant of this compound has been determined by applying spectrophotometric methods. Under optimum conditions for complex formation a very sensitive spectrophotometric method for the estimation of Demeclocycline in the concentration range 5.0–35.1 μg/ml is proposed. The detection limit of the method is 2.5 μg/ml of Demeclocycline. The relative standard deviation ( n =10) varies between 0.76% and 1.46%. The method proved to be accurate and sensitive for the analysis of the Demeclocycline content in tablets.

  • Synthesis of two pH-responsive copolymers in pilot scale and its application in aqueous two-phase system

    Abstract Recycling pH-responsive aqueous two-phase system (pH-ATPS) has the advantages of low interfacial tension, high recovery, mild condition and low cost. In this paper, two pH-responsive copolymers 4% (w/v) PADB4.05 and 3% (w/v) PMDB3.36 were used to construct a recycling pH-ATPS (the recoveries of two copolymers were both higher than 97%) for partition of Demeclocycline and Lincomycin. They were successfully synthesized in pilot scale (500 L) through optimizing the monomer molar ratio and synthesis conditions and the yield of PADB4.05 and PMDB3.36 were 97% and 96% respectively. The characteristics of two scaled-up copolymers were consistent with those synthesized in laboratory scale. The optimal partition coefficient (K) of Demeclocycline were 4.223 with 20 mM LiCl at room temperature, respectively. For lincomycin, the K reached 2.629 in the presence of 4 mM MnCl2 at 45 °C. Partition of antibiotics in this pH-ATPS can be improved by varying pH, type and concentration of salts and temperature, which means it is available to antibiotics extraction and the further industrial separation.

  • Synthesis of pH-responsive polymers forming recyclable aqueous two-phase systems and application to the extraction of Demeclocycline

    Abstract:

    Abstract The application of aqueous two-phase systems (ATPS) to bioseparation and biocatalyst engineering has attracted interest. However, despite the distinct advantages of the technique, the scaling up of ATPS is limited by the cost of the system components. In this study, two novel recyclable pH-responsive polymers were synthesized and tested for the partition of Demeclocycline. The recoveries of two recyclable pH-responsive polymers could reach over 95.0% by only adjusting the solution pH to the isoelectric points of the polymers. The control variate method was used to investigate the effects of polymerization conditions on polymer synthesis. Polymer PADB4.6 was firstly synthesized in 500 l reactor with two-stage heating method (25–40 ℃ and 40–62 ℃), as the heating rate is an important factor in the scale-up process. The polymer characteristics (molecular weight, viscosity, surface tension, and zeta potential) were studied to understand their influences on the synthetic process. Furthermore, the phase formation mechanism was studied with low-field nuclear magnetic resonance (LF-NMR). The optimal partition coefficient and recovery of Demeclocycline using the recyclable ATPS were 0.24 and 82.9% at pH 6.30 in presence of 20 mmol/L MgSO4.

    Abstract:

    The syndrome of inappropriate secretion of antidiuretic hormone (SIADH) is a frequent complication of severe head trauma. Fluid restriction is treatment choice of SIADH in patients with traumatic brain injury (TBI), but fluid restriction is limited because they reed sufficient calories. We described a patient who, on the four months after a head injury, presented with deterioration of consciousness, which coincided with the development of the SIADH, and which rapidly reversed with the correction of the hyponatremia by Demeclocycline with minimal fluid restriction. We suggest that SIADH should be included in the differential diagnosis of deterioration of consciousness during the recovery period of the patients suffering from head injury because unexpected clinical deterioration may often have a reversible cause. Also, Demeclocycline will be useful, which allows for increased fluid liberalization and for provision of adequate calories, in the treatment of the SIADH in patients with TBI.
