Many medical interventions are available, including manipulations, with conflicting results. Purpose To assess the short- and long-term effects of spinal manipulations on acute back pain and sciatica with Disc Protrusion. Study design/setting Randomized double-blind trial comparing active and simulated manipulations in rehabilitation medical centers in Rome and suburbs. Patient sample 102 ambulatory patients with at least moderate pain on a visual analog scale for local pain (VAS1) and/or radiating pain (VAS2). Outcome measures Pain-free patients at end of treatment; treatment failure (proportion of patients stopping the assigned treatment for lack of effect on pain); number of days with no, mild, moderate, or severe pain; quality of life; number of days on nonsteroidal anti-inflammatory drugs; number of drug prescriptions; VAS1 and VAS2 scores; quality of life and psychosocial findings; and reduction of Disc Protrusion on magnetic resonance imaging. Methods Manipulations or simulated manipulations were done 5 days per week by experienced chiropractors, with a number of sessions which depended on pain relief or up to a maximum of 20, using a rapid thrust technique. Patients were assessed at admission and at 15, 30, 45, 90, and 180 days. At each visit, all indicators of pain relief were used. Results A total of 64 men and 38 women aged 19–63 years were randomized to manipulations (53) or simulated manipulations (49). Manipulations appeared more effective on the basis of the percentage of pain-free cases (local pain 28 vs. 6%; p Conclusions Active manipulations have more effect than simulated manipulations on pain relief for acute back pain and sciatica with Disc Protrusion.

Chronic Abdominal Syndrome Due to Nervous Compression. Study of 100 Cases and Proposed Diagnostic-Therapeutic Algorithm

Oliva MuñozAbstract:Objective In the medical literature, thoracic Disc Protrusion has traditionally been considered a rare occurrence. We hypothesise that the incidence of such Protrusions and their abdominal symptoms is higher than is generally believed and that their presence may account for a significant proportion of chronic non-visceral abdominal pains. Accordingly, the present study was designed to identify and quantify the symptoms experienced by patients with thoracic Disc Protrusion and to assess the relative risk of these symptoms being presented, compared to the general population. Design We conducted a cross-sectional study with a control group. The following comparison groups were analysed: case group, consisting of 100 patients diagnosed with thoracic Disc Protrusion in our hospital between February 2007 and October 2012, and control group consisting of 100 subjects from the general population, chosen at random. To compare the symptoms observed in each group, the following tests were applied to all study subjects: clinical examination, gastrointestinal-related quality of life (GIQLI) questionnaire and DN4 questionnaire. We also reviewed the subjects’ medical records for the previous 3 years. Results The subjects in the case group had a significantly higher incidence of digestive-urologic symptoms, a poorer gastrointestinal quality of life and greater need of medical care than those in the control group. The differences were statistically significant for all the parameters studied. Almost all the case group subjects suffered chronic abdominal pain and/or digestive-urologic symptoms. We term this group of symptoms “chronic abdominal syndrome due to nervous compression”. Nevertheless, in most cases, no neurologic aetiology was suspected, and therefore the treatment given was ineffective. In view of the results obtained, we propose a diagnostic-therapeutic algorithm for such patients. Conclusion Thoracic Disc Protrusion, as well as having a non-negligible incidence, is often associated with a digestive-urologic clinical syndrome, and this factor should be taken into account in all cases of chronic abdominal pain and other digestive-urologic symptoms when standard tests are negative, so that appropriate treatment may be given.

This case study describes a 45-year-old patient whose symptoms of lumbar radiculopathy resolved and follow-up imaging showed complete disappearance of the Disc prolapse. This phenomenon strengthens the role of conservative treatment in the management of lumbar Disc Protrusions.

Cervical myelopathy in a child with Sprengel shoulder and Klippel–Feil syndrome

Cervical myelopathy in a child due to a concomitant omovertebral bar causing posterior cord compression and a hypermobile cervical Disc Protrusion adjacent to fused cervical segments causing anterior compression at the same level is very rare. We report the presentation, findings, surgical management and results of treatment in such a child. Methods A 9-year-old girl with Sprengel shoulder presented with cervical myelopathy (Frankel D). Imaging revealed a bony omovertebral bar connected to the left scapula compressing the spinal cord posteriorly through a lamina defect at C5 resulting in significant cervical stenosis. A hypermobile Disc Protrusion adjacent to congenitally fused segments resulted in anterior compression at the same level. She was treated surgically with cervical laminectomy and instrumented fusion, excision of the omovertebral bar and modified Woodward procedure for the left Sprengel shoulder. Results At 2 year follow-up, she had improved neurologically (Frankel E) and there was improved shoulder symmetry and abduction. MRI showed resolution of cervical stenosis. Although there was no significant progression of congenital scoliosis, it will need to be monitored. Conclusions Cervical myelopathy due to an omovertebral bar and cervical Disc Protrusion in a child with Klippel–Feil syndrome and Sprengel shoulder is a rare presentation and can be treated with a single posterior approach addressing both pathologies.

effect of acupuncture on pain management in patients before and after lumbar Disc Protrusion surgery a randomized control study

The effectiveness of this form of therapy is dependent upon compliance, which in turn is dependent on availability, response, treatment of proper acupoints, and the placebo effect. We hypothesized that classical acupuncture would be more effective than placebo acupuncture. One hundred and thirty-two patients with acute and chronic low back and leg pain were examined before and after surgery for lumbar Disc Protrusion. Diagnosis was based on CT and MRT findings. Patients received acupuncture drugfree throughout the study period. The visual analogue scale was used to assess pain intensity before and after (i.e. 30min. 60min. 2h and 6h) acupuncture. Classical acupuncture resulted in a significant reduction in pain that become increasingly stronger during the 6h study period. Placebo acupuncture lead to same early pain relief that did not reach statistic significant and then declined thereafter.
