    Abstract:

    Ross River virus is the most common vector-borne Disease in Australia, with the majority of Notifications being in Queensland. This study describes a retrospective spatial analysis of Queensland Ross River virus Disease Notifications spanning a 10-year period. Notifications were mapped to the local government area (LGA) of the residence of the patient. Ross River virus Disease outbreaks within each LGA were detected by applying a Poisson model. Estimates of the seasonal incidence rates indicated wide variation between seasons and LGAs. Positive spatial autocorrelation between LGAs experiencing outbreaks indicated that LGAs within the same region often experience outbreaks at the same time. A hierarchical cluster analysis of the outbreak data was used to group LGAs with similar temporal outbreak patterns. This analysis highlights the variability in Ross River virus Disease Notification rates across Queensland, and provides a robust method for identifying Disease outbreaks.

    Notifications of Ross River (RR) virus Disease in Maroochy Shire were mapped according to the patient's place of residence, and standardized morbidity ratios were calculated for each of 11 census districts, with 4 areas having higher than average overall rates of RR virus Disease Notification. Temporal analysis of RR virus Disease Notifications from each of the 11 areas indicated that epidemics of RR virus Disease either were widespread, resulting in higher than average numbers of cases from the majority of areas, or limited to only certain areas. Mosquitoes were collected in CO2 light traps, and the numbers analyzed to determine whether there was any association between mosquito abundance and Disease incidence. Seasonal light trap indices for the fresh water-breeding Culex annulirostris Skuse and brackish water-breeding Aedes funereus (Theobald) were correlated positively with Disease incidence. There was no apparent association between Aedes vigilax (Skuse) or Aedes procax (Skuse) light trap indices and Disease incidence. Identification of human subpopulations with higher than average rates of RR virus Disease, and the mosquito vectors associated with virus transmission, should provide a framework for the development of focused and therefore more effective control programs.

Risk factors such as human migration and movement of pigs/pork, as well as the increasing trend in pig rearing with outside access are discussed in this review. The entry of a tapeworm carrier into the EU seems a lot more plausible than the import of infected pork. The establishment of local transmission in the EU is presently very unlikely. However, considering the potential changes in risk factors, such as the increasing trend in pig farming with outdoor access, the increasing human migration from endemic areas into the EU, this situation might change, warranting the establishment of an early warning system, which should include Disease Notification of taeniasis/cysticercosis both in human and animal hosts. As currently human-to-human transmission is the highest risk, prevention strategies should focus on the early detection and treatment of tapeworm carriers, and should be designed in a concerted way, across the EU and across the different sectors.

    OBJECTIVES--The aim was, firstly, to study the validity of the sentinel network data by analysing the correlation between the weekly influenza cases detected by the network and the number of cases notified to the compulsory Disease Notification system and, secondly, to describe the epidemiology of the period 1991-92 in terms of the sentinel network data. DESIGN--The study design was descriptive. SUBJECTS--The population resident in the city of Madrid. MAIN RESULTS--There was a high correlation between the cases notified to the compulsory Disease Notification system and those listed by the sentinel network (r = 0.91, p < 0.001, r2 = 0.82). The epidemic activity during the period 1991-92 was centred approximately on week 48 in 1991 and week 6 in 1992 (24 November 1991-8 February 1992). Altogether 913 cases of influenza were notified through the sentinel network. The greatest number of cases occurred in people aged between 20 and 39 years. CONCLUSION--For the epidemiological surveillance of influenza it is necessary to develop systems that can quickly detect epidemic periods and provide information about populations at risk, in addition to systems that isolate and identify seasonal epidemic viruses. Both types of data will help the development of adequate public health policies. Sentinel networks provide these data and offer additional advantages such as lower cost.
