Clinical Schizophrenia & Related Psychoses, 2015Co-Authors: Eyal Dahan, Avi Bleich, Yuval MelamedAbstract:Owing to unresolved questions concerning the efficacy and safety of electroconvulsive therapy (ECT) in the treatment of Schizophrenia, and widespread negative attitudes toward ECT, maintenance ECT (mECT) is generally considered only as a last resort. Nevertheless, in some clinical situations, the advantages of mECT may outweigh the risks and associated concerns. We report the case of a patient suffering from Disorganized Schizophrenia who had life-threatening hematological side effects to treatment with antipsychotic agents. Long-term mECT was administered and the patient achieved remission with no notable side effects. He was able to maintain a peaceful daily routine and improved functioning. Considering the lack of controlled trials in this area, this case and other similar cases reported in the literature add support to a possible benefit of mECT in Disorganized Schizophrenia, particularly when pharmacotherapy is insufficient or contraindicated

Diagnóstico en psicopatología. Incidencias en la clínica

The original objective was to characterize the psychopathological profile of hospitalized patients and their evolution, according to the treatments during psychiatric hospitalizations and to diagnoses of Schizophrenia and other psychotic disorders, and evaluating them at the time of their entry, stabilization, and discharge. This study was prospective, based on real clinical situations, and comprised two cohorts of patients: 36 cases of Schizophrenia and 17 cases of other non-schizophrenic psychotic disorders. The sample was distributed as follows: The 36 cases of Schizophrenia (67.92 %), were distributed: 8 cases of Disorganized type (15.09 %), 17 cases of paranoid type (32.08 %), no cases of catatonic type (0 %), 8 cases of residual type (15.09 %), and 3 cases of undifferentiated type (5.66 %). The remaining psychotic disorders (32.08 %): 6 cases of delusional disorder (11.32 %), 1 case of brief psychotic disorder (1.89 %), 1 case of schizophreniform disorder (1.89 %), 4 cases of schizoaffective disorders (7.55 %), 4 cases of non-specified psychotic disorders (7l.55 %) and 1 case of paranoid personality disorder (1.89 %). This sample allowed us to establish types and degrees of psychotic decompensation according to variables of Schizophrenia and other psychoses, and the evolution of these treatments. Although it was not possible to establish a direct relationship between diagnoses, stabilization and discharge times, it was possible to establish a certain relationship between hospitalization criteria and those of stabilization and discharge: the cases of Disorganized Schizophrenia tended to decrease in conviction, concern and interference of delusional ideas towards very slight levels or absence, whereas in the cases of delusional disorder and paranoid Schizophrenia, conviction continued to be severe; concern, moderate to severe; and interference, moderate, despite the implementation of treatments. Despite the difference in the yield curve of treatments by subtype of Schizophrenia or psychotic episode, discharge conditions stemmed from the modulating variables to a larger extent than they did from the stabilization of the patient as predicted. Consequently, the expectation of the internalization-stabilization-discharge process is fulfilled in a proportion of 18.87 %, with the moderating variables (influence of the law, role of families, administrative aspects, etc.), receiving an unexpected role.La presente investigación fue realizada entre los años 2014 y 2016. Su objetivo original consistió en caracterizar el perfil psicopatológico de pacientes internados en dispositivos psiquiátricos y su evolución según los tratamientos realizados durante la internación psiquiátrica, de acuerdo a diagnósticos de esquizofrenia y otros trastornos psicóticos. Se los evaluó al momento de su ingreso, estabilización y externación. Se trató de una investigación de situaciones clínicas reales, prospectiva, con dos cohortes de pacientes: 36 casos de esquizofrenia y 17 casos de trastornos psicóticos no esquizofrénicos. La muestra se distribuyó de la siguiente manera: los 36 casos de esquizofrenia (67,92 %), se distribuyeron en 8 casos de tipo desorganizado (15,09 %), 17 tipo paranoide (32,08 %), 0 casos de tipo catatónico (0 %), 8 tipo residual (15,09 %), y 3 tipo indiferenciado (5,66 %). Los restantes cuadros psicóticos (32,08 %) corresponden a 6 casos de trastorno delirante (11,32 %), 1 trastorno psicótico breve (1,89 %), 1 trastorno esquizofreniforme (1,89 %), 4 trastornos esquizoafectivos (7,55 %), 4 trastornos psicóticos no especificados (7,55 %) y 1 trastorno paranoide de la personalidad (1,89 %). La muestra permitió establecer tipos y grados de descompensación psicótica de acuerdo a variables propias de la esquizofrenia y otros cuadros psicóticos, además de la evolución de estos tratamientos. Pese a que no se pudo establecer una relación directa entre diagnósticos, tiempos de estabilización y tiempos de externación, se pudo establecer cierta correspondencia entre los criterios de internación, estabilización y externación: los casos de esquizofrenia desorganizada tendían a disminuir en convicción, preocupación e interferencia de las ideas delirantes hacia niveles muy leves o de ausencia, mientras que en los cuadros de trastorno delirante y esquizofrenia paranoide la convicción continuaba siendo severa, la preocupación de moderada a severa y la interferencia moderada, pese a la implementación de los tratamientos. No obstante la diferencia curva de rendimiento en los tratamientos por subtipo de esquizofrenia y/o cuadro psicótico, las condiciones de externación se desprendían en mayor medida de las variables moduladoras que de la estabilización del paciente, tal como estaba previsto. Consecuentemente, se destaca que la expectativa del proceso internación-estabilización-externación se cumple en una proporción del 18,87 %, y que cobran un protagonismo inesperado las variables moderadoras (influencia de la ley, rol de las familias, aspectos administrativos, etc.)

From these ones, were selected those diagnosed with Schizophrenia category in its different forms. We studied the treatment of these patients by the application of different instruments of psychiatric evaluation (transversal and longitudinal).This study has the advantage of counting with various controlled variables because the psychiatric hospitalization device. This investigation is foresight, based in real clinical situations, with two cohorts of patients: the first is composed by patients that present diagnostic of Schizophrenia, the second includes patients that present otherpsychotic disorders, with the objective of weighting identities and/or differences in relation to the first group of patients. We worked with 53 patients (36 of them were schizophrenic patients and 17 have psychoticdisorders), of which 51 have completed its psychiatric hospitalization treatments and 2 were still hospitalized at closing data collection. This sample allowed to establish types and degrees of psychotic decompensation in agreement of variables of Schizophrenia and other psychoses, and the evolution of these treatments. Among the results, the fact that the expectation of psychiatric hospitalization - stabilization – externation processes is accomplished in a ratio of 1 in 6 cases, charging an unexpected role of the moderating variables (the influence of the law, role standsfamilies, administrative, etc.). Moreover, without epidemiological claims by the present study, it is relevant that in two and a half year of working with patients had not shown any cases of catatonic Schizophrenia. From the results in relation to the evidence generated, facing the stabilization achieved by patients according to the distribution diagnosis, we can warn that within the subgroups of schizophrenic patients, the degree of stabilization do not follow a consistent pattern, while in delusional disorder diagnoses the degree of stabilization is low or zero. Meanwhile, cases of Disorganized Schizophrenia have a higher propensity for re-hospitalization.La presente investigación se propuso caracterizar el perfil psicopatológico de pacientes internados y su evolución de acuerdo al tratamiento recibido en un dispositivo de internación psiquiátrica. Se incluyó en la muestra personas que presentaron diferentes cuadros psicóticos, de estos se seleccionaron aquellos casos diagnosticados con la categoría esquizofrenia, según sus diversas formas, y se estudiaron los tratamientos de estos pacientes de acuerdo a la aplicación de diferentes instrumentos de evaluación psiquiátrica (trasversal y longitudinal). Se trata de una investigación de situaciones clínicas reales, prospectiva, con dos cohortes de pacientes: la primera presenta diagnóstico de esquizofrenia como más relevante, mientras que la segunda incluye pacientes que presentan otros cuadros psicóticos, con el fin de poder ponderar identidades y/o diferencias en relación a los pacientes diagnosticados de esquizofrenia. Se trabajó con 53 personas (36 con esquizofrenia y 17 con cuadros psicóticos no esquizofrénicos), de las cuales 51 completaron sus tratamientos de internación psiquiátrica y 2 continuaban internadas al momento del cierre de la recolección de datos. Esta muestra permitió establecer tipos y grados de descompensación psicótica de acuerdo a variables propias de la esquizofrenia y otros cuadros psicóticos, y la evolución de estos tratamientos. De los resultados en relación a la evidencia generada de cara a la estabilización alcanzada por los pacientes de acuerdo a la distribución por diagnóstico, se puede advertir que, dentro de los subgrupos de pacientes esquizofrénicos, los grados de estabilización no siguen un patrón congruente, mientras en diagnósticos con trastorno delirante el grado de estabilización es bajo o nulo. Por su parte, los casos de esquizofrenia desorganizada presentan una mayor propensión a la reinternación

However, there has been controversy about the standardized methods of continuation and maintenance ECT in the management of treatment-resistant Schizophrenia. We describe a patient with a serious case of Disorganized Schizophrenia who had not responded well with any typical and atypical antipsychotic drug for seven years, but responded remarkably to acute ECT. Continuation ECT was necessary to sustain a positive therapeutic response. The patient showed dramatic improvement from 70 to 20 in the 18-item Brief Psychiatric Rating Scale (BPRS) score (71% reduction) after acute ECT and continuation ECT. Using maintenance ECT, she was able to live in the custody of her parents after 7-years hospitalization. This case report suggests that continuation and maintenance ECT benefits patients with serious cases of refractory Schizophrenia.
