Universidad Privada Antenor Orrego - UPAO, 2017Co-Authors: Garcia Alvarado, Maivi MilagrosAbstract:The present thesis entitled "Occupational Health and Safety System design according to the norm OHSAS 18001 standard to reduce the cost of SUNAFIL’s penalt y in the production area of the company Active Life, Lima"" tests the application of topics taught along the career of industrial engineering, such as the study to improve working conditions and provide a safe and healthy environment by proposing the implementation of a Health, Safety and Occupational Security System (HSOS). The design of this system will seek to avoid the economic penalty imposed by SUNAFIL (National Superintendence of Labor Inspection in English) to the company Active Life, for not having a HSOS. The problem that the company is facing is the economic penalty imposed by SUNAFIL, which would be S/. 132,720.00, for not having a HSOS. The best alternative that the company would have would be to implement this area within the company in order to avoid the penalty imposed. The first thing that was done to design this system was to propose the formation of the Committee on Safety and Health at Work. These will be the ones that approve the different issues in the HSOS; this Procedure was consulted in the SUNAFIL implementation manual. Subsequently, proposals were made for the Occupational Health and Safety Policy of the company, which reflects management's commitment to HSOS issues. The items of these proposals were derived from the requirements of the norm OHSAS 18001: 2007. Subsequent to this, the matrix entitled “Baseline Hazard Identification and Risk Assessment (HIRA)” was develop because it is the first time is being done. The matrix and the description of how to do it were obtained from the report titled “IPER” from BS group. Through this, it was possible to establish a Documented Procedure for the continuous identification of the most risky activities and to determine the necessary controls to reduce them. Finally, the risk map of the company was elaborated, the different signs for security to realize this map comes from the document ent itled “Norma Técnica Peruana NTP 399.010. With the elaboration of this, it was possible to have a greater prevention of the risky places that exist in the plant and to be accessible for all employees. In this way it was possible to reduce the cost of the penalty imposed by SUNAFIL in 12.25 UIT (Peruvian Tax Unit).La presente tesis titulada “Diseño de un Sistema de Seguridad y Salud Ocupacional según la Norma OHSAS 18001 para reducir el costo de la penalidad de SUNAFIL en el área de producción de la empresa Active Life, Lima” puso a prueba la aplicación de temas enseñados a lo largo de la carrera de ingeniería industrial, como fue el estudio para mejorar las condiciones de trabajo y brindar un ambiente seguro y saludable proponiendo la implementación de un Sistema de Seguridad y Salud en el Trabajo (SST). Con el diseño de este sistema, se buscó evitar la penalidad económica exigida por SUNAFIL, que es la Superintendencia Nacional de Fiscalización Laboral, para la empresa Active Life, por no contar con un sistema de SST. El problema al que se enfrentó la empresa es la penalidad económica que impone SUNAFIL, esta fue de S/. 132,720.00, por no contar con un sistema de gestión de SST. La mejor alternativa que tuvo la empresa fue implementar un sistema de seguridad y salud ocupacional para que no incurra en gastos adicionales como penalidades o multas. Lo primero que se realizó para diseñar un sistema de SST, fue plantear la conformación del Comité de Seguridad y Salud en el Trabajo, debido a que éstos, serán los que aprueben y den el visto bueno para los diferentes temas en el sistema de SST, este procedimiento fue consultado del manual de implementación de SUNAFIL. A continuación, se procedió a realizar propuestas de Política de Seguridad y Salud Ocupacional de la empresa, la cual refleja el compromiso de la gerencia en temas referentes a SST. Los ítems de estas propuestas, fueron derivados de los requisitos que exige la norma OHSAS 18001:2007. Posterior a esto, se elaboró la matriz de Identificación de Peligros y Evaluación de Riesgos (IPER) de línea base, debido a que es la primera vez que se realiza, la matriz y la descripción de cómo realizarlo fue obtenida del informe titulado IPER de BS grupo. A través de esto, se logró establecer un procedimiento documentado para la continua identificación de las actividades más riesgosas y determinar los controles necesarios para reducirlos. Finalmente, se elaboró el mapa de riesgos de la empresa, la señalización para realizar este mapa procede de la norma técnica peruana NTP 399.010. Con la elaboración del mismo, se logró tener una mayor prevención de los lugares riesgosos que existen en la planta y para que sea accesible para todos. De esta manera se logró reducir el costo de la penalidad impuesta por SUNAFIL en 12.25 UIT

Results The survey found that there were many different West-gard rules used either every shift, daily or at some other frequency. Most laboratories had automated rules, with about 50% using external software. Not all laboratories reported having a Documented Procedure to follow in the advent of a QC failure. A significant number of respondents used the QC material manufacturer’s recommendations to set the mean and SD. There were also differences in the way means and SDs were set for multiple instruments using the same analyte and how laboratories assessed comparability of analytes from different analysers. QC targets for means and SDs were reassessed at different time periods. Discussion Despite the importance of quality assurance there are still inconsistencies in the way laboratories deal with internal QC processes in term of frequency of QC, interpretation and dealing with out of control results and setting QC material targets. There is an argument to harmonise many of these aspects of QC policy.

De conformidad con el código Internacional de Gestión de la Seguridad y prevención de la contaminación (IGS), todas las compañías tienen la obligatoriedad de implementar un Sistema de Gestión de la Seguridad (SGS) que incluya entre otras prescripciones de orden funcional, “instrucciones y procedimientos que garanticen la seguridad operacional del buque y la protección del medio ambiente […]” El objetivo de este trabajo es desarrollar una metodología que permita diseñar un procedimiento documentado aplicable al proceso de Evaluación Inicial de la Travesía que sea adaptable al SGS de cualquier naviera certificada bien por la administración o por una organización aprobada, de conformidad con el cumplimiento de las disposiciones del código Internacional de Gestión de la Seguridad y prevención de la contaminación (IGS) y con al menos, los requisitos de calidad establecidos en la ISO9001:2015 sobre sistemas de gestión de calidad certificada por cualquier organismo de certificación acreditado1. La metodología es una guía que permite diseñar el procedimiento aplicable al proceso de evaluación inicial de la travesía y adaptarlo al sistema de gestión de una naviera. The methodology is a guide for designing the Procedure for the Apraissal process and adapting it to the management system of a shipping company.
