    Background Efforts at physician-payment reform in the United States have focused largely on the relative incomes of primary care physicians and specialists, who more often have procedure-based practices. Comparisons of the incomes of physicians and other professional groups have received less attention. Methods We used standard financial techniques to determine the return on Educational investment over a working lifetime for five groups of professionals: primary care physicians, specialist physicians, dentists, attorneys, and graduates of business schools. Results In current dollars, the difference in the average future hourly income between a given professional and a high-school graduate of the same age, after Educational expenses are subtracted (average hours-adjusted net present value of the Educational investment) was greatest for specialist physicians and attorneys; dentists and businesspeople had intermediate values; and primary care physicians had the lowest value.     Objectives To investigate the financial and Educational Costs of the urology residency interview process, we performed a survey of the applicants to the 2006 urology match. Methods All applicants registered for the 2006 urology match were invited to participate. In January 2006 prior to the match, an anonymous online survey containing 8 questions on the financial and Educational Costs of the interview process was distributed via email. Results Survey response rate was 61% (287/468). The median Educational debt of the applicants was $125,000 (IQR 65,000 to 160,000). Respondents reported having a median 12 interviews (IQR 8 to 15) with urology residencies and spending a median 20 days (IQR 14 to 30) on the interview trail. The total cost of the interview process was a median $4000 (IQR 2000 to 5200) with a median expense per interview of $330 (IQR 211 to 455). Applicants reported that travel expenses accounted for a median 60% of overall interview expenses, whereas the remainder of the expense was accounted for by lodging (25%), food (10%) and clothing (5%). The money to cover these interview-related expenses was obtained primarily by loans. Forty-six percent of the applicants reported that skipping medical school clerkships and classes for urology interviews was “not at all detrimental” to their medical education, whereas 1% reported that it was “greatly detrimental.” Conclusions The financial cost of the interview process for urology applicants is substantial, although the Educational cost appears to be limited. Efforts to reduce the financial impact of the interview process should be initiated at both a regional and national level.

    Based on individual occupational choice in the model including a production function with public investment, this paper presents an examination of how public investment affects the dynamics. Individuals work as skilled labourers or unskilled labourers. As in the model described by Caselli (1999), Educational Costs are necessary to work as a skilled labourer. Results show that life expectancy determines whether income growth occurs or not. Public investment can bring about income growth if life expectancy is sufficiently high. However, with low life expectancy, the government cannot bring about income growth with an increase in public investment. Therefore, both public investment and life expectancy should be pulled up for income growth.

    Abstract:

    Based on individual occupational choice in a model including a production function with public investment and public health infrastructure, this paper presents an examination of how allocation of public investment and public health infrastructure affects the dynamics of income. Individuals work as skilled laborers or unskilled laborers, as in the model described by Caselli (1999), and Educational Costs are necessary to work as a skilled laborer. Results show that government should provide both public investment and public health infrastructure to escape from the poverty trap with low income. Moreover, based on an initial allocation between public investment and public health infrastructure, it is decided how the government should form a policy to increase income growth.
