    Abstract:

    Objective:To treat inoperable severe tracheal stenosis by using nitinol Endotracheal Stent and to study the biocompatibility of which being long term implanted in the body Methods: The Stent was deformed to smaller size in iced water With a self made introducer, the Stent was placed in the location of stenosis After hot water was injected into the introducer, the Stent resumed its original form and anchored there Items such as roentgenography, endoscopy were performed By means of electronic microscope and micro balance, corrosion situation on the surface of Stents were studied after long term implantation(2,10,14months) Results: Altogether 10 patients with severe tracheal stenosis received this treatment Dyspnea was relieved immediately after implantation of Stents in all patients Long term follow up also showed satisfactory results The Stent has a good biocompatibility after long term implantation in the body Conclusions: Nitinol Stent as a new endoprosthesis is effective in treatment of inoperable severe tracheal stenosis It has some remarkable advantages as compared with other tracheal Stents

    Abstract:

    A new method used in treatment of tracheal stenosis by means of nitinol alloy intraluminal tracheal Stent (Ni-Ti ITS) is introduced. Altogether 7 Stents were implanted into tracheas of 7 mongrels each with a model of tracheal stenosis. The observation periods ranged from 2 to 6 months. The experimental results showed: The Ni-Ti ITSs could maintain long term patency without severe infection or obstruction by granulations or secretion. No displacement was found. There were no signs of either generalized or localized corrosion on the surface of Stents. The shape memory character remained unchanged. On the basis of animal experimental study, the Stents were successfully used in treatment of 2 patients with tracheal stenosis. One was caused by advanced tracheal tumor, the other was due to recurrent anastomotic stricture.

    Abstract:

    Objective  To explore the clinical results of implanting expanding nitinol alloy Stent in the treatment of tracheobronchial stenosis. Methods  The nitinol alloy Endotracheal Stent was put into the site of stricture of trachea or bronchus in 42 patients through an optical fibrobronchoscopy under X-ray examination. The oxygen saturation of blood (SaO2 ) and pulse rate were detected before , during and after operation respectively. Results  After implanting nitinol alloy Stent , 32 patients got satisfactory response , and other 10 patients got partial response. In all patients , the diameters of the stenostic airway were increased significantly , and ventilation function improved remarkably. The postoperative SaO2 was greatly higher than the preoperative one ( P < 0. 001) . The quality of life was also improved. Conclusion  The nitinol alloy Endotracheal Stent was an effictive , safe and reliable method to release tracheobronchial stenosis. DOI: 10.3779/j.issn.1009-3419.2000.02.12

    Abstract:

    We used a Dumon-type Endotracheal Stent for left main bronchial stenosis occurred after the operation of combined true and Type III dissecant aneurysm. A 65-year-old man with acute Type III dissection with true thoracic aneurysm was operated on by means of arch replacement with thoracic aortic replacement. He complained of dyspnea and the respirator support was not able to discontinued. The cause of dyspnea was oppression of the left main bronchus with remnant thoracic dissecting aneurysm together with a hematoma. A Dumon-type Stent was successfully inserted into the left main bronchus under bronchoscopic guide via tracheostomy. It is a choice of conservative therapy after major surgery such as operation of thoracic aneurysm to relieve the bronchial obstruction.

    Abstract:

    To relieve tracheal stenosis due to esophageal carcinoma, we have inserted Dumon-type Endotracheal Stent tube to 2 patients with stenosis of the left main bronchus and a patient with tracheal stenosis. Severe dyspnea and stridor found in these patients was much improved in all three cases. After insertion of the Stent tube to 2 patients with stenosis of the left main bronchus, radiation therapy with the dose of 65 Gy to main esophageal lesion was undergone, which resulted in the partial remission. These management allowed the patients to discharge. The last case with tracheal stenosis was due to recurrence of the esophageal carcinoma, which required additional insertion of the Stent tube at the proximal site after initial insertion in a short period. We conclude that the quality of life and prognosis of patients with tracheal stenosis due to advanced esophageal carcinoma may be improved by the insertion of Dumon-type Endotracheal Stent tube.
