Exercise During Pregnancy, maternal prePregnancy body mass index, and birth weight.

Furthermore, we estimated the indirect association between maternal BMI and birth weight, explained by Exercise During Pregnancy. This study included pregnant women and their offspring recruited from 1999 to 2006 in the Norwegian Mother and Child Cohort Study, conducted by the Norwegian Institute of Public Health. Linear regression analyses were based on exposure data from two self-administered questionnaires During Pregnancy and birth weight data from the Medical Birth Registry of Norway. The study included 43,705 pregnancies. The median Exercise frequency During the first 17 weeks of gestation was six times per month and four times per month thereafter until week 30. Mean maternal prePregnancy BMI was 24 kg/m, and mean birth weight of the offspring was 3,677 g. The adjusted direct association between Exercise and birth weight was a 2.9-g decrease in birth weight per unit increase in Exercise (one time per month). In contrast, the adjusted direct association between BMI and birth weight was a 20.3-g increase in birth weight for a one-unit increase in BMI (1 kg/m), and the indirect association explained by Exercise was only a 0.3-g increase in birth weight. Exercise During Pregnancy has a minor impact on birth weight, whereas maternal prePregnancy BMI has a larger influence. Thus, we suggest that health care professionals should focus on normalizing the BMI of women in fertile ages. II.

Exercise During Pregnancy and risk of cesarean delivery in nulliparous women a large population based cohort study

Given the rising cesarean delivery rates worldwide over the past decades, a search for modifiable factors that are associated with cesarean delivery is needed. Exercise may be a modifiable factor that is associated with type of delivery, but the results of previous studies are not conclusive. Objective The purpose of this study was to investigate the association between Exercise During Pregnancy and cesarean delivery, both acute and elective, in nulliparous women. Study Design We conducted a population-based cohort study that involved 39,187 nulliparous women with a singleton Pregnancy who were enrolled in the Norwegian Mother and Child Cohort Study between 2000 and 2009. All women answered 2 questionnaires in Pregnancy weeks 17 and 30. Acute and elective cesarean delivery data were obtained from the Medical Birth Registry of Norway. Information on Exercise frequency and type was assessed prospectively by questionnaires in Pregnancy weeks 17 and 30. Generalized linear models estimated risk differences of acute and elective cesarean delivery for different frequencies and types of Exercise During Pregnancy weeks 17 and 30. We used restricted cubic splines to examine dose-response associations of Exercise frequency and acute cesarean delivery. A test for nonlinearity was also conducted. Results The total cesarean delivery rate was 15.4% (n=6030), of which 77.8% (n=4689) was acute cesarean delivery. Exercise During Pregnancy was associated with a reduced risk of cesarean delivery, particularly for acute cesarean delivery. A nonlinear association was observed for Exercise frequency in weeks 17 and 30 and risk of acute cesarean delivery (test for nonlinearity, P =.003 and P =.027, respectively). Using data from the Norwegian Mother and Child Cohort Study conducted by the Norwegian Institute of Public Health, 34 508 pregnancies were included in the present study. Data were collected by self-completed questionnaires in gestational weeks 17 and 30, and analyzed by logistic regression analysis. The results are presented as adjusted odds ratios (aOR) with a 95% confidence interval. The proportion of women exercising regularly was 46.4% before Pregnancy and decreased to 28.0 and 20.4% in weeks 17 and 30, respectively. Walking and bicycling were the most frequently reported activities before and During Pregnancy. The prevalence of swimming tended to increase from prePregnancy to week 30. Exercising regularly prePregnancy was highly related to regular Exercise in week 17, aOR 5 18.4 (17.1–19.7) and 30, aOR 4.3 (4.1–4.6). Low gestational weight gain was positively associated with regular Exercise in week 30, aOR 5 1.2 (1.1–1.4), whereas being overweight before Pregnancy was inversely associated with regular Exercise in week 17, aOR 5 0.8 (0.7–0.8) and 30, aOR 5 0.7 (0.6–0.7). Also, women experiencing a multiple Pregnancy, pelvic girdle pain, or nausea were less likely to Exercise regularly.

To induce predator stress in pregnant rats, rats were exposed to a hunting dog in an enclosed room. Exposure time was three 10‑min daily sessions separated by 1 h, starting at week 1 of Pregnancy until delivery. Pregnant rats in the Exercise group were forced to run on a treadmill for 30 min once a day, starting one week following Pregnancy until delivery. Rats receiving predator stress During Pregnancy exhibited PTSD anxiety‑like behaviors following delivery. Expression of 5‑hydroxytryptamine (5‑HT) and its synthesizing enzyme tryptophan hydroxylase (TPH) in the dorsal raphe was increased compared with unstressed rats. Expression of c‑Fos and neuronal nitric oxide synthases (nNOS) in the hypothalamus and locus coeruleus were higher in the rats receiving stress During Pregnancy compared with unstressed rats. By contrast, treadmill Exercise During Pregnancy ameliorated anxiety‑like behaviors and reduced the expression of 5‑HT, TPH, c‑Fos and nNOS in the PTSD maternal rats. The results of the present study indicate that Exercise During Pregnancy is suitable for use as a therapeutic strategy to reduce anxiety‑related disorders, including PTSD.

Neurodevelopmental outcome in 7-year-old children is not affected by Exercise During Pregnancy: Follow-up of a multicentre randomised controlled trial.

Design Follow up of a multicentre randomised controlled trial. Setting St Olavs Hospital, Trondheim University Hospital and Stavanger University Hospital, Norway (2007-09). Population Women randomised to follow a 12-week structured Exercise protocol or standard antenatal care During Pregnancy. Methods At 7 years of age, neurodevelopmental outcome was assessed by the parent questionnaire Five-To-Fifteen (FTF), including motor skills, executive functions, perception, memory, language, social skills and possible emotional/behavioural problems. Main outcome measure Continuous and dichotomised (cut-off 90th centile) FTF scores. Results A total of 855 women were randomised to Exercise (n = 429) or standard antenatal care (n = 426) During Pregnancy. At follow up, 164 (38.2%) children born to mothers in the intervention group and 115 (27.0%) children born to mothers in the control group participated. We found no group differences in FTF scores or in the proportion of children with scores ≥90th centile. Stratified analyses by sex, subgroup analyses of women who adhered to the Exercise protocol or sensitivity analyses excluding preterm children and/or children who had been admitted to the neonatal intensive care unit did not change the results. Conclusions In the present randomised controlled trial follow-up study, regular moderate intensity Exercise During Pregnancy did not have adverse effect on neurodevelopment of offspring at 7 years of age. Tweetable abstract Moderate intensity Exercise During Pregnancy had no adverse effect on neurodevelopment of offspring at 7 years of age.

A total of 855 pregnant women were randomized to intervention or control groups. Methods. The intervention was a 12 week Exercise program, including aerobic and strengthening Exercises, conducted between 20 and 36 weeks of Pregnancy. One weekly group session was led by physiotherapists, and home Exercises were encouraged twice a week. The control group received standard antenatal care. Main outcome measures. Self-reports of lumbopelvic pain and sick leave due to lumbopelvic pain. The data were analysed according to the intention-to-treat principle. Results. There were no significant differences between groups of women reporting lumbopelvic pain at 36 weeks (74 vs. 75%, p=0.76). The proportion of women on sick leave due to lumbopelvic pain was lower in the intervention group (22% vs 31%, p=0.01). Conclusions. Exercise During Pregnancy does not influence the prevalence of lumbopelvic pain, but women offered a regular Exercise course seem to handle the disorder better. (Less)

Design Randomized controlled trial. Setting Trondheim and Stavanger University Hospitals, Norway. Population and sample Eight hundred and fifty-five pregnant women were randomized to intervention or control groups. Methods The intervention was a 12 week Exercise program, including aerobic and strengthening Exercises, conducted between week 20 and 36 of Pregnancy. One weekly group session was led by physiotherapists, and home Exercises were encouraged twice a week. Control women received regular antenatal care. Main outcome measures Edinburgh Postnatal Depression Scale (EPDS) completed three months after birth. Scores of 10 or more and 13 or more suggested probable minor and major depression, respectively. Results Fourteen of 379 (3.7%) women in the intervention group and 17 of 340 (5.0%) in the control group had an EPDS score of ≥10 (p=0.46), and four of 379 (1.2%) women in the intervention group and eight of 340 (2.4%) in the control group had an EPDS score of ≥13 (p=0.25). Among women who did not Exercise prior to Pregnancy, two of 100 (2.0%) women in the intervention group and nine of 95 (9.5%) in the control group had an EPDS score of ≥10 (p=0.03). Conclusions We did not find a lower prevalence of high EPDS scores among women randomized to regular Exercise During Pregnancy compared with the control group. However, a subgroup of women in the intervention group who did not Exercise regularly prior to Pregnancy had a reduced risk of postnatal depression.

We searched for studies published between January 1994 and June 2017 that appeared in the Web of Science, Scopus, ClinicalTrials.gov or Cochrane library databases. Randomized controlled trials that investigated the preventive effect of Exercise on GDM in normal-weight women were included. Interventions including any confounding factors (e.g., dietary) were excluded. We extracted maternal characteristics, the diagnostic criteria of GDM, and basic information for intervention and obstetric outcomes. The primary outcome was the occurrence of GDM, and the secondary outcomes included gestational weight gain, gestational age at birth, birth weight, and the odds of cesarean section. A meta-analysis was conducted based on calculations of pooled estimates using the random-effects model. Eight studies were included in this systematic review and meta-analysis. Exercise During Pregnancy was shown to decrease the occurrence of GDM [RR = 0.58, 95% CI (0.37, 0.90), P = 0.01 and RR = 0.60, 95% CI (0.36, 0.98), P = 0.04 based on different diagnosis criteria, respectively] in normal-weight women. Regarding secondary outcomes, Exercise During Pregnancy can decrease gestational weight gain [MD = − 1.61, 95% CI (− 1.99, − 1.22), P<0.01], and had no significant effects on gestational age at birth [MD = − 0.55, 95% CI (− 1.57, 0.47), P = 0.29], birth weight [MD = − 18.70, 95% CI (− 52.49, 15.08), P = 0.28], and the odds of caesarean section [RR = 0.88, 95% CI (0.72, 1.08), P = 0.21], respectively. Exercise During Pregnancy can ostensibly decrease the occurrence of GDM without reducing gestational age at delivery and increasing the odds of cesarean section in normal-weight women.
