Quality of life of families of adolescents undergoing Fixed Orthodontic Appliance therapy: evaluation of a cohort of parents/guardians of treated and untreated individuals.

AIM: To assess the effect of the initial 12 months of Fixed Orthodontic Appliance therapy on the families of Brazilian adolescents. DESIGN: A cohort of 11- to 12-year-old adolescents and their parents/guardians participated. Treatment group (TG) consisted of 101 parents/guardians of adolescents who began Fixed Orthodontic Appliance therapy. They completed four sets of the Family Impact Scale (FIS) at baseline, 1 month, 6 months and 12 months after the bonding of Fixed Appliances. Control group (CG) was composed of 175 parents/guardians of adolescents, who did not begin treatment, answering four sets of the FIS at the same intervals. The subscales of FIS were parental/family activity (PA), parental emotions (PE), family conflict (FC) and financial burden (FB). Statistical analysis was performed. CONCLUSIONS: The effect of Fixed Orthodontic Appliance therapy on the families of adolescents was highly beneficial. Materials and Methods: A case-control study (1∶2) was carried out with a population-based randomized sample of 327 adolescents aged 11 to 14 years enrolled at public and private schools in the City of Brumadinho, southeast of Brazil. The case group (n = 109) was made up of adolescents with a high negative impact on OHRQoL, and the control group (n = 218) was made up of adolescents with a low negative impact. The outcome variable was the impact on OHRQoL measured by the Brazilian version of the Child Perceptions Questionnaire (CPQ11–14) – Impact Short Form (ISF:16). Design: Descriptive study. Setting: Department of Pediatric Dentistry and Orthodontics at Universidade Federal de Minas Gerais, Belo Horizonte, Brazil. Participants: A sample of parents and caregivers of 95 adolescents undergoing Orthodontic treatment with a Fixed Appliance. Methods: Participants were required to answer the Brazilian version of the Parental-Caregivers Perceptions Questionnaire (P-CPQ) before adolescent’s treatment (T1) and 4 months after bonding of the Fixed Appliance (T2). Statistical analysis was carried out using the Wilcoxon signed rank test and the Bonferroni correction for the domains of P-CPQ. Results: Among the 95 participants, there were 73 mothers, 18 fathers and 4 were other relations. There was a statistically significant improvement in the overall score as well as in both emotional and social wellbeing subscales (P,0.001). Conclusion: Parents and caregivers report an improvement on their adolescent’s OHRQoL in the first 4 months of Orthodontic treatment with a Fixed Appliance.

Discomfort associated with Fixed Orthodontic Appliances: determinant factors and influence on quality of life

MATERIAL AND METHODS : Two hundred and seventy-two individuals aged between 9 and 18 years old, enrolled in public and private schools and undergoing Orthodontic treatment with Fixed Appliance participated in this cross-sectional study. The participants were randomly selected from a sample comprising 62,496 individuals of the same age group. Data was collected by means of questionnaires and an interview. Discomfort intensity and bio-psychosocial variables were assessed using the Oral Impact on Daily Performance questionnaire. Self-esteem was determined using the Global Negative Self-Evaluation questionnaire. Statistical analysis involved the chi-square test and both simple and multiple Poisson regression analyses. RESULTS: Although most individuals did not present discomfort, there was a prevalence of 15.9% of impact on individuals' daily life exclusively due to the use of Fixed Orthodontic Appliance . Age [PR: 3.2 (95% CI: 1.2-8.5)], speech impairment [PR: 2.2 (95% CI: 1.1-4.6)], poor oral hygiene [PR: 2.4 (95% CI: 1.2-4.8)] and tooth mobility [PR: 3.9 (95% CI: 1.8-8.1)] remained independently associated with a greater prevalence of discomfort (P ≤ 0.05). CONCLUSIONS: Discomfort associated with the use of Fixed Orthodontic Appliances exerted a negative influence on the quality of life of the adolescents comprising the present study. The determinants of this association were age, poor oral hygiene, speech impairment and tooth mobility.

As such a fine adjustment is only required for selected teeth, the aim of this pilot study was to investigate tooth mobility in vivo on corrected and uncorrected subgroups under positioner therapy. METHODS: Mobility was measured on upper teeth of 10 patients (mean age 16.8) by applying loadings for 0.1, 1.0 and 10.0 s with a novel device directly after multibracket Appliance debonding as much as 2d, 1, 2 and 6 weeks later. Positioners were inserted at day 2. Specimens were divided into Group C (teeth corrected via positioner), Group N (uncorrected teeth adjacent to teeth from group C), and Group U (uncorrected teeth in an anchorage block). Untreated individuals served as controls (n = 10, mean age 22.4). Statistics were performed via Kolmogorov-Smirnov test and Welch's unequal variances t-test for comparisons between groups. P < 0.05 was considered statistically significant. RESULTS: After 1 week, tooth mobility in Group U almost resembled controls (13.0-15.7 N), and reached physiological values after 6 weeks (17.4 N vs. 17.3 N in controls). Group C (9.0-13.4 N) and Group N (9.2-14.7 N) maintained increased mobility after 6 weeks. Tooth mobility was generally higher by reason of long loading durations (10.0 s). CONCLUSIONS: Positioner therapy can selectively utilized increased tooth mobility upon Orthodontic Fixed Appliance treatment for case refinements. Here, uncorrected teeth in anchorage blocks are not entailed by unwanted side effects and recover after 6 weeks post treatment. Corrected teeth and their neighbors exhibit enhanced mobility even after 6 weeks, which represents a necessity for the proper correction of tooth position, and concurrently arouses the requirement for an adequate retention protocol.
