Although it is performed less frequently with the advancement of limb salvage surgery, FQA remains an important treatment in select patients. The purpose of this study is to illustrate the surgical technique in a case series of 4 patients. Methods Between 2010 and 2012, 4 patients (mean age, 61 years; range, 36-78 years) presented with malignant disease of the upper extremity that was not amenable to or had failed limb salvage. All patients had FQA by the illustrated anterior clavicular osteotomy technique. Patient data were retrospectively reviewed from preoperative workup until last follow-up or death. Results All patients had tumors that involved major neurovascular structures of the upper extremity and shoulder girdle. One presented with neuroendocrine carcinoma and has achieved local control after FQA. Three presented with high-grade sarcoma. One of these had recurrence after prior limb salvage and neoadjuvant radiation and unfortunately succumbed to metastatic disease 6 months after FQA. An additional sarcoma patient who presented after shoulder arthroscopy for a “labral cyst” with recurrent and fulminant synovial sarcoma succumbed to her disease. The remaining sarcoma patient has had no recurrence and minimal phantom pain at last follow-up. Discussion Obtaining vascular control early in the procedure is crucial to minimize blood loss. When it is indicated, FQA is a relatively safe and reliable procedure for dealing with otherwise challenging tumors of the shoulder girdle and upper extremity.

Forequarter Amputation with fasciocutaneous deltoid flap reconstruction for malignant tumors of the upper extremity

KarakousisAbstract:Background: Malignant tumors of the upper extremity involving a considerable portion of the medial axillary wall may require Forequarter Amputation to achieve gross resection of tumor. These resections frequently leave a large defect, often requiring a split thickness skin graft or free flap to close the wound. To address this problem of wound closure, we have modified our technique and devised a reconstructive component as part of our Forequarter Amputation procedure. Methods: The medical records of seven patients who underwent Forequarter Amputation and fasciocutaneous deltoid flap reconstruction between 1982 and 1994 were reviewed. Results: All the Amputation sites were completely closed with a fasciocutaneous deltoid flap without the use of additional skin grafts or free flaps. After a median follow-up of 12 months, there were no local recurrences. Three patients (43%) are alive and disease free 5, 12, and 19 months after their Forequarter Amputation. One patient is alive with disease after 14 months. The remaining three patients died of their disease. Conclusion: The fasciocutaneous deltoid flap is technically easy to perform, provides wound coverage without the use of skin grafts, and is especially useful for tumors involving the media axillary wall and in patients with previous axillary radiation.

Claviculectomy for the exposure and en bloc resection of adjacent tumors

ZografosAbstract:Claviculectomy has been described for primary or metastatic tumors of the clavicle. In 11 patients, claviculectomy was used as a technical expedient for the exposure and en bloc resection of large, underlying nodal metastases from melanoma (7 patients) and soft tissue tumors (4 patients). There were no wound complications. Three patients developed moderate edema of the arm. There was little limitation at the shoulder, and the use of the upper extremity has been essentially normal. Claviculectomy is well tolerated, provides good exposure of the underlying neurovascular structures, and, in some patients, provides a method of limb salvage for underlying tumors in preference to Forequarter Amputation.
