Design and methods 90 outpatients with recurrent herpes labialis were instructed to apply the antiviral cream to the lesions according to package insert for up to 10 days. Symptoms were recorded before beginning of treatment, after three days, and at the end of treatment. Results Paresthesia and swelling as well as the number of vesicles were reduced after three days of observation, and were improved by about 90% at the end of treatment. Erosions and crust formation also improved markedly. Efficacy as well as tolerability were evaluated as positive by the majority of physicians and patients. No adverse reactions were reported. Conclusions Foscarnet Sodium 2% cream is an effective and safe medication for the treatment of recurrent labial herpes in daily routine practice of dermatologists.

Hintergrund und Fragestellung Ziel der Anwendungsbeobachtung war, weitere Erkenntnisse zur Wirksamkeit und Verträglichkeit von Foscarnet-Natrium-Creme 2% bei Patienten mit Herpes labialis recidivans unter den Bedingungen der täglichen Praxis zu gewinnen. Studiendesign und Untersuchungsmethoden 90 Patienten mit Herpes labialis recidivans wurden angewiesen, die Antiviralcreme bis zu 10 Tage lang auf die betroffenen Stellen aufzutragen. Symptome wurden zu Behandlungsbeginn, nach 3 Tagen und am Ende der Behandlung dokumentiert. Ergebnisse Die Symptome Parästhesie und Schwellung sowie die Zahl an Vesikeln waren im Vergleich zum Behandlungsbeginn schon bei der Verlaufskontrolle nach 3 Tagen deutlich reduziert und hatten sich bis zum Behandlungsende durchschnittlich um etwa 90% gebessert. Zum Ende der Behandlung war auch beim Befall mit Erosionen und Krusten eine deutliche Verbesserung zu beobachten. Sowohl Wirksamkeit als auch Verträglichkeit wurden von der Mehrheit von Ärzten und Patienten positiv beurteilt. Kein Patient berichtete unerwünschte Arzneimittelwirkungen. Schlussfolgerungen Die 2%ige Foscarnet-Natrium-Creme erweist sich in unserer Anwendungsbeobachtung in der Praxis des niedergelassenen und ambulant tätigen Dermatologen unter Alltagsbedingungen als ein gut wirksames und äußerst verträgliches Arzneimittel zur Behandlung von Herpes labialis recidivans.

Synthesis, Hydrolytic Behavior, and Anti-Hiv Activity of Selected Acyloxyalkyl Esters of TriSodium Phosphonoformate (Foscarnet Sodium)

Thakker, William EganAbstract:The synthesis and anti-HIV activity of selected (acyloxy)alkyl esters of triSodium phosphonoformate (Foscarnet Sodium) are described. The conversion of bis(trimethylsilyl) (alkoxycarbonyl)phosphonates 11a–d to the corresponding disilver salts 12a–d and their subsequent reaction with iodoalkyl acrylates 4a–c gave the desired bis-(acyloxyalkyl) phosphonates 6–9(a–c). Of the analogs tested, only the dichlorophenyl analog 9a showed a dose-dependent inhibition of HIV activity in H9 cells. Using 31P-NMR, bioreversibility has been investigated in an attempt to rationalize these results.

A Study of Efficacy of Phosphonoformate in Treatment of Chronic Hepatitis B
The Journal of Medical Theory and Practice, 2005Co-Authors: Zeng ChuanshenAbstract:Objective: To study the efficacy of phosphonoformate (Foscarnet Sodium) in treatment of chronic hepatitis B. Methods: 100 patients with chronic hepatitis B were chosen and given phosphonoformate injection 3g/300ml ivgtt, bid×15 days, then qd, total 30 days; the control group (31 cases) were given only liver protecting drugs, ivgtt ,total 30 days. Before and after treatment, patients' blood was examined for serum HBV markers, HBV-DNA and liver function changes. Results: After treatment, the negative reactions of HBeAg in both groups were 46% ,22.58%, respectively, and the negative reactions of HBV-DNA were 66% and 19.35%, respectively, with significant difference (P0.05). Also liver functions were recovered better in the Phosphonoformate group than that in the control group. HBsAg negative reactions had no meaning. Conclusion: Infusion of phosphonoformate in treatment of chronic hepatitis B has a definite effect on inhibition of HBV replication and recovery of liver functions.

Thalidomide for the treatment of acquired immunodeficiency syndrome-associated refractory oral ulcers

The lesions did not respond to acyclovir, prednisone, pentoxifylline, or Foscarnet Sodium therapy. Dramatic clinical improvement and progressive ulcer healing were observed after starting oral thalidomide therapy. Clinicians should be aware of the usefulness of thalidomide for the treatment of acquired immunodeficiency syndrome-associated not otherwise specified ulcerations.
