    Abstract:

    Background: Effective perioperative analgesia is lacking for children owing to interindividual variations and underdosing of opioids caused by fear of adverse effects. We investigated the role of COMT SNPs on postoperative Pain management in children. Methods: One hundred and forty nine children undergoing adenotonsillectomy were enrolled. The associations of four COMT SNPs (rs6269, rs4633, rs4818 and rs4680) with postoperative Pain were analyzed and outcome measures included maximum Pain scores, need for postoperative opioid interventions and postoperative morphine requirements. Results: We detected an association of postoperative opioid intervention need with all four COMT SNPs. Minor allele carriers of COMT SNPs were approximately three-times more likely to require analgesic interventions than homozygotes of major alleles (p-value range: 0.0031–0.0127; odds ratio range: 2.6–3.1).     Abstract:

    Pain is an increasing clinical challenge affecting about half the population, with a substantial number of people suffering daily intense Pain. Such suffering can be linked to the dramatic rise in opioid use and associated deaths in the United States. There is a pressing need for new analgesics with limited side effects. Here, we summarize what we know about the Genetics of Pain and implications for drug development. We make the case that chronic Pain is not one but a set of disease states, with peripheral drive a key element in most. We argue that understanding redundancy and plasticity, hallmarks of the nervous system, is critical in developing analgesic drug strategies. We describe the exploitation of monogenic Pain syndromes and genetic association studies to define analgesic targets, as well as issues associated with animal models of Pain. We appraise present-day screening technologies and describe recent approaches to Pain treatment that hold promise.

    The debate over a reproducibility crisis has been simmering for years now, amplified by growing concerns over a number of reproducibility studies that have failed to replicate previous positive results. Additional evidence from larger meta‐analysis of past papers also points to a lack of reproducibility in biomedical research with potentially dire consequences for drug development and investment into research. One of the largest meta‐analyses concluded that low levels of reproducibility, at best around 50% of all preclinical biomedical research, were delaying lifesaving therapies, increasing pressure on research budgets and raising costs of drug development [1]. The paper claimed that about US$28 billion a year was spent largely fruitlessly on preclinical research in the USA alone. ### A problem of statistics However, the assertion that a 50% level of reproducibility equates to a crisis, or that many of the original studies were really fruitless, has been disputed by some specialists in replication. “A 50% level of reproducibility is generally reported as being bad, but that is a complete misconstrual of what to expect”, commented Jeffrey Mogil, who holds the Canada Research Chair in Genetics of Pain at McGill University in Montreal. “There is no way you could expect 100% reproducibility, and if you did, then the studies could not have been very good in the first place. If people could replicate published studies all the time then they could not have been cutting edge and pushing the boundaries”. One reason not to expect 100% reproducibility in preclinical studies is that cutting edge or exploratory research deals with a lot of uncertainty and competing hypotheses of which only a few can be correct.
