Hormonal therapy and Genital Tract Cancer.

WrenAbstract:The use of hormonal replacement therapy following Genital Tract Cancer is often denied to women because of the fear of increasing the risk of recurrence or new growth. The paucity of articles written on this subject during the past decade is an indication of the attitude of the medical profession to the needs of women suffering from symptoms of sex hormone deficiency. During 1994-1995 there were few articles published on the use of hormonal therapy to treat menopausal symptoms following Genital Tract Cancer. Several articles, however, reviewed the relationship between hormones and Genital Tract Cancer, some explored the value of antioestrogens in controlling recurrent or secondary disease, and a few others discussed the risk of developing uterine Cancer when tamoxifen was used to manage postmenopausal breast Cancer. Some suggestions are made that will allow women suffering from symptoms of hormone deficiency to receive alternative regimens of hormonal therapy. Maintaining quality of life without reducing the potential length of life is paramount in reaching a decision on the use of hormonal therapy following Genital Tract Cancer.

WrenAbstract:Following treatment of Genital Tract Cancer, women are often left in an estrogen-deficient state suffering both short- and long-term effects which markedly reduce the quality of life. In the belief that sex hormones may induce an adverse effect on all these women, attending physicians often refuse to prescribe hormonal replacement therapy (HRT). In an attempt to introduce a rational approach to this problem the effect of sex hormones on Genital Tract Cancer growth is reviewed and some general guidelines for administering sex hormones to women following treatment for Genital Tract Cancer are developed. A plea is made to improve the well-being of patients following Cancer treatment by considering the judicious use of appropriate hormonal therapy, including estrogens.

Warts, Cancer and ubiquitylation: lessons from the papillomaviruses.

HowleyAbstract:Certain human papillomaviruses (such as HPV-16 and HPV-18) are associated with specific anoGenital Cancers, most notably cervical Cancer. These viruses encode two oncoproteins, E6 and E7, which are expressed in the HPV positive Cancers. E7 functions in cellular transformation, at least in part, through inactivation of pRB, and the other pRB related “pocket proteins” p107 and p130. The major target of the E6 oncoprotein encoded by the Genital Tract, Cancer-associated human papillomaviruses is the p53 tumor suppressor protein. E6 binding to p53 is mediated by a cellular protein, the E6-associated protein (E6AP). In the presence of E6, E6AP catalyses the ubiquitylation and proteolysis of p53. E6AP is an E3 ubiquitin protein ligase and is not normally involved in the regulation of p53 stability in the absence of E6. E6AP is the prototype for the HECT domain family of E3 ubiquitin protein ligases.

Several lines of evidence suggest that E6 and E7 have additional targets important to the oncogenic potential of the virus. Work from a number of laboratories has focused on determining other activities of HPV relevant to carcinogenesis and identifying additional cellular targets of E6 and E7. This paper will review the state of the field at the time of the 19th International Papillomavirus Workshop in September 2001 with respect to the HPV encoded oncoproteins.

From the point of view of the infection, human papillomaviruses have developed several molecular mechanisms to enable infected cells to suppress apoptosis. This review provides a comprehensive and critical summary of the current literature that focuses on cervical carcinoma and Cancer of the head and neck caused by HPV. In particular, we discuss HPV virology, the molecular mechanisms of carcinogenesis, the role of the tumor suppressor protein p53 and the E6/E7 zinc finger proteins. Classification of HPV according to diagnosis is also described.
