    This section of the International Society for Heart and Lung Transplantation (ISHLT) Registry Report covers the pediatric lung and Heart-Lung Transplantation experience from January 1986 through June 2011. During this time, 1,770 pediatric lung Transplant and 660 pediatric Heart-Lung Transplant procedures were reported to the Registry. In 2010, the last complete year included in this 15th official Pediatric Lung and Heart-Lung Transplantation Report, 126 pediatric lung Transplant procedures were reported, compared with 97 procedures in 2005. The figures, however, contain only the pediatric lung Transplant procedures that were reported to the ISHLT Registry and, thus, may not reflect numbers of pediatric lung procedures undertaken worldwide. This year’s Registry report incorporates a summary of data analyses performed by the Registry this year that is supplemented by further analyses and additional figures that are available on the ISHLT Registry’s Web site (www.ishlt.org/registries). The 2012 Registry Report does not include detailed analyses and figures of pediatric lung reTransplantation because those data were presented in last year’s report and the annually performed number of pediatric lung reTransplant procedures is low. Data on pediatric Heart-Lung Transplantation are presented in this year’s ISHLT Registry Report as these data were not displayed in the 2011 Registry Report.

    The Registry of the International Society for Heart and Lung Transplantation (ISHLT) has documented trends in clinical lung and Heart-Lung Transplantation since the inception of these procedures. Detailed Heart-Lung Transplantation data have been included in the Registry annual reports since 1984 and lung Transplantation data since 1989. Through June 30, 2010, the Registry contains data on 4,248 Heart-Lung and 38,119 lung Transplants from centers around the world. This 28th Lung and Heart-Lung Registry Report summarizes the current status of lung and Heart-Lung Transplantation by reporting data on this international group of patients, focusing on adults. More detailed information is presented in the full set of more than 200 slides that can be found on the ISHLT Web site at www.ishlt.org/registries/.

    This Fourteenth Pediatric Lung and Heart-Lung Transplantation Report covers the pediatric lung Transplantation and Heart-Lung Transplantation experience from January 1986 through June 2010. During this period, 1,664 pediatric lung Transplant procedures and 653 pediatric Heart-Lung Transplant procedures were reported to the International Society for Heart and Lung Transplantation (ISHLT) Registry. It is important to note that recent figures include pediatric lung and Heart-Lung Transplant procedures dating back to 1987 that were previously not reported to the ISHLT Registry but were retrospectively entered in 2010. In 2009, the last complete year included in this year’s ISHLT Registry report, 127 pediatric lung Transplant procedures were reported, which is an increase of 43% compared with 2004. These figures, however, include only the pediatric lung Transplant procedures that were reported to the ISHLT Registry and do not necessarily reflect the trend in numbers of pediatric lung procedures performed worldwide. This pediatric lung and Heart-Lung Transplantation report includes a summary of the data analyses performed by the Registry this year and is supplemented by further analyses and additional figures that are available on the ISHLT Registry Web site (www.ishlt.org/registries).     This tenth official pediatric report of the International Society for Heart and Lung Transplantation (ISHLT) covers the international pediatric lung and Heart-Lung Transplantation experience from 1982 to 2006. As of last year's report, pediatric lung and Heart-Lung Transplant data are now reported separately from pediatric heart Transplant data and adult lung Transplant data. For the first time this year, Registry data are analyzed by geographic region in addition to the usual aggregate analyses. All figures and tables included in this report and additional supplementary slides are available from the ISHLT website (www.ishlt.org/registries).

    Abstract:

    Over the last decade, improvements in surgical techniques, lung preservation, immunosuppression, and management of ischemia–reperfusion injury and infections have contributed to increase the 1-year patient survival after lung Transplantation to 70–80% (1). However, long-term survival is threatened by bronchiolitis obliterans, which is thought to be a form of chronic allograft rejection. Bronchiolitis obliterans after lung Transplantation was first described in 1984 at Stanford University in heart–lung Transplant recipients who showed a progressive decline in FEV 1 (2). Lung biopsies from these patients showed intraluminal polyps comprised of fibromyxoid granulation tissue and plaques of dense submucosal eosinophilic scar. Obliteration of the small airways by these lesions produces progressive airflow obstruction, often accompanied by recurrent lower respiratory tract infection. Bronchiolitis obliterans, and its clinical correlate bronchiolitis obliterans syndrome, affect up to 50–60% of patients who survive 5 years after Transplantation (3). In most patients, bronchiolitis obliterans is a progressive process that responds poorly to augmented immunosuppression, and it accounts for more than 30% of all deaths occurring after the third postoperative year (1). Survival at 5 years after the onset of bronchiolitis obliterans is only 30–40%, and survival at 5 years after Transplantation is 20–40% lower in patients with than in patients without bronchiolitis obliterans (4). In this review, we present current concepts regarding post-Transplant bronchiolitis obliterans, including: ( 1 ) the recently updated classification system for bronchiolitis obliterans syndrome (BOS), ( 2 ) an overview of current concepts regarding its pathogenesis and risk factors, ( 3 ) potential surrogate markers that may contribute to early detection, and ( 4 ) approaches to the management of this devastating complication.

    This section of the 19th Official Registry Report of 2016 summarizes data from pediatric lung Transplant recipients and their donors for Transplants that occurred through June 30, 2015. This report describes donor and recipient characteristics, Transplant type and recipient outcomes data. The full Registry slide set available online (www.ishlt.org/ registries) provides more detail, additional analyses and other information not included in this printed report. This year’s report focuses on primary diagnostic indications for Transplant. We present data on the distribution of diagnostic categories, demographics of patients with the different lung disorders leading to the need for Transplant (Figure 1), associations of the diagnoses with outcomes, and other data of interest related to this topic. Data on Heart-Lung Transplantation in children are not presented in this year’s report, as the number of pediatric heart–lung Transplant procedures has remained very low.     This Fourteenth Pediatric Lung and Heart-Lung Transplantation Report covers the pediatric lung Transplantation and Heart-Lung Transplantation experience from January 1986 through June 2010. During this period, 1,664 pediatric lung Transplant procedures and 653 pediatric Heart-Lung Transplant procedures were reported to the International Society for Heart and Lung Transplantation (ISHLT) Registry. It is important to note that recent figures include pediatric lung and Heart-Lung Transplant procedures dating back to 1987 that were previously not reported to the ISHLT Registry but were retrospectively entered in 2010. In 2009, the last complete year included in this year’s ISHLT Registry report, 127 pediatric lung Transplant procedures were reported, which is an increase of 43% compared with 2004. These figures, however, include only the pediatric lung Transplant procedures that were reported to the ISHLT Registry and do not necessarily reflect the trend in numbers of pediatric lung procedures performed worldwide. This pediatric lung and Heart-Lung Transplantation report includes a summary of the data analyses performed by the Registry this year and is supplemented by further analyses and additional figures that are available on the ISHLT Registry Web site (www.ishlt.org/registries).     This section of the International Society for Heart and Lung Transplantation (ISHLT) Registry Report covers the pediatric lung and Heart-Lung Transplantation experience from January 1986 through June 2011. During this time, 1,770 pediatric lung Transplant and 660 pediatric Heart-Lung Transplant procedures were reported to the Registry. In 2010, the last complete year included in this 15th official Pediatric Lung and Heart-Lung Transplantation Report, 126 pediatric lung Transplant procedures were reported, compared with 97 procedures in 2005. The figures, however, contain only the pediatric lung Transplant procedures that were reported to the ISHLT Registry and, thus, may not reflect numbers of pediatric lung procedures undertaken worldwide. This year’s Registry report incorporates a summary of data analyses performed by the Registry this year that is supplemented by further analyses and additional figures that are available on the ISHLT Registry’s Web site (www.ishlt.org/registries).     The Registry of the International Society for Heart and Lung Transplantation (ISHLT) has documented trends in clinical lung and Heart-Lung Transplantation since the inception of these procedures. Detailed Heart-Lung Transplantation data have been included in the Registry annual reports since 1984 and lung Transplantation data since 1989. Through June 30, 2010, the Registry contains data on 4,248 Heart-Lung and 38,119 lung Transplants from centers around the world. This 28th Lung and Heart-Lung Registry Report summarizes the current status of lung and Heart-Lung Transplantation by reporting data on this international group of patients, focusing on adults.     This tenth official pediatric report of the International Society for Heart and Lung Transplantation (ISHLT) covers the international pediatric lung and Heart-Lung Transplantation experience from 1982 to 2006. As of last year's report, pediatric lung and Heart-Lung Transplant data are now reported separately from pediatric heart Transplant data and adult lung Transplant data. For the first time this year, Registry data are analyzed by geographic region in addition to the usual aggregate analyses.
