LEIOMYOSARCOMA OF THE SUPERIOR VENA CAVA PRODUCING SUPERIOR VENA CAVA SYNDROME AND Heart Tamponade

A 44 year old Japanese man presented with superior vena cava syndrome and eventually died due to Heart Tamponade and acute renal failure. Autopsy revealed that the superior vena cava was occluded with a tumor that had invaded the pericardium and right thoracic cavity. Primary caval venous leiomyosarcoma is a rare but lethal disease and most cases arise from the inferior vena cava. This case represents a very rare case of leiomyosarcoma with the rare clinical findings of superior vena cava syndrome and Heart Tamponade.

Mitral mechanical valve without long-term anticoagulation. Eight-year follow-up.
Scandinavian journal of thoracic and cardiovascular surgery, 1994Co-Authors: Viking Olov Björk, Ary Ribeiro, Mario Canetti, Vollmer BomfimAbstract:In 12 patients with sinus rhythm (including 5 children and 6 young women), mitral valve replacement was performed with a microporous-surfaced valve similar to the Bjork-Shiley Monostrut. After the first 3 months, permitting endothelialization of the suture ring to continue over the groove and adjacent metal valve ring, no long-term anticoagulant treatment was given. There was no thromboembolic complication in this group during follow-up for 6-8 years, during which four women gave birth to a total of seven children. In eight other cases, one mitral case with atrial fibrillation, anti-coagulant was not discontinued, and in the remaining aortic cases it was reinstituted. One of them (with atrial fibrillation) had hematuria during inadequate anticoagulant medication, but no thromboembolism. Of five patients with only aortic valve replacement, two had thromboembolic complications, one without residual symptoms and one with slight hand weakness. Another had a transient ischemic attack while on anticoagulant and acetylsalicylic acid was added. Two patients with aortic and mitral valve replacement died, one from Heart Tamponade and the other from venous thrombosis with pulmonary embolism.

Primary cardiac B-cell lymphoma presented as Heart Tamponade and atrioventricular block: a case report.

CASE A 49-year-old man presented with Heart Tamponade and atrioventricular block. Bloody pericardiac effusion showed a monotonous proliferation of atypical large mononuclear cells, which demonstrated a lambda light-chain monoclonality by the fluorescence-activated cell-sorter method and clonal rearrangement bands by Southern blot analysis of the IgH gene. Transvenous biopsy excised from the right atrial tumor was diagnosed as diffuse large B-cell lymphoma. He underwent chemotherapy and permanent pacemaker implantation and is alive and well. CONCLUSION Liquid cytology of cardiac effusion was very useful for rapid diagnosis, leading to a better prognosis.

A soccer victim: late rupture of the ascending aorta after violent physical trauma
International journal of cardiology, 1998Co-Authors: S.a.m Said, L.m Keeris, G Van IngenAbstract:A 56-year-old man without previous medical history or risk factors for coronary artery disease, presented with chest pain 2 weeks after violent blunt chest trauma. He was as a spectator, beaten and kicked almost to death during an amateur soccer match. Because of embarrassment, this was denied till shortly after his death. Shortly before the patient died, thoracic CT-scan showed a dissecting aneurysm of the ascending aorta in association with pericardial and bilateral pleural effusion. He suddenly died in-hospital due to Heart Tamponade secondary to rupture of the aneurysm into the pericardial space. At autopsy, the intimal layer of the aorta showed no atherosclerotic changes. The intimal rupture was located just above the origin of the right coronary artery.
