    Abstract:

    Objective To identify a form of binocular vision dysfunction (vertical Heterophoria) in a traumatic brain injury (TBI) population and to assess the effect of individualized prismatic spectacle lenses on postconcussive symptoms. Design Retrospective study. Setting Private physical medicine and rehabilitation practice and private optometric practice. Patients A subset of TBI patients who were initially evaluated by a single physiatrist and who received standard treatments and medications yet had persistent postconcussive symptoms. These patients were then assessed by a single optometrist, and those found to have vertical Heterophoria were treated with individualized prismatic spectacle lenses. A total of 83 patients were referred for testing; 77 were positive for vertical Heterophoria on screening, of which 43 had complete data sets and were included for analysis. Interventions All patients were treated with individualized prismatic spectacle lenses to correct for vertical Heterophoria. Main Outcome Measures Outcomes were measured by the difference in score before and after intervention of an objective, self-administered vertical Heterophoria symptom burden instrument (Vertical Heterophoria Symptom Questionnaire [VHS-Q], presently undergoing validation) and by subjective improvement in symptoms as expressed by the patient at the end of intervention. Results There was a 71.8% decrease in subjective symptom burden when compared with preintervention baseline. There was a mean 16.7 point absolute reduction in the VHS-Q score on a 75-point scale, which represents a relative reduction in VHS-Q score of 48.1%. Conclusion Vertical Heterophoria was identified in a group of TBI patients with postconcussive symptoms and treatment of the vertical Heterophoria with individualized prismatic spectacle lenses resulted in a 71.8% decrease in subjective symptom burden and a relative reduction in VHS-Q score of 48.1%. It appears that vertical Heterophoria can be acquired from TBI.

    Purpose The aim of this study was to determine the prevalence and risk factors of near decompensated Heterophoria in a population of university students. Methods In a randomized study, 406 students of six schools of Mashhad University of Medical Sciences were selected and classified into symptomatic and non symptomatic groups. Visual acuity (VA), near point of convergence (NPC), Near point of accommodation (NPA), dissociated and associated phoria, suppression, stereopsis, accommodative facility, relative accommodation, accommodative convergence/accommodation ( AC/A ratio and fusional reserves were measured in two groups. The data were analysed by using SPSS software, version 11.5o. Results The prevalence of symptoms and binocular disorders in students were 41.6% and 21.8% respectively . Of the students,9.1% had near decompensated Heterophoria (Heterophoria with symptoms). The results of this study showed that VA, NPC, NPA, Heterophoria, stereopsis, accommodative facility, relative accommodation, AC/A ratio and fusional reserves were different in symptomatic and asymptomatic subjects and were worsen in students with decompensated Heterophoria (P<0.05). Conclusion The results of this study indicated relatively high prevalence of symptomatic students, binocular dysfunction and decompensated Heterophoria in university students. Students with decompensated Heterophoria failed in more visual skills which can be risk factors for decompensating the Heterophoria.

    Introduction The study aims to explore the prevalence of Heterophoria and associate factors in Tibetan grade-one students. Methods The Lhasa Childhood Eye Study (LCES) is a school-based cohort study. 1942 grade-one students from 7 elementary schools were randomly sampled by stratified cluster sampling. Ocular examinations were performed in participants, including ocular motility, distance and near visual acuity, cycloplegic autorefraction, and stereoacuity. The near (33 cm) and distance (6 m) fixation cover test was used to differentiate orthophoria, Heterophoria, and heterotropia, and the magnitude of the phoria was measured by the Maddox rod and prisma. Results Of 1856 grade-one students completing all the ocular examinations, 1852 participants finished the ocular alignment test. The mean age was 6.82 ± 0.46 years (range 6∼10 years); 981 (52.97%) were males, and 871 (47.03%) were females. The prevalence of phoria was 22.89% (n = 424). At distance fixation, the prevalence of Heterophoria, exophoria, and esophoria was 4.64%, 4.21%, and 0.43% separately, while at near fixation, the prevalence was 22.73%, 22.35%, and 0.38%. No vertical phoria was detected. The mean magnitude of Heterophoria at near and distance fixation was -7.63 ± 5.15 PD (exo: -7.83 ± 4.91 PD, eso: +5.67 ± 3.61 PD) and -4.84 ± 5.94 PD (exo: -6.26 ± 4.20 PD, eso: +8.13 ± 3.04 PD). The prevalence of esophoria was associated with hyperopia (OR = 6.38, 95% CI: 1.15-35.28, P = 0.03; OR = 5.42, 95% CI: 1.04-28.24, P = 0.04) and amblyopia (OR = 16.02, 95% CI: 1.81∼141.96, P = 0.01; OR = 11.37, 95% CI: 1.34∼96.52, P = 0.03) at near and distance fixation. The prevalence of exophoria was associated with myopia at near fixation (OR = 2.43, 95% CI: 1.47-4.00, P<0.01). In the near Heterophoria group, the proportion of children with abnormal stereoacuity was 23.26% (n = 97), significantly higher (χ2 = 5.70, P = 0.017) than that in orthophoria (17.99%, n = 244). Conclusions In Lhasa, grade-one pupils have a lower prevalence of Heterophoria. Near exophoria was associated with myopia, while esophoria was related to hyperopia and amblyopia both near and distance. Heterophoria may be one of the affected factors for reducing stereoacuity.

    Background: Previous studies have found that crossed disparities give better stereoacuity than uncrossed. When near phoria is considered, exophores had a better crossed stereoacuity and esophores had a better uncrossed stereoacuity. The current study investigated the effect of Heterophoria on distant crossed and uncrossed stereoacuity. Methods: Seventy-two subjects were recruited and their distant Heterophoria measured. Heterophoria within two prism dioptres was considered as orthophoric. Esophoric group had esophoria greater than two prism dioptres. Exophoric group had exophoria greater than two prism dioptres. There were 40 orthophores, 23 exophores and nine esophores. Their stereoacuity was measured with a three-rod apparatus at 6 m. Results: The mean crossed stereoacuity was 4.8″ and uncrossed stereoacuity was 7.2″ (t=−3.03, p < 0.01). The mean stereoacuity for orthophoric subjects was 5.31″, 6.02″ for exophores and 8.91″ for esophores. The distant crossed stereoacuity is better than uncrossed stereoacuity in all three groups but the difference is only significant for the exophores. Exophoric subjects demonstrated a significant difference between crossed (4.10″) and uncrossed (7.95″) stereoacuity. Conclusions: Orthophores have the best stereoacuity, followed by exophores and esophores. Exophoric subjects have a better crossed than uncrossed stereoacuity. More esophoric subjects should be recruited to confirm the difference between crossed and uncrossed stereoacuity.
