Thymic Humoral Factor gamma 2 is a thymus-derived synthetic octapeptide, shown to be effective in chronic hepatitis B virus infection; as the latter is needed to support hepatitis D virus, thymic Humoral Factor-gamma 2 may have a therapeutic role in hepatitis D. Aim . To evaluate tolerability and efficacy of thymic Humoral Factor-gamma 2 in chronic hepatitis D. Methods . Intramuscular thymic Humoral Factor-gamma 2, 40 μg, was given for 15 consecutive days and twice weekly for 22 additional weeks to adult patients with chronic hepatitis D virus hepatitis. Results . A total of 11 patients (male/female 912, mean age 45.9 years) completed the treatment period, 10 the 6-month follow-up. At baseline, hepatitis D virus-RNA was positive in 8/11 (73%) patients. During treatment, hepatitis D virus-RNA became undetectable in 3/8 (37%), decreased in 1/8 (13%), remained unchanged in 4/8 (50%) and persisted undetectable in 3 patients, negative at baseline. During follow-up hepatitis D-viraemia relapsed in all patients but 2, one already negative at baseline. No changes in hepatitis B virus markers occurred. Mean serum alanine aminotransferase levels did not change significantly. None of the patients reached normal serum alanine aminotransferase levels. Conclusion . At the doses given, thymic Humoral Factor-gamma 2 has been of limited efficacy. A possible role of thymic Humoral Factor-gamma 2 in the treatment of chronic hepatitis D requires further dose-finding studies and/or combination with other antivirals.

Methods: Nine HBeAg and HBV-DNA seropositive patients removed thymus Humoral Factor-gamma 2 alone for 2 months, thymus Humoral Factor-gamma 2 plus alpha-interferon for 2 months and finally alpha-interferon alone for 2 months. Results: Treatment with thymus Humoral Factor-gamma 2 alone was not associated with any side effects. The interferon-induced lymphopenia was significantly less marked during the combined therapy in comparison to the previous course with interferon alone (mean reduction of lymphocyte counts 33.5±11.6% versus 56.3±16.7%, respectively, p p Conclusions: These data suggest that thymus Humoral Factor-gamma 2 may be useful in a combined therapeutic approach in chronic HBV carriers.

Zidovudine and thymus Humoral Factor gamma-2 in the treatment of HIV infection: Preliminary clinical experience

Suter, Maria Grazia Pravettoni, Manuela Leone, Antonella IngrossoAbstract:A case-control, prospective, open-label, clinical trial to evaluate efficacy and safety of a combined zidovudine/Thymus Humoral Factor Gamma-2 (THF) therapy in HIV-infected subjects was conducted in 13 patients. Twenty-six patients were included as controls receiving only zidovudine. The two groups of patients were matched according to sex, age, CDC stage of HIV infection, number of CD4+ T cells and type of previous opportunistic infections (if any) and all patients and controls were naive for antiretroviral therapy at the moment they entered the trial. The observation period was protracted up to 47 months (mean 28±13 months). No significant difference was observed between the two groups as far as surrogate markers of HIV disease progression are concerned. However, patients receiving zidovudine and THF showed a lower number of opportunistic complications. Only one patient in this group progressed to manifest AIDS while 9 of 18 controls presented disease progression. Four patients died in the case group, all of them were CDC stage IV at admission, and 15 of 26 died in the control group (all CDC stage IV at admission, and four patients who presented disease progression during the study period). Survival time was increased in the case group. The exact immunological effect of thymus hormones in HIV infection has still to be elucidated, but a possible therapeutic role of these agents is foreseeable.
