    Abstract:

    Objective To compare the sensitivity, specificity and efficiency between Mantoux and Interferon-gamma techniques, using secondary sources. Design Study of the published literature with a high evidence-based sources of scientific information. Data sources A bibliographic search in the Cochrane database and Trip-database. Study selection We found three Clinical Practice Guidelines and nine articles, which included one Systematic Review, one Meta-analysis, one article on the sensitivity of the new techniques, three cost-efficiency studies, and three prospective studies on the role of interferon-gamma release in the diagnosis of TB. Results The new techniques have a higher specificity than Mantoux for the diagnosis of LTBI in contacts vaccinated in childhood with BCG. The most cost-effective diagnostic strategy for LTBI was Mantoux screening and confirmation with QFT-G. Conclusion Resources should be coordinated, limiting its use in studies of contacts in immunocompetent adults vaccinated with BCG, with Mantoux>5mm or Tuberculin doubtful cases.

    Resumen Objetivo Comparar, utilizando fuentes secundarias, sensibilidad, especificidad y eficiencia, entre Mantoux y tecnicas de inmunoferon-γ. Diseno Estudio de revision bibliografica con fuentes de informacion de alta evidencia cientifica. Fuentes de datos Busqueda bibliografica en las bases de datos Cochrane y Trip Database. Estrategia de busqueda: «tuberculosis and (interferon or quantiferon or quantiferonTB) and diagnosis». Limites: publicado en los ultimos 5 anos, seres humanos, metaanalisis, guia de practica clinica, Ensayo aleatorio controlado. Se concluye la busqueda en octubre de 2009. Seleccion de los estudios Localizamos 3 guias de practica clinica y 9 articulos que incluyen una revision sistematica, un metaanalisis, un articulo sobre sensibilidad de las nuevas tecnicas, 3 estudios sobre coste-eficiencia y 3 estudios prospectivos sobre el papel de la liberacion de inmunoferon-γ en el diagnostico de tuberculosis. Resultados Las nuevas tecnicas presentan mayor especificidad que el Mantoux para diagnostico de la infeccion tuberculosa latente en contactos vacunados en infancia con BCG. La estrategia diagnostica mas coste-efectiva para la infeccion tuberculosa latente fue screening con Mantoux y confirmacion con QuantiFERON-Gold in tube. Conclusion Deberian coordinarse los recursos y limitar su uso en los estudios de contactos a adultos inmunocompetentes vacunados con BCG, con Mantoux superior a 5 mm o casos con tuberculina dudosa.

    Abstract:

    Se reporta el caso de un niño de 11 meses de edad con historia de sepsis generalizada por Pseudomona aeruginosa a los 5 meses, neutropenia, leucopenia, anemia asociada con la sepsis y desde entonces, infecciones respiratorias agudas recurrentes del tracto respiratorio superior, manifestaciones alérgicas y forunculosis por pseudomona. El estudio inmunológico realizado mostró cifra disminuida de IgG con ligero incremento de IgM y valores normales de IgA y de la población de células B CD19+, así como disminución de la subpoblación de células T cooperadoras CD4+. Se encontró, además, neutropenia y defecto marcado de la fagocitosis. Se estableció el diagnóstico de hipogammaglobulinemia congénita asociada con trastornos de los granulocitos. El paciente recibió tratamiento con gammaglobulina humana por vía intramuscular, factor de transferencia e inmunoferón, con mejoría clínica evidente.This is the case of a child aged 11 months with a history of systemic sepsis from Pseudomona aeruginosa at 5 months, neutropenia, leucopenia, sepsis-associated anemia and from then, recurrent acute respiratory infections of the high respiratory tract, allergic manifestations and furunculosis from pseudomona. Immunologic study conducted showed a decreased figure of IgG with a light increase of CD4+ cooperative-IgM of T cells. Also, we found the presence of neutropenia and marked defect of phagocytosis. We made the diagnosis of granulocyte-associate congenital hypogammaglobulinemia. The patient was treated with human gamma globulin by intramuscular route, transference factor and Immunoferon, with an obvious improvement

    Se reporta el caso de un niño de 6 años de edad con infecciones respiratorias y digestivas recurrentes desde los 8 meses de edad. El estudio inmunológico reveló marcada disminución de la activación linfocitaria, niveles disminuidos de subclases de IgG2, IgG3 e IgG4 y retardo de la fagocitosis. Se estableció el diagnóstico de inmunodeficiencia primaria combinada.     Abstract:

    Over the last decades extended medical knowledge has been an important health care benefit in terms of disease prevention and management. However, probably with no exception, most pharmaceutical products are not devoid of adverse consequences. Immunomodulators are commonly considered a “benign” drug whose advantages bypass consequences. The immunomodulator AM3 (Immunoferon®) is a clinically used, orally administered compound whose active principle is stabilised in an inorganic matrix of calcium. We report the misuse of AM3 in three members of a family; father and two children. The drug was prescribed to the father who subsequently administered it to the children without seeking medical advice. Two months later, all subjects developed abdominal and/or flank colicky pain. Hypercalciuria was diagnosed in the children with different degrees of severity. It is likely that the calcium content of the inorganic matrix played an important role in the onset of symptoms. No adverse side effects related to the inorganic matrix of calcium of Immunoferon® have been documented so far. This family case report calls attention to the risks of self -medication in a susceptible family. Paediatric patients are vulnerable as they rely on adults for the supply of medications. Concerning the use of drugs in family, especially nonprescription drugs, the quality of health care provided to the children depends on the health literacy of their parents

    Abstract:

    Inmunoferon® is a gljcoconjugate of natural origin, formed by the noncovalent association of a protein from Ricinus communis and a polysaccharidic moiety, and endowed with immunomodulatory as well as pharmacological activities. This study investigated the nature of the polypeptidic component of Inmunoferon. Through biochemical procedures and comparison with protein databases, the isolated protein was identified as the processed form of the seed of Ricinus communis 2S storage polypeptide, which has been termed RicC3. Further analysis of the isolated protein has revealed that it is composed of two different subunits, alpha and beta, which form an heterodimer of high stability and resistance to denaturation, acidic pH and proteolytic cleavage. These findings confirm the excellent properties of the product after oral administration and provide additional support for the pharmacological activities of Inmunoferon.

    Abstract:

    A new method for the labeling of polysaccharides by using tyrosine derivatives after CNBr activation is presented, along with its use for pharmacokinetic studies of an orally active immunomodulating noncovalent glycoconjugate, which was administered labeled either in the polysaccharide or the protein moiety. The relationship between the obtained results and the mechanism of action are also discussed.
