Estudo correlacional entre o desempenho em tarefas lingüísticas e audiológicas de indivíduos afásicos
Universidade Federal de Minas Gerais, 2007Co-Authors: Flaviana Gomes Da SilvaAbstract:Nessa dissertação realizou-se um estudo correlacional entre o desempenho em tarefas lingüísticas e tarefas audiológicas em indivíduos afásicos, além do estudo piloto em indivíduos normais. Este estudo foi motivado pela ausência de similares na literatura e pela importância que seus resultados adquirem quando interpretados à luz dos modelos de percepção da fala, que supõem a independência entre linguagem e audição. Os 11 indivíduos normais do estudo piloto de ambos os sexos, idade média de 26,9 anos, realizaram as tarefas de discriminação e segmentação de fonemas e tarefas para obtenção dos limiares de fusão auditiva e ordem temporal, apenas com o objetivo de averigüar se as tarefas teriam proporção de acerto próxima ao total. Os indivíduos afásicos foram submetidos às tarefas de compreensão auditiva, discriminação e segmentação de fonemas, além da avaliação audiológica composta por imitanciometria, audiometria tonal, logoaudiometria, otoemissão acústica, potencial evocado do tronco encefálico, tarefa para obtenção dos limiares de fusão auditiva e ordem temporal e o teste de detecção de gap no ruído branco. A amostra (n=16) foi selecionada dentre os pacientes atendidos no Centro Clínico de Fonoaudiologia da Pontifícia Universidade Católica de Minas Gerais, de ambos os sexos, média de idade 58,4 anos e com a compreensão auditiva preservada. Os resultados mostraram algumas correlações significativas entre a tarefa de discriminação fonêmica e os seguintes achados audiológicos: média das freqüências de 500, 1000 e 2000 Hz por via óssea e potencial evocado auditivo de tronco encefálico nas latências absolutas das ondas III e V. A tarefa de segmentação (quanto ao ponto e vozeamento) também correlacionou-se com as latências absolutas das ondas III e V. Os resultados do GIN encontraram-se aumentados, com média para a orelha direita de 9,4 ms e para orelha esquerda de 10,9 ms, acima dos 5 ms encontrados para sujeitos adultos normais na literatura. Em conclusao, foram encontrados resultados significativos relacionando BERA e tarefas de discriminação e de segmentação de sons da fala, o que remete à possível importância do tronco cerebral para a percepcao da fala. Por outro lado, a falta de correlação com as demais variáveis audiologicas investigadas apontam para uma abordagem em que a "fala é especial".This dissertation presents a correlational study of linguistic tasks in comparison to audiological findings in aphasic subjects, and a pilot study with normal subjects. The study was motivated by the absence of a similar study in the literature. Notwithstanding, its results could be important in the light of speech perception models, which assume independence between language and auditory capacity. In the pilot study, 11 normal subjects were submitted to the tasks of phoneme discrimination, phoneme segmentation, auditory fusion and temporal order with the aim of verifying if the hit results would be close to 100%. Aphasic subjects were submitted to the tasks of oral comprehension, phoneme discrimination and phoneme segmentation, and to a complete auditory assessment comprising Impedance Audiometry, tonal Audiometry using both air and bone conduction, verbal Audiometry, evoked acoustic otoemission, BERA, auditory fusion, temporal order, and gap detection in white noise. The sample (n=16) was selected from the patients under treatment at Centro Clínico de Fonoaudiologia / Pontifícia Universidade Católica de Minas Gerais, from both sexes and age mean 58,4 years without comprehension deficits. The results showed some significant correlations between phoneme discrimination and the following audiologic findings: mean of speech frequencies (500, 1000 and 2000 Hz) using bone conduction, and absolute latencies in wave III and V in evoked acoustic otoemission. Segmentation task (concerning place of articulation and voicing) was also correlated to absolute latencies in wave III and V in evoked acoustic otoemission. GIN scores were higher than found in normal population (5 ms): mean of 9,4 ms for right ear, and 10,9 ms for left ear. In conclusion, there were significant results relating BERA and phoneme segmentation and discrimination, and these point to the possible role of brain stem to speech perception. Study of eustachian tube function in normal adults and those with middle ear disease.
2014Co-Authors: Vijay Babu DAbstract:Eustachian tube dysfunction is widely recognised as the triggering factor of middle ear disease. Though it happens to be the important factor for various middle ear problems, Eustachian tube function tests are not routinely done. Eustachian tube dysfunction predisposes the ear for chronic middle ear disease and it is a cause for concern to ENT Surgeon as the success rate of surgery is less when performed in ear with impaired tubal function. Eustachian tube function’s role in successful outcome of surgeries for chronic otitis media has become a topic of interest in recent years. From our study it is evident that Eustachian tube dysfunction is an important factor that determines the outcome of chronic otitis media. In most patients with healed central perforation and in post operative patients who were successfully treated by surgery, the Eustachian tube function was found to be normal. ET function was found to be partially or grossly impaired in most patients with recurrent or residual CP. In most of the CSOM patients and patients with retracted ear drum also the impairment of Eustachian tube function was found to be statistically significant. Chronic sinusitis, allergic rhinitis and smoking were the important causes of ET dysfunction. Mucosal disease is the leading cause of ET dysfunction and a careful search for the underlying aetiology can be effective in directing specific treatment to resolve or mediate the ear sequelae. When medical treatment is inadequate, surgical intervention may be indicated. Tympanostomy tubes have a long proven record of efficacy but repeated insertion may lead to tympanic membrane pathology. Adenoidectomy may be effective, especially in children or in adults with significant hypertrophy compromising the tubal dilation. Eustachian tuboplasty is a new treatment to widen the tubal orifice and aid the valve in dilation when medical and conventional surgical procedures have proven inadequate. But the method of Eustachian tuboplasty is yet to be standardized. Further studies are needed before it finds universal acceptance. Eustachian tube function testing by Impedance Audiometry is a simple non-invasive method of testing the Eustachian tube function and helps in identifying the tubal dysfunction. Patients with tubal dysfunction should be evaluated for underlying cause and treatable causes should addressed before proceeding for surgery as it increases the success rate of tympanoplasty

The waves I-III and I-V interpeak latency intervals were significantly prolonged one year after radiotherapy. The study shows that radiotherapy for nasopharyngeal carcinoma impairs hearing by acting on the middle ear, the cochlea and the brain stem auditory pathway.
