We describe an HIV-Infected patient with 24 soft tissue infections and multiple colonization events. Molecular genotyping from 33 nonduplicate isolates showed all strains were USA300, Panton-Valentine leukocidin (PVL) and arginine catabolic mobile element (ACME) positive, and genetically related.

The Challenge of Hepatitis C in the HIV-Infected Person

ThomasAbstract:Hepatitis C virus (HCV) coinfection occurs in an estimated one quarter of HIV-Infected Persons in Europe, Australia, and the United States. As use of highly active antiretroviral drugs has markedly reduced opportunistic infections, HCV-related liver disease has emerged as a leading cause of death. HIV infection adversely affects both the natural history and the treatment of hepatitis C. Because there are no experimental models of coinfection and because the pathogenesis of each infection is incompletely understood, how HIV infection alters hepatitis C is not clear. This review considers the epidemiology, natural history, treatment, and pathogenesis of hepatitis C in HIV-Infected Persons.

Hepatitis C in the HIV-Infected Person

ThomasAbstract:Because of shared routes of transmission, hepatitis C virus (HCV) infection is common in HIV-Infected Persons, who have been experiencing increasing HCV-related morbidity and mortality since the advent of effective antiretroviral therapy. Infection with HIV appears to adversely affect the outcome of hepatitis C, leading to increased viral persistence after acute infection, higher levels of viremia, and accelerated progression of HCV-related liver disease. In addition, hepatitis C may affect the course and management of HIV infection. The medical management of hepatitis C in HIV-Infected Persons is complicated by immune suppression, potential drug interactions and toxicities, and other forms of liver disease. In addition, there is little published experience with the safety and efficacy of the best available anti-HCV medications in HIV-Infected Persons. Thus, current efforts must be directed at preventing HCV and HIV infections and applying the principles learned in treating Persons with either infection to manage those with both. Future efforts should include studies of the pathogenesis of HCV infection in HIV-Infected Persons and large, prospective studies that demonstrate the optimal management of Persons co-Infected with HIV and HCV. Such efforts will help to eliminate HCV-related liver disease as an emerging threat to HIV-Infected Persons.

Issues Concerning Survival of Viruses on Surfaces

The most common viral illnesses are produced by enteric and respiratory viruses. Transmission of these viruses from an Infected Person or animal to a new host can occur via several routes. Existing studies strongly suggest that contaminated fomites or surfaces play an important role in the spreading of viral diseases. The potential of viral spreading via contaminated surfaces depends particularly on the ability of the virus to maintain infectivity whilst it is in the environment. This is affected by a combination of biological, physical and chemical factors. This review summarises current knowledge about the influence of environmental factors on the survival and spread of viruses via contaminated surfaces.
