Nine-Year Analysis of Isolated Pathogens and Antibiotic Susceptibilities of Infectious Endophthalmitis from a Large Referral Eye Center in Southern China

Methods Data from all inpatients who were clinically diagnosed with Infectious Endophthalmitis and underwent microbiological evaluation at the Zhongshan Ophthalmic Center from January 2010 to December 2018 were collected retrospectively and analyzed according to different clinical etiologies. Results A total of 816 cases were collected in the study. Open-globe injuries had caused 473 (57.97%) cases, 70 (8.58%) cases presented Endophthalmitis after Infectious keratitis, 156 (19.12%) cases were postoperative, and endogenous causes accounted for 117 (14.34%) cases. Among the 309 culture-positive cases, the predominant pathogen for both postoperative and posttraumatic Endophthalmitis was gram-positive cocci (59.52% and 49.72%, respectively). Regarding keratitis-related Endophthalmitis, the main pathogens were filamentous fungi (57.58%) and gram-negative bacilli (30.30%). The pathogens of endogenous Endophthalmitis were almost evenly distributed among gram-positive cocci, gram-negative bacilli, and fungi. Eighty-five (10.42%) cases underwent evisceration/enucleation, including 42 cases secondary to keratitis-related Endophthalmitis. Overall bacteria showed high susceptibility to fluoroquinolones (75.36-100.00%). Gram-positive cocci showed much higher sensitivity to cephalosporins compared to gram-negative bacilli (85.11-92.59% vs 25.42-35.72%). For the five first-line antibiotics analyzed for time trend of susceptibility, four exhibited a significant decrease of susceptibility from 2010-2014 to 2015-2018. Conclusion Between 2010 and 2018, posttraumatic Endophthalmitis was the most common form of the treated Endophthalmitis in Zhongshan Ophthalmic Center. The causative pathogens varied according to different clinical settings. Even though the overall antibiotic susceptibilities were fairly high, we observed a substantial decrease of susceptibility for most first-line antibiotics.

Causative Microorganisms of Infectious Endophthalmitis: A 5-Year Retrospective Study.

A retrospective analysis was carried out on 330 patients with clinically diagnosed Infectious Endophthalmitis who underwent microbiological evaluation from January 2010 to December 2014. Of the 330 patients, 193 patients (58.5%) had posttraumatic Endophthalmitis, 67 patients (20.3%) had postoperative Endophthalmitis, 61 patients (18.5%) had endogenous Endophthalmitis, and 9 patients (2.7%) had postcorneal infective Endophthalmitis. Of the 105 cases (31.8%) of culture-positive Endophthalmitis, 79 cases (75.2%) had bacterial growth and 26 cases (24.8%) had fungal growth. In posttraumatic Endophthalmitis, Gram-positive bacteria were the predominant species, followed by Gram-negative bacteria and fungi. In endogenous Endophthalmitis, Gram-negative bacteria were the predominant species, followed by fungi and Gram-positive bacteria. In postsurgical Endophthalmitis, all infections were bacterial. However, in postcorneal infective Endophthalmitis, all infections were fungal. Overall, levofloxacin showed the highest activity against bacterial isolates. There was a significant difference in the susceptibility to tobramycin between the isolates from posttraumatic and postoperative Endophthalmitis (). The results of this study identify the microbial spectrum of Infectious Endophthalmitis in this clinical setting.

Infectious Endophthalmitis after Penetrating Injuries with Retained Intraocular Foreign Bodies

LiggettAbstract:Purpose: To determine the risk factors and prognostic indicators of Infectious Endophthalmitis in eyes with penetrating injury and retained intraocular foreign body. Methods: From the National Eye Trauma System (NETS) Registry, 492 eyes with intraocular foreign bodies were reviewed for signs of Infectious Endophthalmitis. Results: Thirty-four eyes (6.9%) with intraocular foreign bodies had evidence of Infectious Endophthalmitis, and 31 (91.2%) of those eyes had signs of infection at the time of removal of the intraocular foreign body. The majority of eyes with an intraocular foreign body with or without Endophthalmitis were in patients between 10 and 39 years of age, but the risk of Endophthalmitis developing increased with age, especially in patients 50 years of age or older with delayed primary repair ( P = 0.005). Endophthalmitis was more likely to develop in eyes with home or occupational injuries (33/358, 9.2%) than in those with injuries from other settings (1/128, 0.8%; P = 0.001). Infectious Endophthalmitis was much less likely to develop in eyes with primary repair within 24 hours of the injury (10/287 = 3.5%) than in eyes with primary repair more than 24 hours after the injury (22/164, 13.4%; P Conclusions: Removal of a retained intraocular foreign body within 24 hours of injury markedly reduces the risk of Infectious Endophthalmitis developing. Older persons are at high risk for Endophthalmitis developing after retaining an intraocular foreign body when there is delayed surgical repair.

Detection of Torque Teno Virus (TTV) and TTV-Like Minivirus in patients with presumed Infectious Endophthalmitis in India.
PloS one, 2020Co-Authors: Poonam Naik, Vivek Pravin Dave, Joveeta JosephAbstract:Human anelloviruses (Torque Teno Virus (TTV) and TTV Like Mini Virus (TLMV)) are now being reported at a high prevalence across the world, with a controversial disease-inducing potential. The aim of this study was to investigate the role of these anellovirus in vitreous of patients with presumed Infectious Endophthalmitis. After informed consent, vitreous fluid from patients with Endophthalmitis (n = 103) and non-Infectious pathologies (n = 102) were analyzed for the presence of TTV and TLMV DNA by qPCR with the limit of quantification defined as 100 copies per reaction. Among the patients clinically diagnosed with Endophthalmitis, 29 of the 40 culture proven samples (72.5%) and 42 out of 63 (66.6%) of culture-negative samples were positive for presence of TTV/TLMV. Interestingly, 51 of the 102 (50%) samples in the control group were also positive for TTV/TLMV. Comparing the clinical outcome among patients diagnosed with Endophthalmitis, we observed no significant association in the final visual acuity of patients who were positive for presence of TTV/TLMV, however, these patients had significantly higher repeat antibiotic injections (p = 0.03). Further evidence is however needed to correlate TTV / TLMV with a particular pathology or group of pathologies in the eye.

Elevated cytokine levels in vitreous as biomarkers of disease severity in Infectious Endophthalmitis.

Materials and methods Vitreous samples of 46 patients clinically diagnosed as Endophthalmitis (of which 25 were culture positive) and 20 non-Infectious controls from patients with Retinal Detachment (RD) or diabetic retinopathy were included in the study. The cytokine and chemokine expression patterns of 40 immune mediators including 6 antiinflammatory cytokines, 15 proinflammatory cytokines, 9 Growth factors and 10 proinflammatory chemokines in the vitreous were were analyzed by multiplex cytokine immunoassay. In addition, significant immune mediators were correlated with initial and final visual acuity (VA). Results Our results demonstrated elevated expression of 16 mediators such as GCSF, GRO, IFN-γ, IL-1α, IL-1β, IL-1 RA, IL-6, IL-8, IP-10, MCP-1, MCP-3, MIP-1α, IL-1β, TGF-α, TNF-α in patients with culture positive Endophthalmitis. Cytokine profile expression significantly differed between patients with proven Endophthalmitis and the non-Infectious controls in heat map analysis. PCoA plot indicated five mediators (IL-1RA, IL-6, IL-8, GRO, G-CSF) as biomarkers that could be Independent Predictors of Disease especially in culture negative cases. Correlation of cytokines with VA revealed strong association between the initial VA and intraocular levels of TGF-α, IL-1β and IL-8 but there was no correlation with the severity or visual outcome of infection. Conclusion In comparison to non-Infectious ocular conditions, the pathogenesis of Infectious Endophthalmitis correlates with increased expression levels of IL-1RA, IL-6, IL-8, GRO, G-CSF. Understanding cytokine profiles in culture negative Endophthalmitis patients could aid in therapy in non-responders to empirical antibiotic therapy.
