    IDENTIFIKASI DAN UJI SENSITIFITAS BAKTERI YANG DIISOLASI DARI SPUTUM PENDERITA PNEUMONIA DI RSUP PROF. DR. R. D. KANDOU-MANADO TERHADAP ANTIBIOTIK AMPISILLIN, CEFIXIME DAN SIPROFLOKSASIN Renalda Febriany Patty1), Fatimawali1), Defny Silvia Wewengkang1) 1)Program Studi Farmasi Fakultas MIPA UNSRAT Manado ABSTRACT Pneumonia is a global health issues with high rate mortality. This research was aimed to determine the type of bacteria and the sensitivity of bacteria isolated and identified from sputum pneumonia patients in the department of RSUP Prof. Dr. R D Kandou Manado against antibiotics ampicillin, cefixime and ciprofloxacin. This research was used sputum sample that was previously performed Instrument Sterilization using and autoclave. Inoculated media that was incubated at temperature of 35-36 oC for 24 hours were isolated on agar slat. Identified covered by gram staining, biochemical test, sensitivity test of bacteria to antibiotics, the planting discs, inhibition zone measurement and data analysis. The results shows that there are a 7 types of bacteria causing the infection is Staphylococcus Sp., Ewingella americana, Clostridium Sp., Escherichia vulneris, Enterobacter Spp., Klebsiella Spp. and Aminobacter. The antibiotics with highest sensitivity showed by ciprofloxacin (44,5%) against Staphylococcus, Ewingella americana, Escherichia vulneris, Enterobacter Spp., Aminobacter, and intermediate (22,2%) against Escherichia vulneris, Clostridium Sp., and resistant (33,3%) against Clostridium Sp., Klebsiella Spp., Aminobacter. The highest resistence were showed by ampicillin and cefixime, (94,44%) against Staphylococcus Sp., Ewingella americana, Clostridium Sp., Escherichia vulneris, Enterobacter Spp., Aminobacter, and intermediate (5,56%) against bacteria Ewingella americana, without any sensitivity to antibiotics. Tujuan penelitian ini untuk menentukan jenis bakteri dan tingkat kepekaan bakteri yang diisolasi dan diidentifikasi dari sputum penderita pneumonia di RSUP Prof. Dr. R. D. Kandou Manado terhadap antibiotik ampisilin, cefixime dan siprofloksasin. Penelitian ini menggunakan sampel sputum yang sebelumnya telah dilakukan sterilisasi menggunakan autoklaf. Inokulasi media yang telah diinkubasi ±24 jam pada suhu 35-36 oC dan diisolasi pada media agar miring. Identifikasi meliputi pewarnaan gram, uji biokimia, uji kepekaan bakteri terhadap antibiotik, penanaman cakram, pengukuran zona hambat dan analisis data. Hasil penelitian ini menunjukkan bahwa Terdapat 7 jenis bakteri penyebab infeksi, yaitu Staphylococcus Sp., Ewingella americana, Clostridium Sp., Escherichia vulneris, Enterobacter Spp., Klebsiella Spp. dan Aminobacter. Dan antibiotik dengan sensitifitas/kepekaan yang tertinggi ditunjukkan oleh Siprofloksasin sebesar 44,5% pada bakteri Staphylococcus Sp., Ewingella americana, Escherichia vulneris, Enterobacter Spp., Aminobacter dan intermediet sebesar 22,2% pada bakteri Escherichia vulneris, Clostridium Sp. dan resisten sebesar 33,3% pada bakteri Clostridium Sp., Klebsiella Spp., Aminobacter.     Background In 2005, of the approximately 4 million births in the United States, 30% were by cesarean section (C-section) delivery, which translates to roughly over 1 million C-sections in 2005 alone. C-section is associated with higher morbidity than vaginal delivery. Women who undergo C-section are 5 times more likely to develop a postpartum infection after delivery than women who undergo vaginal delivery. Objective Estimates of surgical site infection (SSI) after C-section range from 1.50 to 2.64. A quality improvement initiative was implemented at the University of Minnesota Medical School to reduce rates of SSI using changes based on recommended care initiatives. Methods The multidisciplinary team developed a comprehensive staff education and training program, added a preoperative skin preparation protocol using chlorhexidine gluconate (CHG) no-rinse cloths, added CHG with alcohol for interoperative skin preparation, and modified Instrument Sterilization techniques. Results Data analysis revealed a statistically significant reduction in the overall SSI rate from 7.5% (33/441) in January-July 2006 to 1.2% (5/436) in January-July 2007 (χ 2 test statistic, 21.2; P Conclusion Interventions, including staff education, use of CHG no-rinse cloths for preoperative skin prep, CHG with alcohol for intraoperative skin prep, and appropriate Instrument Sterilization management led to reductions in SSI rates in patients undergoing C-section at our institution. Rates of endometritis were also noted to be lower after implementation of the interventions.

    Abstract:

    The Sterilization of dental Instruments is an area of great interest and recent concern in the field of dentistry. The purpose of this study was to compare the ability of three lasers (argon, CO2, and NdYAG) to sterilize dental Instruments. Endodontic reamers were contaminated with microorganisms, lased at various levels of energy, placed in Trypticase soy broth, incubated, and read for growth or no growth to determine sterility. Results indicated that the argon laser is capable of sterilizing selected dental Instruments at the lowest energy level (1 watt for 120 seconds) of the three lasers tested. The other two lasers were able to sterilize the Instruments also, but at higher energy levels. Results indicated all three lasers capable of sterilizing selected dental Instruments; however, the argon laser was able to do so consistently at the lowest energy level of 1 watt for 120 seconds.

    Peracetic acid is recognized as a powerful germicidal agent. However, heretofore its corrosive nature has prevented its direct use for medical Instrument Sterilization. A specially formulated buffered peracetic acid sterilant with anticorrosives has been developed for use in the STERIS PROCESS. The STERIS SYSTEM 1 Processor and the STERIS 20 Sterilant Concentrate have met or exceed the rigorous requirements of the Environmental Protection Agency and Food and Drug Administration for making claims related to Sterilization. With this system, medical devices including endoscopes are sterilized and ready for use in less than 30 min.

    Medical services influence the people’s health and lives. It is a sacred and professional work with the features that do not allow errors and cannot be redone. Therefore, the quality of medical services directly affects the health and safety of patients. In the operating room, the central supply room is responsible for sterilizing surgical Instruments. It is also an important place for infection control. Once incompletely sterilized medical devices are used, it may lead to nosocomial infections, thus affecting the safety and quality of the surgical patients. Therefore, to ensure the integrity of the Sterilization process and to avoid the use of unsafe surgical Instruments. This study uses the sensing technology of the Internet of Things to monitor the Sterilization process of surgical Instruments Sterilization with using the automatic monitoring system to alarm the validity period of surgical Instruments pack, and to calculate the required safety stock of surgical Instruments packing. With the system, it reduces the additional cost of emergency Sterilization when surgical Instruments packs are out of supply. Ensure the safety of the patient’s use of surgical Instruments, so that patients can be treated in a safer medical environment to achieve a win-win benefit.
