outcome of Intestine Fistula patients treated with home enteral nutrition before definitive operation
Parenteral & Enteral Nutrition, 2008Co-Authors: L I JieshouAbstract:Objective:The purpose of this study was to assess the outcome of Intestine Fistula patients treated with home enteral nutrition(HEN) support before definitive operation.Methods:Fifty-eighty patients were divided into HEN support group and hospital enteral nutrition support group when their illness were stable and total enteral nutrition(TEN) were full resumed.All the patients were supervised by the members of nutrition support team.Days of TEN and hospital stay and medical cost was recorded.Plasma protein,body weight and body mass index were measured on the first day of full enteral nutriton and the day enteral nutrition was stop for operation preparation.Results:Nutritional status of the patients improved.There was no significant difference between two groups in nutritional status,complications and duration of TEN.The time patients in HEN group stayed at home was(61.11±27.71) d and in hospital(60.79±17.11) d.The cost in HEN group was less than that in hospital EN support group(140.21±61.2 vs 189.2±72.2 thousand Chinese dollar).Conclusion:Under the close supervise of nutrition support team,HEN is of the same safety and effect and can reduce the time of hospital-stay and medical cost when compared hospital EN.

the diagnostic value of endoscopy in gallbladder Intestine Fistula
Chinese Journal of Digestive Endoscopy, 2005Co-Authors: Huang JiefeAbstract:Objective To study on the effectiveness of endoscopy in diagnosing the gallbladder-Intestine Fistula. Methods The clinical datum of 24 patients with gallbladder-Intestine Fistula from January 1982 to March 2004 were reviewed and analyzed retrospectively. All patients were examined by endoscopy and confirmed from operation. Results Twenty-one out of 24 patients were finally diagnosed by endoscopy while 3 of them were misdiagnosed. Other examinations among the 24 patients included B ultrasound 11 , computer tomography (CT) 5, and abdominal plain films 7; none of them was finally diagnosed by these measures. Two out of 8 patients were finally diagnosed by barium meal or enema roentgenogram. Conclusion Endoscopy was an important method in diagnosing the gallbladder-Intestine Fistula. The diagnostic level would be improved if the specific characters of endoscopies in this entity were fully recognized.

The Japanese Journal of Gastroenterological Surgery, 1999Co-Authors: Takami Fukui, Naoki Yokoo, Takahiro Yoshida, Chihiro Tanaka, Tatsushi Kato, Hisaya Azuma, Takashi Shiroko, Tetsuya Yamaguchi, Kiyonao OkamotoAbstract:イレウスを契機に発見された, 多発小腸瘻と異所性胃粘膜を伴う重複腸管の1例を経験したので報告する. 症例は71歳の女性. イレウス症状にて, 当科紹介受診. 腹部は膨満し, 軽度の圧痛, 筋性防御を認めた. 腹部単純X線およびCTにて, 高度に拡張した小腸ガス像を認めたことより小腸イレウスと診断, 同日, 緊急開腹術を施行した. 手術所見はトライツ靭帯より約130cm肛門側の小腸間に1か所の瘻孔形成を認めた. また, 回盲部より約100cm口側に高度に拡張した小腸が癒着, 同部位口側に近接して重複腸管を認めた. さらに高度に拡張した小腸間に計3か所の瘻孔形成を認めた. トライツ靭帯より約130cm肛門側の瘻孔部分は瘻孔切除にとどめ, 重複腸管は拡張腸管とともに切除し手術を終了した. 病理組織所見では小腸本管に異所性胃幽門腺を認めた. 本症例は, 胃所性胃粘膜より瘻孔が形成されたことが, イレウスの一因となった極めてまれな症例と考えられた.
