    Abstract:

    Objective To study the clinical efficacy and safety of propofol and remifentanil total Intravenous Anesthesia in elderly gastroscopy.Methods 200 cases ASA grade II~III patients with the elder gastroscopy for the study were randomly divided into control group of 100 cases and observation group of 100 cases.Observation group house later,throat spray surface Anesthesia,application propofol-remifentanil total Intravenous Anesthesia for gastroscopy;The control group did not use any medicine,throat spray to the surface after Anesthesia to be checked.Results The observation group and the control group in the process of gastroscopy the symptoms,upper gastrointestinal peristalsis level,blood pressure,heart rate and other indicators of changes in the VAS score and comfort and has great significant difference(P0.01).Conclusion Propofol-remifentanil total Intravenous Anesthesia in elderly Gastroscopy,effectively reduced the stress response to maintain hemodynamic stability.

    Background: The majority of anesthetic agents, including inhalation anesthetics, inhibit platelet function, but the effect of propofol on platelets is controversial. This study was designed to compare the effects of propofol-remifentanil total Intravenous Anesthesia and sevoflurane inhalation Anesthesia on platelet function. Methods: Patients undergoing major hip surgery were divided into two groups: inhalation Anesthesia with sevoflurane or total Intravenous Anesthesia with propofol and remifentanil. Hemoglobin, hematocrit, platelet count, prothrombin time, activated partial thromboplastin time, and platelet aggregation were measured 5 minutes before induction, the first hour intra-operatively, and the first hour postoperatively. Results: Total Intravenous Anesthesia with propofol and remifentanil significantly diminished collagen-induced platelet aggregation values intraoperatively compared with preoperative values. Also, intraoperative collagen- and adenosine diphosphate (ADP)-induced platelet aggregation was significantly lower in Anesthesia with propofol and remifentanil compared with sevoflurane Anesthesia. Sevoflurane did not induce significant changes in collagen- or ADP-induced platelet aggregation. Conclusions: Total Intravenous Anesthesia with propofol and remifentanil significantly reduced collagen-induced platelet aggregation intraoperatively, while inhalation Anesthesia with sevoflurane did not reduce collagen-or ADP-induced platelet aggregation at all.     Abstract:

    Objective:To investigate the effects of remifentanil and propofol total Intravenous Anesthesia on tracheal intubation and hemodynamics,and to observe the characteristics of postoperative recovery on pediatric patients undergoing adenotonsillectomy.Methods:40 pediatric patients(5~10 years old,ASAI) were randomly divided into 2 groups:group R(remifentanil+ propofol total Intravenous Anesthesia),group F (fentanyl+propofol total Intravenous Anesthesia),each group included 20 cases.The variation of blood pressure,heart rate,oxygen hemoglobin saturation(SpO_2) were observed at four time points:5 minutes before operation(T1),just after tracheal intubation(T2),the time when cutting out the gland(T3) and just after operation(T4).The duration and the characteristics of postoperative recovery were investigated too.Results:In group R,compared to T1,blood pressures at T2,T3 and heart rates at T2 and T3 were decreased(P0.05).In group F,blood pressures at T3,heart rates at T3 and T4 were higher than T1 (P0.05).The recovery duration in group R was(5.6±2.3) min,which was faster than group F(14.2±5.7) min.There was no fidgetiness during resuscifation time in group R,but the postoperative pain was more obvious.Conclusion:On pediatric patients undergoing adenotonsillectomy,remifentanil and propofol total Intravenous Anesthesia could efficiently restrain harmful stress.They could also maintain hemodynamic stability,the recovery duration and quality were better too.This kind of Anesthesia method was ideal during pediatric adenotonsillectomy.
