    Zusammenfassung Aufgrund von zahlreichen Faktoren wie Effizienzsteigerung, Kostendruck, Personalmangel, Datensicherheit stehen die Krankenhauser in Deutschland vor grosen Herausforderungen. Die Abrechnung von Fallpauschalen bei gleichzeitig erwarteter optimaler medizinischer Versorgung der Patienten erschwert den Alltag der medizinisch Verantwortlichen und der Kostenverantwortlichen. Auch der Wettbewerbsdruck zwischen den Kliniken steigt weiter. Hierbei dreht sich alles um hochste Qualitat bei der Behandlung und Patientenversorgung, um bestmoglichen Service bis hin zur Gesamt-Attraktivitat der Klinik fur die Patienten, um die sie wirbt. Auserdem gilt es, im Klinikalltag umfassende Hygienevorschriften einzuhalten und diese sowie andere Prozesse umfanglich zu dokumentieren, dies besonders in der aktuellen Corona-Pandemie. Um diesen Anforderungen gerecht zu werden, setzen immer mehr Krankenhauser auf die Moglichkeiten des Lean Managements. Durch die Konzentration auf Wertschopfung mithilfe von digitalen Technologien konnen Ablaufe in Kliniken und Einrichtungen des Gesundheitswesens deutlich verbessert werden. Virtuelle und digitale Plattformen mit Anbindung von Tracking- bzw. Lokalisierungstechnologien, mit digitaler Prozesssteuerung und Sensorik-Elementen bergen groses Optimierungspotenzial fur Kliniken. Obwohl die Lean-Management-Methode ursprunglich entwickelt wurde, um die Qualitat und Produktivitat von Automobilfabriken zu verbessern, wird sie mit grosem Erfolg in Branchen und Umgebungen aller Art eingesetzt, einschlieslich Softwareentwicklung, Organisationen der offentlichen Hand, Einzelhandel und anderen Servicestellen. Insbesondere Organisationen des Gesundheitswesens haben festgestellt, dass der Ansatz verwendet werden kann, um Kosten zu senken und gleichzeitig die Qualitat und Patientenzufriedenheit zu verbessern. Eines der Grundprinzipien von Lean ist die Beseitigung von Abfall, der als alles definiert wird, was dem Kunden keinen Mehrwert bringt. Die Praktizierenden zielen auf acht bestimmte Arten von Abfallen ab (ursprunglich waren es sieben – mehr davon nachfolgend). Sie sind im Gesundheitswesen genauso verbreitet wie in der Fertigung. Lean Management zielt daher auf die Beseitigung von Wartezeiten, eine effizienten Behandlung und Ubermedikation ab, wie in Abb. 18.1 (Gupta 2013) dargestellt. Daruber hinaus fokussiert sich das schlanke Krankenhaus auf optimale Ablaufe und die richtige Qualifikation der Mitarbeiter.

    Abstract:

    Lean Management ist ein modernes Konzept zur Prozessoptimierung von Wertschopfungsketten und Prozessen mit dem obersten Ziel der Kundenzufriedenheit (Helmold 2020). Lean Management konzentriert sich darauf, aus Sicht des eigenen Unternehmens (der Produktion) Verschwendungen und Ineffizienzen transparent zu machen und diese in wertschopfende Aktivitaten umzuwandeln (Ohno 1990; Helmold und Terry 2016). Die Wertschopfungskette reicht in diesem Zusammenhang von der Lieferantenseite (Engl.: Upstream) uber die eigenen Operationen bis hin zur Kundenseite (Engl.: Downstream) (Slack et al. 1995).

    Unter VeranderungsManagement (Jap.: 変更管理, Henkō kanri) lassen sich alle Aufgaben, Masnahmen und Tatigkeiten zusammenfassen, die eine umfassende, bereichsubergreifende und inhaltlich weitreichende Veranderung zur Umsetzung neuer Strategien, Strukturen, Systeme, Prozesse oder Verhaltensweisen in einer Organisation bewirken sollen. AnderungsManagement (Engl.: Change Management) kann als die Summe von Aufgaben, Masnahmen und Aktivitaten definiert werden, die eine umfassende, abteilungsubergreifende und weitreichende Veranderung in einem Unternehmen oder einer Organisation bewirken sollen. Das AnderungsManagement umfasst die Implementierung einer neuen Mission, Vision, Strategien, Strukturen, Systeme, Prozesse und Verhaltensweisen in einer Organisation. Das ultimative Ziel des Wandels ist es, durch den Wandel zu einem schlanken Unternehmen eine langfristig gunstige Position auf dem Markt zu erreichen und einen nachhaltigen Wettbewerbsvorteil zu erzielen (Helmold 2020). In der Literatur gefundene Synonyme fur den Terminus AnderungsManagement sind Business Process Reengineering, Turnaround Management, TransformationsManagement, Change Management, InnovationsManagement oder Total Quality Management (Vahs 2019). Veranderungen bestimmen zunehmend die alltaglichen Geschafte und Aktivitaten von Unternehmen. Um Veranderungen optimal zu managen, sind spezielle Change-Management-Techniken erforderlich, die unter dem Begriff AnderungsManagement (Lauer 2019; 2020) zusammengefasst werden konnen. Der menschliche Faktor steht bei allen Uberlegungen im Vordergrund, da die Umsetzung von Veranderungen von der aktiven Unterstutzung der Mitarbeiter abhangt. Da jeder seine eigenen Bedurfnisse, Ideen und Erfahrungen hat, von denen einige nicht den offiziellen Vorgaben der Unternehmensorganisation entsprechen, kann es kein einfaches Rezept fur den erfolgreichen Umgang mit Veranderungen geben. Es ist vielmehr ein komplexer Prozess, der an drei Punkten beginnen muss: der Organisation und den betroffenen Personen, den Unternehmensstrukturen und der Unternehmenskultur (Lauer 2019). Ein weiteres wichtiges Element im Kontext ist der technologische Faktor, einschlieslich Systeme, Routinen, Methoden und Instrumente (Helmold und Samara 2019). Abb. 22.1 fasst die Elemente des AnderungsManagements zusammen. AnderungsManagement ist ein holistischer Ansatz und fangt mit der Unternehmensstrategie, Mission, Vision, Unternehmens- und den Abteilungszielen an.     Seit mehreren Jahrzehnten setzen Hersteller schlanke Prinzipien und Methoden ein, um die betriebliche Komplexitat zu reduzieren und die Produktivitat zu verbessern. Der Lean-Ansatz bildet die Grundlage fur operative Exzellenz, indem Prozesse standardisiert, eine Kultur der kontinuierlichen Verbesserung eingefuhrt und Mitarbeiter in der Werkstatt gefordert werden (Ohno 1990). Angesichts der zunehmenden Komplexitat der Ablaufe haben viele Unternehmen jedoch festgestellt, dass Lean Management allein nicht ausreicht, um ihre betrieblichen Herausforderungen zu bewaltigen. In jungster Zeit wurde eine Reihe fortschrittlicher digitaler Technologien entwickelt, die als Industrie 4.0 bekannt sind und neue Ansatze fur den Umgang mit Komplexitat und die Verbesserung der Produktivitat bieten. Durch den Einsatz der richtigen Technologiekombination konnen Hersteller Geschwindigkeit, Effizienz und Koordination steigern und sogar den selbst verwaltenden Fabrikbetrieb vereinfachen. Hersteller, die ihre Ablaufe optimieren mochten, mussen das Zusammenspiel zwischen traditionellem Lean Management und Industrie 4.0 verstehen. Wir haben in den vergangenen Jahren Hunderte von Operational-Excellence-Programmen unterstutzt und festgestellt, dass Unternehmen wertvolle Synergien erzielen, indem sie Lean Management und Industrie 4.0 ganzheitlich und nicht unabhangig oder nacheinander implementieren. In den meisten Fallen ist die integrierte Anwendung von Lean Management und Industrie 4.0, die wir als Lean Industrie 4.0 bezeichnen, der effektivste Weg, um die nachste Stufe der operativen Exzellenz zu erreichen. Innovative Technologien fordern die Transparenz, Vernetzung und Automatisierung von Geschaftsprozessen im Kontext von Industrie 4.0 und bieten Unternehmen der Fertigungsindustrie ein groses Potenzial. Dies wirft die Frage auf, wie diese neuen Technologien mit den oft bereits etablierten Lean-Management-Prinzipien zur Prozessoptimierung und Komplexitatsreduzierung zusammengefuhrt werden konnen. Eine aktuelle Studie der Unternehmensberatung Bearingpoint zeigt, dass der erfolgreiche Einsatz von Lean Management 4.0 je nach Branche sehr unterschiedlich ist (Bearingpoint 2019).

Prior research suggests that Lean Management and supply Management are potentially important determinants of environmental performance and can be seen as capabilities that ease the adoption of environmental practices. To help understand the roles of Lean and supply Management in regards to improving the firm’s environmental performance, a conceptual model proposes that the magnitude of environmental practices mediates the relationship between Lean and supply Management with environmental performance. To test the model, plant-level survey data from a sample of Canadian manufacturing plants is used. The results indicate that supply Management and Lean activities provide means by which resources are invested in environmental practices. The empirical analysis also confirms that the impact of Lean Management, and to a lesser extent supply Management, on environmental performance is mediated by environmental practices.

    Industrial improvement approaches such as Lean Management are increasingly being adopted in health care. Synthesis is necessary to ensure these approaches are evidence based and requires operationalization of concepts to ensure all relevant studies are included. This article outlines the process utilized to develop an operational definition of Lean in health care. The literature search, screening, data extraction, and data synthesis processes followed the recommendations outlined by the Cochrane Collaboration. Development of the operational definition utilized the methods prescribed by Kinsman et al. and Wieland et al. This involved extracting characteristics of Lean, synthesizing similar components to establish an operational definition, applying this definition, and updating the definition to address shortcomings. We identified two defining characteristics of Lean health-care Management: (1) Lean philosophy, consisting of Lean principles and continuous improvement, and (2) Lean activities, which include Lean assessment activities and Lean improvement activities. The resulting operational definition requires that an organization or subunit of an organization had integrated Lean philosophy into the organization’s mandate, guidelines, or policies and utilized at least one Lean assessment activity or Lean improvement activity. This operational definition of Lean Management in health care will act as an objective screening criterion for our systematic review. To our knowledge, this is the first evidence-based operational definition of Lean Management in health care.

    Abstract This study investigates the determining factors that drive sustainable performance through the application of Lean methods in the primary production segment of the horticultural supply chain for apples and pears. The determining factors, identified through a systematic review of the available literature, are thematically synthesized, conceptually framed and utilized for the development of a case study. The single case study approach is utilized to develop a detailed and nuanced understanding of the context, evaluating the practices of 4 cooperative primary producers operating within a forward integrated supply chain. The study posits that the combination of climatic and biophysical dynamism inherent in the primary producer environment, in combination with the inflexibility of seasonal batch production, imposes itself as a key barrier to the imposition of pull and flow in the chain, the fundamental tenets of a Lean system. A case is outlined where cold infrastructure is employed to break the inflexibility of supply whilst a process of forward contracting establishes fruit orders up to 1-year in advance, beyond the forthcoming annual cultivation cycle, functionally transforming the system of cultivation from a “push” to a “pull” system of production. It is further highlighted that functional partitioning of the organizational-chain structure is necessary to isolate and mitigate the effects of contextual dynamism, whereby downstream chain structures purposed for agility and responsiveness serve as a protective buffer to Lean focused grower operations. The findings reaffirm the positive relationship between the size of the grower operation, the capacitation of the workforce and the ability of the operation to attain superior performance outcomes. However, it is severally highlighted that horizontal cooperation between primary producers may help overcome the resource limitations of smaller growers. Data based decision controls are marked as being a centrally important sustainable performance determinant, both at the level of the grower, in terms of orchard Management and harvest process control, as well as at the level of the cooperative serving the needs of crop programming and practice benchmarking processes. This exposition of determining factors driving Lean sustainable performance in horticultural primary production represents a new contribution to the body of literature linking Lean and sustainable organizational performance. The study should support further development of Lean Management research and operationalized Lean methods within the fruit horticulture subsector as well as the broader agri-food context.
